Performance Work Statement 36C24819Q0688
Bed Bug Eradication and Treatment
A. GENERAL INFORMATION
1. Title of Project – Bed Bug Eradication, Treatment and Preventative Maintenance, Training and Emergency Services for Veterans living/working/visiting at the following two (2) leased facilities.
• Domiciliary – 4700 N. Habana Ave, 5th & 6th Floors Tampa, Florida 33614
• Primary Care Annex (PCA) Clinic – 13515 Lake Terrace Ln Tampa, Florida 33637
B. GENERAL REQUIREMENTS
The contractor shall provide all service, transportation, personnel, labor, supplies, materials, and supervision required for inspection, preventative maintenance, training, and eradication of Bed Bugs within the above-mentioned locations.
1. Domiciliary – 4700 N. Habana Ave, 5th & 6th Floors Tampa, Florida 33614 Approximate square footage of the Domiciliary is 10,000 square feet.
2. Primary Care Annex (PCA) Clinic – 13515 Lake Terrace Ln Tampa, Florida 33637 The approximate square footage of the Primary Care Annex, Clinic, is 117,037 square feet.
a) Approximate square footage of the Outpatient Pharmacy is 9,423 square foot.
b) Approximate square footage of the Dermatology Clinic is 6,241 square feet.
c) Approximate square footage of the Audiology Clinic is 10,000 square feet
d) Approximate square footage of the Eye Clinic is 20,378 square feet
e) Approximate square footage of the Mental Health is 15,172 square feet
f) Approximate square footage of Compensation & Pension is 10,000 square feet
g) Approximate square footage of the Brooksville Clinic is 10,000 square feet
h) Approximate square footage of the South Hillsborough CBOC is 50,000 square feet
i) Approximate square footage of Zephyr Hills COBC is 6,455 square feet
j) Approximate square footage of Lecanto CBOC is 17,083 square feet
The contractor shall provide Emergency services for Bed Bug treatment and eradication to the following areas on an on-call basis (12 Calls Maximum a Year):
• Outpatient Pharmacy – 12210 Bruce B Downs Blvd, Tampa, FL 33612
• Dermatology Clinic – 12210 Bruce B Downs Blvd, Tampa, FL 33617
• Audiology Clinic – 14020 N 46th St, Tampa, FL 33613
• Eye Clinic - 10770 N 46th St, Tampa, FL 33617
• Mental Health - E, 10770 N 46th St, Tampa, FL 33617
• Compensation & Pension - 10770 N 46th St, Tampa, FL 33617
• Brooksville Clinic - 14540 Cortez Blvd #108, Brooksville, FL 34613
• South Hillsborough VAMC – 12920 Summerfield Crossing Blvd, Riverview, FL 33578
• Zephyrhills Hills Clinic – 6937 Medical View Ln, Zephyrhills, FL 33542
• Lacanto Clinic – 2804 W Marc Knighton Ct #A Lecanto, FL 34461
3. Contractor must have capacity to eradicate bed bug infestation by means of convection heat treatment method
a. Must be able to furnish and operate Heat Unit(s) and fan(s) equipment to raise room temperature above 120-degree Fahrenheit and hold temperatures at that rate for minimum of five hours
b. Must be able to provide same day heat treatment within four (4) hours of notification of positive bed bug sighting. Notification will be given during normal business hours 8:00AM – 4:30PM, Monday - Friday.
c. Must be able to treat a minimum of 2,000 sq ft in same day service
C. EMERGENCY SERVICE.
1. Emergency Treatments: A technician representative shall respond by telephone to the
VA within five (5) hours of the original service call.
2. In the event of an outbreak or emergency, the contractor will provide eradication response for both the Domiciliary and the PCA within twenty-four (24) hours.
3. Eradication of bed bugs shall include furniture, office spaces, and closets and so on.
4. Eradiation shall be billed per square foot of area treated for removal of pests.
5. There will be a minimum of a 24-hour response via phone or physical representation once the contractor received notification from the COR.
6. In the event of weekend, after hours, or holidays; contractor shall respond the proceeding work day.
7. The contractor shall not perform any services without the approval of the Contracting
Officer Representative (COR) .
D. COMPLIANCE/ REGULATIONS:
1. Contractor must obey all Environmental Protection Agency (EPA) regulations.
2. Every pesticide bought, sold, or used in the United States must, by law, be registered by the U.S. EPA and in each of the sates. EPA approves not only the product itself, but also each separate use for which it is intended, and the product label. Contractor is responsible for applying only those pesticides that are registered.
3. Contractor must obey all Department of Transportation (DOT) regulations.
4. Shipment of pesticides and other dangerous substances across state lines is regulated by the federal DOT.
5. Contractor must obey all Occupational Safety and Health Administration (OSHA) regulations the contractor will conduct inspections and apply preventative treatments as needed during each visit in all accessible rooms including but not limited to, offices, laundry, conference areas, reception areas, kitchen, bathrooms, sleeping rooms/areas, and any other place where bed-bugs may reside looking for evidence of bed bugs. The process should consist of A MINIMUM of -room inspection with as-needed preventative treatments during each inspection. Inspections should include monitoring devices in each room.
• Contractor shall provide bi-weekly inspection, preventative maintenance treatment and training to government employees for the Domiciliary (5th & 6th Floors). NOTE:
This is the current initial requirement.
• Contractor shall provide quarterly inspection, preventative maintenance treatment and training to government employees for the Primary Care Annex and Domiciliary. All offerors should incorporate the cost for twenty-five (10) Emergency Bed Bug calls of which could be used at any location listed above. All prospective offerors should price their services to include this future requirement as it will be incorporated into the contract for service through written modification by the Contracting Officer (CO).
E. CONTRACTOR EXPERIENCE REQUIREMENTS:
• Each respondent must have an established business, with an office and full-time staff to include a "fully qualified Pest Control staff” "Fully qualified" is based upon training and on experience in the field. The staff must have successfully completed a training program of the Pest Control services and must have a minimum of two years’ experience with respect to bed bug remediation.
• All work shall be performed by "fully qualified" competent Staff.
• The Contractor shall provide written assurance to the competency of their personnel and a list of credentials of approved Staff for services at the VA.
• The Contracting Officer (CO) may authenticate the training requirements, request training certificates, or credentials from the Contractor at any time for any personnel who are servicing or installing any VA equipment.
• The CO and/or Contracting Officer's Representative (COR) specifically reserves the right to reject any of the Contractor's personnel and refuse them permission to work on the VA equipment.
• If subcontractors are used, the CO must approve them in advance.
• The Contractor shall submit any proposed change in subcontractor(s) to the CO for approval/disapproval.
• The contractor should provide basic knowledge/education/training about bed bugs to
Government staff employees. The contractor shall provide one copy of the Material
Safety Data Sheet (MSDS) to the Contract Officer Representative (COR) within 15 days after contract award commencement for all chemical products to be used.
• Contractor will conduct pest control services in accordance with Rule Chapter 5E-2 of the Florida Administrative Code & Florida Administrative Register (web address below). In addition, contractor will supply a list to the COR of all certified employees who will be on the premises along with a copy of their certification(s) no later than ten
(10) days after contract award.
These licenses and certifications shall be in compliance with Florida Statute Title
XXXII, Chapter 482 (See below web addresses).

F. CONTRACTOR’S PERSONNEL:
• A) Contractor Point of Contact: The contractor shall provide the name and contact number of a point of contact and an alternate that shall be responsible for the performance of services no later than three (3) days after award.
• B) Contractor Employees: Contractor’s personnel shall present and carry themselves in a neat and professional manner while on Facility Campus. She/he shall comply with all policy and regulations of this facility.
• C) Identification: Contractor personnel will wear proper I.D. badge at all times while on property. This will be accomplished by coordinating with the COR.
• D) Position Sensitivity – The position sensitivity has been designated as Low Risk - SAC is not required.
G. SITE VISIT:
Site visit is not required for this requirement. In no event shall failure to inspect the site constitute grounds for a claim after contract award.
H. CONTRACT AWARD MEETING:
The contractor shall not commence performance of the tasks in this SOW until the CO has conducted a kick-off meeting or has advised the contractor that a kick-off meeting is waived.
I. LABOR RATES:
Contractor shall provide hourly rates equivalent to the Department of Labor rates for Pest Controller(s) as prescribed in the current wage determination number WD 15-4571 (Rev.-7) was first posted on www.wdol.gov on 01/01/2019 posted on www.wdol.gov for Hillsborough County.
J. WORK HOURS:
Contractor employees will check in with on–site supervisor or designee upon arrival and will perform the duties between 8AM and 4:30PM Monday through Friday.
Contractor’s service schedule for both the Domiciliary, PCA, Outpatient Pharmacy, Dermatology, Audiology & Homeless Program and Eye Clinic will be coordinated through the
COR.
If services fall on a Federal Holiday, the Contractor will make arrangements with the COR or Alternate points of contact to perform the services on an alternate day of the week.

Note: This includes any other day declared by the President of the United States to be a national holiday.
Any of the above holidays falling on Saturday will be observed on the preceding Friday; holidays falling on Sunday will be observed on the following Monday. Contractor’s days to perform are reflected in the Performance Work Statement/Statement of Work.
K. SERVICE INVOICES:
Contractor will submit a service invoice for each service visit that has been completed to the COR or Designee. The invoices will include at a minimum the following information: - Date and Time of Service - Areas Serviced
- Square footage serviced - List of what (if anything) is found - What spray/product/chemical was used to mitigate the issue.
L. CHANGES TO THE STATEMENT OF WORK (SOW):
Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer (CO). A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.

Domiciliary and the PCA within twenty-four (24) hours.

N. SAVE HARMLESS AND INDEMNITY AGREEMENT:
The contractor shall save harmless and shall indemnify this hospital with respect to any and all liability, claims, costs of whatsoever kind or nature for injury to or death of any person or persons and for loss or damage to any property of the hospital or of others occurring in connection with this contract, resulting in whole or in part from the negligent or non-negligent acts or omissions of the contractor, any subcontractor, or any employee, agent or representative of the contractor or of any subcontractor.
O. CERTIFICATES:
1. EPA Identification number.
2. Copies of relevant licenses, permits and certifications
3. Certification of Medical Surveillance Program.
4. Copies of relevant training.
P. POTENTIAL REQUIREMENT GROWTH:
This requirement is for services in Network Contracting Office 8 (NCO8). The initial requirement may only include this station and the surrounding areas as stated in the, PWS or the solicitation. However, the government reserves the right to increase the requirement area to include two (2) or more stations in the NCO8 or VISN 8 Area. The Government reserves the right to include services at a later date at additional locations within the VISN as required to meet the government’s needs. Additional locations may include VA facilities in and around; Bay Pines, Gainesville and North Florida/South GA, Miami, Orlando, Tampa, West Palm Beach FL, and San Juan, PR.
In accordance with Federal Acquisition Regulation (FAR 52.212-4) any changes in the terms and conditions of this contract may be made only by written agreement of the Contractor and Government. The prices for services at additional locations will be based on the initial requirement prices and final pricing will be determined upon supplemental agreement between the Contractor and the Government. Any increased requirements are subject to the availability of funds.
S. COMPLIANCE:
The Contractor shall be fully responsible for compliance with all Veterans Administration (VA), local, state, and Federal environmental/occupational safety laws, rules and regulations. The Contractor shall follow life and safety codes and take necessary actions to avoid conditions that may be hazardous to the health and safety of hospital personnel and patients. The Contractor is responsible for meeting all applicable OSHA and VA guidelines for safe working processes.
All work for the duration of the contract shall be performed by certified technicians fully trained and experienced to work in the field of automatic door installation and repair.
T. ACCESSABILITY AND SECURITY:
Contractor shall keep in mind and, to the extent applicable, follow practices that ensure the security and safety of JAH Veterans Hospital facilities, equipment and personnel.
To this end, accessibility to all work areas will be provided by the COR. No keys will be issued to Contractor employees.
U. CONTRACTOR EMPLOYEES:
Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. Example: wearing of distinctive clothing such as uniform, badges, patches, etc.
V. NOISE AND DISRUPTION:
The contractor is required to continuously consider the impact to the medical center in completing the required work. Any operation causing excessive noise or vibration shall be coordinated in advance with the COR. Also, movement of equipment, supplies, and waste through occupied areas, corridors, and elevators must be planned and scheduled with the COR to minimize disruption to hospital operations.
Cellular phones are acceptable inside the building, except for posted critical care areas. Contractor personnel are expected to use appropriate and courteous language.
Obscene language is not permitted
W. ESCORT AND JAMES A. HALEY SECURITY POLICE CHECK-IN:
Contractor personnel shall check in and out with VA Police dispatch located in the front entrance of the Main Hospital. Access to work areas and performance monitoring will be provided by the COR.
X. SECURITY AND CHECK-IN REQUIREMENTS:
All work shall be scheduled through the Electrical shop or maintenance supervisor.
The contractor shall check in with the VA police dispatch upon arrival at the facility to obtain proper identification prior to initiating work. The contractor shall contact the electrical shop or maintenance supervisor at the provided phone numbers to notify them they are on-station, and also prior to leaving the station the contractor shall contact them to report observations on the equipment and product quality.
Y. PARKING:
It is the responsibility of contractor personnel to park only in designated parking areas.
Parking information is available from the VA police and COR. The VA will not invalidate or provide reimbursement for parking violations of the contractor’s personnel under any circumstances.
Z. SMOKING:
Smoking is not permitted within or around VA Healthcare System facilities, except within designated areas.
