ATTACHMENT A

STATEMENT OF WORK

NF/SG VA Health System – Pharmacy Service Courier Services

1. Scope of Work - The Contractor shall provide courier services to pick-up and deliver emergency medications for the NF/SG VA Health System – Pharmacy Service, on an on-call/emergent basis throughout the NF/SG area. Service is point to point with no deviations in route and medications should be kept inside a refrigerated compartment.

It is important that the courier service be established in the Gainesville area, due to the need of courier services 24/7 with no more than a 1 to 2-hour response time. If “stat” is ordered, it would be immediate – (15 to 30 minutes maximum).

In addition to emergent deliveries, the contractor will make scheduled deliveries of flu vaccine in the Fall of each year to Ocala, Lecanto, Palatka, Perry, Marianna, Valdosta, Waycross, St. Marys and St. Augustine VA community care outpatient clinics. As this is a refrigerated item, the delivery must be accomplished within three hours of departure from VA Medical Center. The schedule is variable but must be able to meet to provide adequate service.

Throughout the year requests are made to transport drugs in a frozen state to VA clinic facilities of Ocala, Lecanto, Palatka, Marianna, St Mary’s, the Villages and St. Augustine. This delivery must be as expeditious as possible to ensure the drug is maintained in frozen status.

It is important that the drivers be familiar with the local areas as some VA clinics can be difficult to find. They may be positioned to not face the access road and time can be lost looking for the clinic. Drivers must know to call the VA for help to minimize waste time being lost.

Medication cannot be left in unattended vehicle, i.e. stop for lunch, or no air conditioning on a hot day.

2. Place of Performance – Medications are transported primarily to/from the VA Medical Centers (Gainesville and Lake City), the Outpatient Clinics (Tallahassee, Jacksonville and the Villages) and the Community Based Outpatient Clinics (Marianna, Perry, Waycross, St. Mary’s, Palatka, St. Augustine, Ocala and Valdosta) as well as, to/from other Department of Veterans Affairs facilities and neighboring medical facilities located in the NF/SG area (travel within +/- 150 miles on short notice).

3. Hours of Operations - The contractor is expected to provide courier services on an "as needed" basis, with pick-ups and deliveries between the hours of 8:00AM and 5:00PM Monday thru Friday, after normal business hours and days between the hours of 5:00 pm and 8:00 am, and 24 hours weekends and all Federal holidays. In general, the longest trip is approximately 2.5 hours (each way). The operations at the Department of Veterans Affairs are considered essential services, and as such will still be operational when other Government facilities are closed due to hazardous weather conditions.

4. Qualifications - All couriers transporting medications under this contract must:

4.1 Be licensed, bonded or insured.

4.2 Have the ability to maintain (at the contractors' expense) communications with the courier dispatching facility while in transit. Must be able to contact courier by phone immediately, (No recordings, call backs, etc).

4.3 Shall present a neat appearance in the performance of their duties, and be easily recognized as a contract employee/courier (with identification prominently displayed or readily available).

4.4 Be able to work under pressure and to expedite special pick-up/delivery jobs on occasion (requiring rush service of less than one (1) hour).

4.5 Successfully complete the required Government Background Investigation.

5 Delivery Orders -The contractor telephonically receives work requests with specific instructions (e.g., what is to be transported; when and where to pick-up the item; when delivery is due) from VA Medical Center personnel.

5.1 All requests shall receive a tracking number. Status of shipments shall be available timely through telephonic or electronic means.

5.2 Deliveries are made in all weather conditions (with additional delivery time allowed for bad weather). The contractor must provide notice to VA Medical Center personnel (upon initial contact) if pick-up/delivery cannot be made to allow the VA Medical Center to pursue other delivery alternatives.

6 Equipment - The contractor shall provide the means (i.e., personnel; vehicles; equipment) to travel and transport Medication throughout the NF/SG VHS area. Approximate weight/size of the Emergency Drugs about 30” x 20” – approx 10 lbs. down to Medication that could fit in a bag. Medications that are temperature controlled and require dry ice – contractor will provide. NF/SG VHS can provide the Styrofoam and ice packs.

7 Security -The Medication (with value ranging from a few dollars to several thousand) being delivered must be protected from loss and damage due to inclement weather, accidents or other unforeseen circumstances. Failure to deliver timely, and loss or damages to transported materials shall be reported to the VA Medical Center point of contact as soon as possible. Medications are usually sealed in the container that they are in. Medications are not locked but put in a small bag that is stapled or a box that is sealed or Styrofoam that is usually packed for temperature control. The receiving clinic usually asks for a certain number of the medication.

8 Reporting Procedures - The contractor shall have the capability to provide status information on all courier deliveries by tracking number. This information may be provided telephonically or electronically to the SEC. The following information shall be available for each delivery: tracking number, time of pickup and delivery, location of pickup and delivery, and signature of acceptance, if required.

9 Invoices - The contractor shall invoice the VA on a monthly basis, and shall itemize each trip/item by tracking number, date and cost.
