Statement of Work Cubical Curtains & Mesh Malcom Randall and Lake City VA Medical Centers

SUMMARY OF WORK AND REQUIREMENTS:

Introduction Cubical Curtains and mesh for Malcom Randall VA Medical Center located in Gainesville, Florida and Lake City VA Medical Center located in Lake City, Florida are required. Cubical Curtains are requested to provide veteran patients and families privacy they deserve. HIPPA Patient privacy to our patients and guests is essential to good patient outcomes. Cubical Curtains and mesh need to meet or exceed criteria of Fire Ratings, and clean ability for healthcare environments and infection control.

Description/Scope/Objective The contractor shall provide product as requested for Malcom Randall VA Medical Center in Gainesville, Florida and Lake City VA Medical Center in Lake City, Florida. Warehouse truck docks are available for separate deliveries to be pre-scheduled with Project Manager, Installation teams, VA Interior Design, and VA warehouse on site. Facilities are located at 1601 SW Archer Rd; Gainesville, FL 32608 and 619 S. Marion Ave, Lake City, FL 32025. Materials and products requested are specifically designed for healthcare environments.

Performance Requirements Contractor shall supply, inspect, deliver, install, service and repair any and all cubical curtains, mesh and accessories for entire area mentioned in this SOW for the Gainesville and Lake City Facilities in support of the Government for proper performance of health care facilities.

Scope Provide FR rated cubical curtains, removable mesh, and installation to existing track for all items requested at the Gainesville and Lake City Facilities. Removal of all debris to include packing materials, boxes etc. will be the responsibility of chosen vendor(s) and is a necessary part of the scope of this project.

If specifying paint finish, indicate paint system and color number.(i.e. Chroma, DC15 – Scout) Show locations of accessory panels on Drawings. Add all applicable materials used in the centerline glass frame that is NOT glass (i.e. 3form, wood veneer over substrate, upholstered panel over substrate, etc.).

Performance Location and Hours

Government will provide detailed areas prior to performance. Primary place of performance is Malcom Randall VA Medical Facility located at 1601 SW Archer Rd; Gainesville, FL 32608; and Lake City VA Medical Center, 619 S. Marion Ave., Lake City, FL 32025. Specific areas may be prioritized upon request by NF/SG interior designer. Deliveries on multiple days will be pre-arranged with Warehouse Manager and Interior Designer of NF/SG

Government normal duty hours are from 0800 – 1700, Monday through Friday, excluding Federal holidays. Some areas of installation however shall need to be done after normal business hours in order to complete project.

All work shall be scheduled with Interior Design and warehouse at Gainesville and Lake City facilities for deliveries to site location as specified. Work to include receipt, inspection and delivery of specified furnishings in SOW.

Federal Holidays observed by the VA:

New Year’s Day Martin Luther King Day President’s Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day

Deliverables

The contractor shall provide a working record of all furniture and installation and submit a report of work accomplished to the Government.

All furniture shall be fully inspected, received, delivered and installed as required within the specified dates of solicitation upon contract award.

Brochures and Physical Samples of products maybe requested.
