52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)
Addendum 52.212-2 Evaluation Factors
The Government will award a firm fixed priced contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The contract will be awarded to the Lowest Priced Technically Acceptable (LPTA) Offeror. The Technical and Past Performance evaluation will be a determination based on information furnished by the offeror. The Government is not responsible for locating or securing any information which is not identified in the offer. Technical Ability and Past performance is rated as acceptable or unacceptable; additionally, Past performance will include the neutral rating if the offeror has no relevant past performance:+

The following factors, will be considered in the evaluation of proposals received under this solicitation.
FACTOR 1: TECHNICAL ABILITY– The Offeror's proposal shall disclose the technical ability in as much detail as possible, including, but not limited to, the requirements in this section of the RFQ. The proposal shall address the questions below. The technical ability should be specific, detailed and complete enough to clearly and fully demonstrate that the offeror thoroughly understands the intent of the Performance Work Statement. Stating that; the offeror understands and shall comply with the performance Work Statement, or paraphrasing the Performance Work Statement or parts thereof, is considered inadequate.
0. Explain your company’s physical and operational security standards for your vehicles and your plant.
0. Explain your company’s control of resources policies. What are your employee standards? What are your equipment standards?
0. Submit a copy of liability insurance.
0. If a subcontractor is utilized, include a letter identifying the anticipated percentages for prime and subcontractor performance. The contractor must be in compliance with the limitations on subcontracting in accordance with FAR clause 52.219-14.
3. Proof of NAID certified, bonded, and insured contractor FACTOR 2: PAST PERFORMANCE- The past performance evaluation will assess the relative risks associated with an Offeror’s likelihood of success in fulfilling the solicitation’s requirements as indicated by the Offeror’s record of past performance. Past performance will be evaluated based on past performance questionnaires’ and any other past performance data available.
The Past Performance Questionnaire included in the solicitation is provided for the offeror or its team members to submit to the client for same/similar project (maximum of 3). Ensure correct phone numbers and email addresses are provided for the client point of contact. Completed Past Performance Questionnaires are to be mailed, faxed, or preferably scanned/emailed from the client directly to shirley.courtney@va.gov, cannot be submitted via the offeror. For the client evaluation to be considered, the Past Performance Questionnaires must be submitted no later than the quote due date and time February 21, 2018 (12:00 PM EST).
The Government will consider any data available in the Past Performance Information Retrieval System (PPIRS). In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available, that Offeror shall not be evaluated favorably or unfavorably on Past Performance (a neutral rating will be given).
FACTOR 3: TOTAL EVALUATED PRICE - All pricing will be evaluated on a total price basis to include option years of the contract. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced or if the overall price quoted is unreasonable. Evaluation of options shall not obligate the Government to exercise the option(s).
