X This amendment accomplishes the following:
1. Adds the approximate square footage and acreage to the SOW. There's approximately 5 acres of lawn and open space, flower beds and shrubs which equates to roughly 217,800 square feet.
Statement of Work Weed Control and Ant Treatment
1. Description of Work: Tuscaloosa VAMC, Engineering Service has a need for a complete weed control and ant treatment/eradication facility-wide. The services to be provided by the contractor under this agreement shall consist of labor, personnel, equipment and materials to complete total weed control (three times per year) and ant treatment (twice per year) on the Tuscaloosa VA campus (approximately 5 acres and approximately 217,800 square feet). Areas include but not limited to, flower beds, grass areas, sidewalks, children’s playground area (bldg. 40), all building perimeters, and all parking lots (a site map is included for the areas needing pesticide and the areas herbicide). Contractor must have a Chemical Applicator License/Certification and proper Personal Protective Equipment (PPE).
Contractor must perform work in accordance with state, federal, OSHA and EPA guidelines. Contractor must follow all safety precautions and protocol as noted above and possess the following certifications: Chemical Applicator License/Certification.
2. Period of Performance: Base Plus Four Option Years (Dates will be determined at award.
3. Type of Contract: Firm Fixed Price
4. Location of Services: Tuscaloosa VA Medical Center, 3701 Loop Road East, Tuscaloosa, AL 35404
5. Hours of Work: Work may be accomplished during the hours of 6:00 am to 3:30pm, Monday through Friday.
6. Physical Security: The Contractor shall be responsible for safeguarding all government equipment, information and property provided to the Contractor Personnel while performing the services described.
7. Invoicing: Invoice/s are to be submitted electronically; in arrears of the start date of the service that were performed. The Department of Veterans Affairs utilizes a third-party invoicing system provider (Tungsten Network) for handling electronic invoices.
8. Work Stoppage/Delays: The Contracting Officer shall be notified at the earliest time available, but, no later than the next business day, via phone of the Contractor’s knowledge of any conditions which may adversely affect the Contractor’s ability to perform under the terms and conditions as stated in the specifications of this requirement. For example, vehicular problems, licenses, or anything that might jeopardize the terms and conditions of the contract should be reported to the Contracting Officer immediately.
9. Service Requirements: The Contractor shall be responsible for providing the personnel for all components of the evaluation for the Department of Veterans Affairs Tuscaloosa VA Medical Center. Upon completion, results must be documented in a fashion acceptable to the Department of Veterans Affairs, to include Safety Data Sheets (SDS). Contractor must follow all safety precautions and protocol for this remediation and possess certifications as listed above in Description of Work. A copy of the certifications listed above should be presented to the COR prior to accomplishing work.
10. Contractor Qualifications: Contractor must submit a copy of their Chemical Applicator License/Certification.
11. Basis of Award: Award will be made to the Lowest Price Technically Acceptable (LPTA) contractor meeting the contractor qualifications stated above.
