DEPARTMENT OF VETERANS AFFAIRS

Justification for Limiting Sources IAW FAR 13.102 for

Brand Name Acquisitions Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Title: Replace Flooring with Shaw Brand Luxury Vinyl Tile in Multiple Areas

RFQ: 36C24719Q0811

1. Contracting Activity: Department of Veterans Affairs, Regional Procurement Office 07, NCO 7, Georgia Services, Carl Vinson VA Medical Center

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Environmental Department require services to remove and dispose of the existing carpeting and 4” or 6” cove base and install new Shaw Quiet Cover Floating Luxury Vinyl Tile (LVT) and 5.25” millwork wall base in approximately 9,000 square feet of hospital space in various areas of the facility, as detailed in the Statement of Work and the Attached Diagram. Areas include Pharmacy in Building 5, IT work areas in Building 40, and Engineering Offices in Building 29. Linear perimeter of millwork is approximately 1,150 linear feet. The contractor shall provide all equipment, parts, labor, and services necessary to complete the project at Carl Vinson VAMC, 1826 Veterans Blvd, Dublin, GA.

3. Unique characteristics that limit availability to only one source (or one specific brand name), with the reason no other supplies or services can be used:

The Shaw Quiet Cover 0186V High Performance Luxury Vinyl Tile with In-Step Locking System, 7” x 48”, 00720 Mink and 00750 Timber is requested to ensure standardization of LVT flooring throughout the facility. The specific tile and colors are currently in other areas of the facility and will continue to be used as standardization is required for maintenance stock and ease of repair.

4. Description of market research conducted and results or statement why it was not conducted:

Market research was completed on LVT flooring options available on the market, and this product was selected based on product integrity, quality, wide availability, and existing use at facility. SOW was sent to multiple vendors (three) and all vendors contacted were able to provide pricing utilizing specified flooring, Shaw Quiet Cover 0186V High Performance Luxury Vinyl Tile.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.102. I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
