Statement of Work
Preventive Maintenance for JLG Lift, Forklifts and Scissor Lift
1. REQUIREMENT:
Requirement is for preventive maintenance service for JLG Lift, Two (2) forklifts, and one (1) Scissor Lift in service at WJB Dorn VAMC, Columbia, SC.
2. BACKGROUND:
The JLG lift, Model 600AJ, Sellick Forklift, Model 3634-1-131-2475XR, John Deere Forklift Model JD482C-2155, and Scissor Lift are in service and need a preventive maintenance service contract for routine maintenance and repairs as necessary. Having a preventive maintenance service contract in place would result in a more cost-effective way to ensure the units remain operational and well maintained. Having a preventive maintenance service contract in place ensures optimum equipment performance, increases equipment up-time, protects the investment and helps to lengthen the useful life of the equipment. In addition, a service representative can often catch potential issues and fix them before the issue becomes more costly.
3. PERIOD OF PERFORMANCE:
Service shall be performed on the equipment during normal operating hours (Monday – Friday, 0700-1630, except federal holidays and weekends) This is a Base Year Plus Four Option Years Requirement.
4. PLACE OF PERFORMANCE:
The place of performance shall be WJB Dorn VA Medical Center, 6439 Garners Ferry Road, Columbia, SC 29209. When not being used in operation, the JLG Lift, Forklifts and Scissor Lift are parked in an area adjacent to Building 22.
5. PERFORMANCE REQUIREMENTS:
Contractor shall replace /repair the following categories of items.
Audio warning items (horns, sirens, back-up alarms) Visual warning items (lights, strobes, flashers, etc.)
Operator restraint systems (seat belts, harnesses, etc.)
Furnish the following planned Maintenance service for lift trucks John Deere serial number T0482CM7300016 Sellick serial number 9072404
Internal combustion powered equipment.
1. Blow out radiator core and clean machine with compressed air
1. Clean grease fittings and lubricate.
1. Fill clutch through bearing grease cup.
1. Change engine oil and filter element.
1. Check transmission and differential oil level, fill if necessary.
1. Check air cleaner, inspect hose and connections.
1. Check upright assembly for proper adjustment and lubricate, if applicable.
1. Check battery and clean terminals
1. Check hydraulic sump filter cap; clean and replace if necessary.
1. Check hydraulic oil level; fill if necessary.
1. Check and lubricate all connecting shafts and levers.
1. Check steering gear box level; fill if necessary.
1. Check power steering oil level; fill if necessary
1. Check parking brake for proper operation.
1. Check all visual warning items.
1. Check all operator restraint systems.
1. Check all audible warning items.
1. Check power steering for proper adjustment.
1. Check fuel oil, water systems for leaks. Check radiator for proper coolant level.
1. Inspect brake pedal linkage for proper adjustment.
1. Inspect clutch/inching pedal and linkage for proper adjustment.
1. Check master cylinder; fill if necessary.
1. Check engine vent pipes and pvc valve.
1. Check all accessory drive belts for proper tension.
1. Check distributor points for adjustment and condition.
1. Inspect spark plugs and ignition wiring.
1. Inspect lift and tilt cylinders for leaks.
1. Check tilt cylinder rod ends and anchors; adjust and tighten if necessary.
1. Check tires for cuts, wear, or deformations.
1. Operational check and physical inspection of machine.
Aerial work platforms
1. Clean machine
1. Clean grease fittings and lubricate
1. Check boom/scissor arms for cracks/deformations and lubricate as necessary.
1. Check hydraulic oil level; fill if necessary.
1. Check all visual warning items.
1. Check all operator restraint systems.
1. Check all audible warning items.
1. Check battery and clean terminals.
1. Check differential oil level; fill if necessary.
1. Inspect lift and tilt cylinder for leaks.
1. Inspect brake pedal linkage for proper adjustment.
1. Check battery cables and receptacles.
1. Check contractor assembly and all switches for proper operation.
1. Operational check and physical inspection of machine.
1. Check motor brushes.
1. Check tires for cuts, wear, or deformations.
Contractor shall deliver the Government a maintenance checklist advising of any remedial work performed by it at the time of inspection and recommending to owner of any repairs that need to be done for units to remain operational.
