PERFORMANCE BASED WORK STATEMENT

Inspection and Testing of Fire Protection Equipment

1. GENERAL:

The Contractor shall provide all plant, labor, equipment, materials, supervision, necessary to perform annual fire alarm testing and certification at the Veterans Health Administration, Ralph H. Johnson, Medical Center in Charleston, South Carolina, Fisher House in Charleston, SC, and the Myrtle Beach Primary Care Clinic, in Myrtle Beach, SC.

2. BACKGROUND:

The RHJ VAMC is a 133-bed, facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located in Charleston, South Carolina and provides primary, secondary, and some tertiary care. The hospital is approx. 500,000 square feet, six stories high. The Fisher House is in Charleston, South Carolina, it is approx.. 45,000 square feet and three stories high. The Fisher House provides housing for families of veterans during the long term hospitalization period. The Myrtle Beach Primary Care Clinic is in Myrtle Beach, South Carolina, and provides outpatient services to veterans. The Myrtle Beach Clinic is approx. 12,000 square feet and a one story building. There are approximately 40 valve tampers ad 2 flow switches located under the main hospital building, which is a non-permit confined space, however requires physical agility for the technicians to test the valves (some crawling, and some climbing over pipes to access the tampers).

4. PLACE OF PERFORMANCE:

5. SCOPE:

a. The Contractor shall provide all labor, equipment, materials, and supervision, to inspect and test the following fire alarm/sprinkler devices, at the Ralph H. Johnson VA Medical Center (main hospital): Supervisory pressure (pre-action)(1); Air pressure switch (2); tamper switches (96); waterflow switches (53); duct detectors (57); heat detectors (43); pull stations (194); smoke detectors (396); horn/strobe (34); releasing devices (50); speaker (278); speaker/strobe (420); strobes (78); communicator/dialer (1); battery (91); fire pump (2); kitchen hood (2); power supply (36); emergency power off/relay (1); elevator (6); fire dept. connection (5); fire hydrant (4); standpipe (1); main drain test. Inspection of both wet/dry sprinkler systems. Perform 6 year fire damper inspection, IAW NFPA 80. Perform 5 year stand pipe flow testing. Perform 5 year internal inspections. Perform fire pump flow test and provide fire pump curve.

b. The Contractor shall provide all labor, equipment, materials, and supervision, to inspect and test the following fire alarm/sprinkler devices at the Fisher House: heat detectors (3); pull station (14); smoke detectors (43); supervisory devices (3); misc. devices (5); fire dept. connection (1); main drain test (1); wet system test; tampers/riser test. Perform 6 year fire damper inspection, IAW NFPA 80. Perform 5 year stand pipe flow testing. Perform 5 year internal inspections. Perform fire pump flow test and provide fire pump curve.

c. The Contractor shall provide all labor, equipment, materials, and supervision, to inspect and test the following fire alarm/sprinkler devices at the Myrtle Beach Primary Care Clinic: Tamper switch (1); waterflow switch (1); duct detector (5); pull station (7); smoke detector (18); horn/strobe (18); strobe (10); communicator (1); control panel (1); annunciator (1); battery (2); fire dept. connection (1). Perform 6 year fire damper inspection, IAW NFPA 80. Perform 5 year stand pipe flow testing. Perform 5 year internal inspections. Perform fire pump flow test and provide fire pump curve.

d. There must be a minimum of two NFPA certified technicians onsite during testing. (1) to reset the system after a device is tested and one (1) to test the device.

e. The Contractor shall provide test and certify all devices associated with systems and provide documentation of inspections in accordance with current applicable NFPA (72)(25), Joint Commission (life safety), VA, and OSHA requirements.

f. The Contractor shall provide a detailed report documenting test results and findings by location and in VHA/JACHO compliant format.

g. The contractor shall contact Michael Tucker, Occupational Health and Safety Specialist, to schedule inspection, testing, and certification work.

h. During the progress of the work and upon completion, all work shall be subject to inspection by the Contracting Officer and the COTR or the designated representatives.

i. No work is to be performed on the site on Saturdays, Sundays or legal holidays unless otherwise specified and the COTR is duly notified in writing no less than forty-eight hours in advance of the contemplated work and written approval for such is obtained. The following are National Legal Holidays:

6. CONTRACT PERFORMANCE MONITORING:

The government reserves the right to monitor services in accordance with Performance Based Matrix.

7. INVOICES:

Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor, validated by the Contracting Officer’s Technical Representative (COTR), and submitted by appropriate method (online billing) to VA FSC, P. O. BOX 149971, AUSTIN, TX 78714.

A properly prepared invoice will contain:

· Invoice Number and Date

· Contractor’s Name and Address

· Accurate Purchase Order Number

· Supply or Service provided

· Total Amount Due
