B.3 PERFORMANCE WORK STATEMENT (PWS)

(1) General: The Contractor will provide all transportation, material hardware and parts, tools, specialized equipment, labor, supervision, and technical expertise to provide Document Shredding Service at Charles George VAMC Asheville NC, VA Franklin Outpatient clinic at 647 Wayah St., Franklin NC, HBPC clinic 212 Wayah St. Franklin NC, Outpatient clinic 374 Charlotte Road, Rutherfordton NC. Contractor will abide by all HIPPA privacy rules to include federal, state, and local regulations at VAMC Asheville. This contract does not include the procurement of associated equipment for service.

(2) Coordination: Environmental Management Services will serve as the focal point and Contraction Office Representative (COR) for this contract.

(2.1) The Contractor must pre –coordinate work and if needed, associated outages with the COR. This work will be accomplished during regular work hours. The contractor will respond to Building 47, Room BF132 on the prescheduled date/time, and the COR. Contact must be made with the COR prior to accomplishing the starting any work.

(2.2) The COR will be immediately notified whenever the document shredding, or related activities interfere with normal VAMC operation.

(3) Specific Requirements: The Contractors work shall include, but not be limited to:

· The contractor shall provide on-site shredding, pick up and emptying of containers for Asheville VMAC, CBOC’s and Franklin HBPC, complying with all directives.

· Contractor shall develop a Document Shredding Plan for approval. The plan will require approval by the Chief of Environmental Management Services prior to implementation and is subject to the contracting officer’s approval.

· The contractors Document Shredding Plan shall establish the strategy and methods for conducting a safe, effective, and environmentally sound Shredding program in accordance with all HIPPA privacy rules, Federal, State and Local regulations.

· Contractor shall provide Certificate of Destruction upon completion of on-site shredding. Contractor shall also provide COR with monthly poundage report of recyclables (all material shall be recycled).

· Contractor personnel are required to interrupt their work at any time so as not to interfere with the normal functioning of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment, and carts. Contractor will perform services every Wednesday for the Asheville VAMC.

· Contractors are required to provide license to Government prior to start of work.

· Contractors are required to have Bodily Injury Liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence.

· Contractors are required to have Property Damage Liability insurance coverage of at least $500,000.

· In the event of inclement weather, the entire work shall be rescheduled to a date and time satisfactory to the contractor and the Chief, Environmental Management Services. Postponement will not reflect any penalties assessed to the contractor, nor shall there be any additional cost to the Government.

· NOTE: Work is being completed in a hospital environment. The Contractor must take all precautions to ensure all tools and materials are under continuous observation and accountability. At no time will tools be left unattended or unsecured.

· When complete, notify the COR to inspect and accept all accomplished work.

· Ensure all invoicing documents are submitted in-accordance with the instructions provided in the formal contract.

(4) Scope of Work:

· Provide bulk purging on site shredding.

· Provide barcode scanning of shred bins to insure Secure Chain of Custody.

· Definition of on-site shredding: Truck and driver arrives at facility, shred is collected and taken to truck to be shredded. This process continues until all material is shredded. There will be no collecting and staging of shred material, awaiting the arrival of a shred truck.

· Vendor must be NAID Certified at time of quote submission i.e., Vendor and/or subcontractor will adhere to the National Association for Information Destruction (NAID) requirement that shredded materials be destroyed to a particle or cross-cut result.

· Vendor must provide Certificate of Destruction after every service.

· Final Destruction to be: The process through which temporary paper records are pulped, macerated, or shredded by pierce and tear method to a degree that definitively ensures that they are not readable or unable to reconstruct to any degree.

· Vendor must provide Third Party Security Audits & Testing -SOC2 (Compliant Service Organization Control) reporting platform.

· Vendor must conduct Semi Annual Security Training.

· Provide Certificate of Environmental Accomplishment Annually.

· Drivers must never to touch paper and other materials such as tapes, plastic, or CD’s.

· Vendor will provide simi-annual security training of personnel.

· Provide appropriate locked bins (96 gal) and executive console (40’H x 19 ½ W x 19” D)

· Provide labor to pick up containers throughout the medical center once a week and perform on-site shred.

· Provide labor to pick up containers at HBPC/Franklin, Franklin CBOC, and Rutherfordton CBOC once a month.

· Provide labor to pick up containers at Hickory CBOC twice a month.

· Provide a certificate of destruction upon completion of on-site shredding.

· Turn around for emergency calls 1-2 days.

· All materials to be recycled.

(5) Other Station Rules and Guidelines. The Contractor will adhere to the following station rules.

4.1. All work will be accomplished to industry standards and compliant with all current CODE requirements (Life Safety, NFPA, Electrical and Building). All Dept. of Veteran Affairs, Federal regulations and codes will be complied with.

4.2. This is a hospital environment. The Contractor and his personnel are expected to be appropriately dressed (preferably uniformed) and to conduct themselves professionally.

4.3. This is a federal facility as well as a medical center. As such, there is NO tobacco usage allowed inside any federal building or on the grounds except in designated areas. The COR will brief the Contractor as to designated smoking areas prior to commencement of the contract. Failure to follow these regulations can result in removal of the worker or Contractor from the facility and/or a US Magistrate’s ticket and fine.

4.4. Parking will be in designated lots. Consult with the COR as to allowable parking and material drop-off.

4.5. Any work requiring a shutdown or utility outage will be requested in advance and scheduled by the COR prior to the work being accomplished.

4.6. The Contractor will be responsible for ensuring that all the employees and other personnel visiting the work site have and are using the appropriate Personal Protective Equipment (PPE) that addresses the hazards found at the site. The Contractor will be responsible for the upkeep, issue, and training for the PPE used and maintaining all required documentation.

4.7. Since the incidents of September 11, 2001, the Federal Government has begun to implement new security measures to help protect federal facilities. The Contractor will provide to the COR a list of employees expected to work on this contract. This list will be on company letterhead and contain the employees’ name, driver’s license number, address, and nationality. This list will be verified and signed by a corporate officer. These personnel are subject to a background check and fingerprinting to be eligible to receive a security badge.

4.8. Identification Badges will be picked up from Work Order Desk Room EB22 in Building 47.

4.9. Compliance Awareness Training shall be completed by vendor prior to access to Charles George VAMC. Vendor has responsibility for “Duty to Report” any fraud, waste and abuse witnessed while on VA premises to the Compliance Officer.
