VA Pittsburgh Healthcare System UD Damper Accessibility & Testing Statement of Work 646-19-100
1.0 GENERAL:
1.1 The Joint commission requires that fire and smoke dampers be tested every six (6) years to confirm that the fire and smoke dampers are operating as required and that a fire/smoke barrier is provided in all ductwork that penetrates fire/smoke rated barrier walls.
1.2 In 2017, fire and smoke dampers at University Drive were tested and a limited number of deficiencies were found. This project scope is to test the remaining fire and smoke dampers that do not meet the requirements as set forth by The Joint Commission Fire Protection Team and to bring the VAPHS up to EC.02.03.05-EP 18 – Fire and Smoke Dampers, NFPA 80, NFPA 105, and NFPA 101 standards. See Attachment – “A” for more information on Non-Accessible dampers.
2.0 SPECIFICS:
2.1 On the University Drive Campus, VA Pittsburgh Health Systems has identified dampers that have not satisfied the functional test requirement for healthcare occupancy. All dampers identified in Attachment “A” – Summary of Non-Accessible Dampers (Pages 1 -2), and as indicated on Attachment “B” – Floor plans (Drawings 1 - 12) are to be tested and certified by a qualified testing agency certifying that the fire/smoke dampers are operational as required by The joint Commission Fire Protection EC.02.03.05-EP 18 – Fire and Smoke Dampers, NFPA 80, NFPA 105, and NFPA 101 standards.
2.2 The VA Pittsburgh Healthcare System (VAPHS) has a requirement for all labor, equipment, tools, materials and travel to perform Repair and Testing Services for the Non-Accessible dampers at the University Drive Campus. The services are to achieve proper access to view the internal dampers for the periodical testing of the dampers.
2.3 There are thirty-one (31) dampers that have been identified in Buildings 1, 29, and 30 as either having failed a functional test and need to be repaired, or the damper could not be tested due to lack of accessibility in which case access needs to be provided, repaired and dampers tested where required. (See Attachment “A”) . The thirty-one (31) Non-Accessible dampers are as follows:
2.3.1 22 Dampers in Building 1
2.3.2 7 Dampers in Building 29
2.3.3 2 Dampers in building 30
2.4 The Contractor shall develop and submit a plan to obtain access to the Non-Accessible dampers, make corrections required to obtain access, perform all repairs required to make the dampers operational, and perform functional tests on the dampers to certify that the dampers meet the testing requirements as required by The joint Commission Fire Protection EC.02.03.05-EP 18 – Fire and Smoke Dampers, NFPA 80, NFPA 105, and NFPA 101 standards.
2.5 Contractor must review and determine the modifications required to any obstruction to complete the proper access to view and test the internal dampers.
2.5.1 Work will include the installation and/or repair of access doors, and/or modification to ductwork, pipe, wire, sensors, conduit, and related obstructions to achieve and maintain proper access.
2.6 Contractor as part of this work must submit a detailed plan indicating remedy of each damper, issues surrounding nearby utilities, schedule, and applicable outage(s) and planning per VAPHS medical center policies for disruptions to obtain VAPHS prior to before starting construction.
2.7 The Contractor must coordinate with CO, COR, Environmental Services, Infection Prevention, VA Safety, Construction Safety, and other departments to schedule work to cause minimal disruptions of medical center activities.
2.7.1 Contractor may be required to conduct survey of normally occupied patient care areas outside of standard business hours, IE Survey between 6P-6A, or on weekends, to mitigate impact to patient care.
2.7.2 Contractor must follow all applicable VAPHS medical center policies, including Infection Control Risk Assessment (ICRA) barrier as needed to protect patients and Staff in all areas of work, typical.
2.8 Reports: A report is required upon completion of the work
2.8.1 Report shall be submitted in 8.5”x11” format, both electronically and hard copy.
2.8.2 Summary of results shall be included (Pass/Fail).
2.8.3 Photo documentation (illustrating dampers in both Open & Closed position of each Damper – with a timestamp shall be provided.
2.8.4 Report shall indicate “Fire & Smoke Damper Inspection Services” in compliance with EC.02.03.05-EP 18 – FIRE AND SMOKE DAMPERS per NFPA 80, NFPA 105 and NFPA 101 (Rich confirmed per below)
3.0 Special Contract Requirements
3.1 The Contractor must obtain all necessary licenses and/or permits required to perform this work.
3.2 He/She must take all precautions necessary to protect persons and property from injury or damage during the performance of this Contract.
3.3 He must be responsible for any injury to himself, his employees, or others, as well as for any damage to personal or public property that occurs during the performance of this Contract that is caused by his or his employees’ fault or negligence.
3.4 The Contractor must maintain personal liability, automobile liability, and property damage insurance, as prescribed by the laws of the state of Pennsylvania.
3.5 Evidence of coverage is required before commencing work under this Contract (copy of Certificate of Insurance), and it may not be changed or cancelled without thirty (30) calendar days prior written notice to the CO.
3.6 Safety Requirements: To protect the lives and health of occupants of the buildings, the Contractor in the performance of this Contract must take such safety precautions as the CO, or his designate, may determine to be reasonably necessary.
3.6.1 The CO, or his designate, will notify the Contractor of any non-compliance with the foregoing provisions and the action to be taken.
3.6.2 The Contractor must, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his representative must be deemed sufficient for the purpose aforesaid.
3.6.3 If the Contractor fails or refuses to comply promptly, the CO may issue an order, stopping all or any part of the work, and hold the Contractor in default. Noncompliance may result in Contract termination.
3.7 Interference with Normal Functions: The Contractor’s personnel are required to interrupt their work at any time so as not to interfere with the normal functioning of the medical facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment and emergency and/or service carts. These interruptions should only be occasional. Any interruptions considered to be excessive should be brought to the attention of the CO.
3.8 Disposal of Hazardous Waste: The Contractor must not dispose of any hazardous materials at any location on the medical facility premises except as specified by the CO.
3.9 HIPAA Compliance: Contractor must adhere to the provisions of Public Law 104-91, Health Insurance Portability and Accountability Act (HIPAA) of 1966 and the National Standards to Protect the Privacy and Security of Protected Health Information (PHI).
4.0 Place of Performance: University Drive Campus of VA Pittsburgh Healthcare System
5.0 Estimated Period of Performance: Work shall be completed within 60 days of contract award.
