Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C244-19-AP-2077

1. Contracting Activity: Department of Veterans Affairs, VISN 04, Philadelphia VA Medical Center, 3900 Woodland Avenue, Eye Clinic, Philadelphia, PA 19104; 642-19-1-315-0031.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The estimated value of the proposed action is $133,809.86

This action is being submitted as a new request for brand name OPTOS California 200DTx to be used during Diagnostics, Disease Monitoring, Disease Treatment and Medical Procedures. The California Optomap (200DTx Optos) Unit has been specifically designed for General Ophthalmologist and Vitreo-Retinal specialists with a need for multiple imaging modalities capturing 220 degrees or 97% of the retina. The instrument provides comprehensive visualization of the retina and choroid for enhanced detection, diagnosis and management of disease. The Optomap provides non-mydriatic, high- resolution imaging, comprehensive retinal analysis through multiple wavelengths and image modalities.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

It is imperative that the Eye Clinic uses OPTOS California 200 DTx because it is the only imaging system that captures non-mydriatic 200-degree field of view through 2mm pupils showing up to 97% of the retina in a single image or non-montaged image. It is clinically efficient to not be able to get these images that show the mid-retina and peripheral retina that are required to diagnose and treat common diseases such as diabetic retinopathy. Having to refer patients to other non-VA facilities for testing and treatment could result in medical errors that often leads to adverse patient effects. No other brand offers the capabilities of the OPTOS California 200 DTx.

4. Description of market research conducted and results or statement why it was not conducted: Market Research was conducted by querying the Vendor Information Page (VIP), GSA Advantage, NAC, and the Internet for potenetial sources. Only one Service Disabled Veteran Owned Small Business (SDVOSB) vendor, Speco Medical Supply, one Women Owned Small Business, Metro Medical and one small business, OPTOS Inc., were found capable of providing the requested equipment.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. Note: COs are required to make a determination of price reasonableness IAW FAR 13.106-3.
