1. Amendment A00002 is hereby incorporated into RFQ number 36C244-18-Q-9267 and shall become part of any resultant Purchase Order. This amendment contains the following information:

a. List of attending vendors at site visit.

b. Information provided at the site visit.

c. Contractor Questions and Government Answers

d. A Revised Statement of Work (SOW)

e. Provide the Schedule of Supplies and Services in WORD Format

f. An extension to the due date of this RFQ.

2. The following vendors attended the site visit held at the VAPHS on Wednesday, June 27, 2018:

3. The following information was provided to vendors at the site visit:

· Contractors working on site are required to have an annual PPD TB test on file with the COR

· Contractors working on site are required to complete annual privacy training.

· Contractors working on site are required to have an annual flu shot or wear a face mask while on site.

· There are after normal working hours required to complete the air flow checks. Saturdays are preferred.

· Site visit attendees had a site tour to all of the areas where air flow checks will be conducted.

· VA will provide 2 HVAC mechanics to escort and assist contractor personnel to perform the air flow testing.

· If there is an air flow out of compliance the VA HVAC mechanics will make attempts to correct, contractor will recheck while on site to verify compliance.

· If there are any readings out of compliance the contractor is required to notify the VA Utility System Supervisor, contact information will be provided upon award of contract.

4. The following are responses to all contractor submitted questions and Government answers received in response to this RFQ:

Question 1: This solicitation is currently listed under a testing laboratory NAICS code. In the past, I have normally seen this code reserved for analytical laboratory services. Would you be willing to consider a more common NAICS code for these types of services such as 541690 - Technical Consulting Services? A recent award for solicitation 36C24818Q0302 was listed under this code for similar services. I have also seen NAICS code 541620 used.

Answer 1: The Contracting Officer has determined the appropriate NAICS code for this procurement to be 541380. The Small Business size standard for this NAICS code is $15M. Should an interested vendor not be currently certified with this NAICS code in SAM or VIP, they can still submit quote. The Contracting Officer must be able to determine them to be a VIP certified SDVOSB since this is SDVOSB set-aside. Then, the Contracting Officer must be able to determine that they meet the size standard of $15M from the SAM FAR Report. If awarded this contract, or any time prior to, the FAR Report can be updated to reflect this NAICs code.

Question 2: As of June 10th, 2018, SAM actually requires a notarized letter for me to update the NAICS codes in our account, so I am not sure if our SAM account will be updated by the due date of the Solicitation (July, 6th). Should we still submit a response under the listed NAICS for testing laboratories? I apologize, we have not seen this NAICS code used for this type of services before, but we will definitely add it to our profile.

Answer 2: Please see Answer 1 to Question 1.

Question 3: In accordance with the Statement of Work, "Qualifications of Personnel Conducing Air Flow Testing Services", must the individual(s) performing the actual on site air flow testing be a Certified Industrial Hygienist (CIH) and a Certified Safety Professional (CSP) or can the individual(s) perform the air flow testing work under the supervision of a CIH/CSP?

Answer 3: Any individual conducting the actual on site air flow testing must be a Certified Industrial Hygienist and a Certified Safety Professional.

Question 4: Is Airflow Testing to be performed on both University Drive and HJ Heinz Campus?

Answer 4: Airflow Testing is to be performed at the University Drive Campus only. This was stated in the Statement of Work (SOW). The solicitation inadvertently stated both the University Drive Campus and the HJ Heinz Campus.

Question 5: Are the dimensions (cubic feet) of the rooms available?

Answer 5: The cubic feet (room volume) of the rooms to be tested will be provided to the contract awardee.

Question 6: Does the Government require metric measurements on report?

Answer 6: No, the Government does not require metric measurements.

Question 7: Will the VA consider ventilation and indoor environmental experience that is not exclusive to a major medical facility? We are able to provide a Certified Industrial Hygienist (CIH), Certified Safety Professional (CSP), and Certified Indoor Environmental Consultant (CIEC) with graduate education in industrial hygiene, to include ventilation design and evaluation, and over nine years of industry experience overseeing and conducting ventilation and indoor air quality surveys. Although some of our consultant’s ventilation experience is related to medical facilities and research laboratories, it is not exclusive to this field, but we believe their experience and credentials are more than sufficient to conduct the basic air flow measurements required by this RFQ. Additionally our consultant is familiar with VHA operations and has performed industrial hygiene surveys, lock-out tag-out procedure development, Legionella management planning, and asbestos work at multiple VAMCs in VISNs 1, 4, and 10, to include the Pittsburgh VAMC?

Answer 7: No, the Government cannot consider other than major medical facility experience as stated in the addenda to FAR provision 52.212-1. The reason for this is that the vendor awarded this contract must be able to hit the ground running and perform the scope of work. The vendor must be able to function in the environment wherein the air flow services are going to be performed. A vendor that has the major medical facility experience will be familiar with infection prevention precautions and personal protection equipment measures Also, the rooms described in the scope of work (i.e. Operating Room (OR)) require specific protocol will allow for the performance of this contract while maintaining the stringent medical standards required for patient safety. It is incumbent upon each interested vendor to provide narrative describing the experience that meets this standard.

Question 8: Is it acceptable to conduct air flow measurements during consecutive periods for each monthly survey (e.g. Monday through Friday) to minimize cost to the government related to travel?

Answer 8: The Government requested clarification to this question in order to answer it accurately. The clarification and answer are provided below in Question 8a and Answer 8a.

Question 8a: We are asking how many travel periods we must plan for each line item. In the scope of work there are three frequencies of testing, monthly, quarterly, and semi-annual. Here are two example scenarios to assist with clarification:

1. Monthly Testing - During monthly testing may the contractor plan one trip, (e.g. a few consecutive days of overnight travel), to complete all ventilation measurements? Since the VA will likely require an escort, can this be accommodated to minimize the need for addition travel to Pittsburgh? We would like to ensure we will not need to account for additional flights or mileage to the Pittsburgh VAMC due to intermittent escort availability or limited access to areas requiring measurement. If consecutive days of overnight travel are not acceptable, how many travel periods to the Pittsburgh VAMC should be planned? For example, would two trips per month be acceptable?

2. Quarterly and Semi-Annual Testing - If testing during consecutive days is acceptable can we combine quarterly and semi-annual testing with the monthly testing to complete all events during one trip. For example, monthly, quarterly, and semi-annual testing would all be completed during the same week, requiring only one trip to Pittsburgh with overnight travel.

Answer 8a:

1. In reference to Monthly testing, the monthly testing required for critical care areas under current conditions can be completed in an 8 hour period. The testing is conducted on Saturdays to facilitate access to critical care areas. It is feasible to conduct the testing in most critical care areas during the week on second shift but the SPS areas will not be available on week days. Escorts; 2 personnel will be provided during visits as long as a reasonable advanced notification is given. If the contractor would like to schedule consecutive days as long as there is advanced notice the escorts will be available excluding government holidays. The key will be consistent and clear communication as to when the contractor is scheduled to be on site.

2. Quarterly and semiannual testing can be conducted simultaneously during normal business hours that are not critical care areas. If the contractor would like to conduct the semiannual or quarterly on a Thursday and Friday and critical care on Saturday that would be acceptable.

Question 9: Approximately, what percentage of work does the VA anticipate will need to be performed outside of normal business hours (e.g. nights or weekends)?

Answer 9: Below are the anticipated percentages for the areas required to be performed outside normal business hours. As such, it is strongly recommended that the areas identified be conducted on Saturdays to avoid interference with OR surgery’s and critical care areas.

Outside Normal Business Hours: OR’S, SPS, Same Day Surgery, Recovery, ICU’S on floor 3 and 4, Urology, Isolation on 5 and 4, Morgue and Special procedure = 85% Normal Working Hours: Soiled Utility, Animal Labs = 15%

Question 10: On page five of the statement of work you request an excel spread sheet with fields for “air balance required” and “air balance actual.” Are you requesting the supply or exhaust air-flow in CFM that is required to maintain the regulatory air change rates listed in the scope of work? Can you provide the source of these regulatory requirements for air exchange for informational purposes (e.g. VHA directive, ASHRAE 170, JCAH)?

Answer 10: No, the Government is requesting that the contractor provide the actual air changes per hour and the actual air balances based on field measurements of the supply and exhaust and the room volumes that will be provided by the VA. The regulatory source of these requirements is the DVA HVAC Design Manual dated November 1, 2017 and DVA Deputy Under Secretary for Health for Operations and Management (10N) Memorandum dated September 5, 2017.

Question 11: ANSI/ASHRAE/ASHE Standard 170 “Ventilation for Health Care Facilities” requires the measurement of an outdoor air exchange rate (i.e. %OA) which is not included in this scope of work. Can you verify %OA measurements are excluded from this contract? If required, would surrogate testing using temperature or ambient CO2 concentrations to estimate the outdoor air flow percentage for each system be an acceptable technique? Also, if access to the HVAC systems is required for measurement (e.g. supply ducts, outside air ducts, air handling unit access locations etc.), will a VA escort be able to provide safe access?

Answer 11: The Government confirms that %OA measurements are excluded from this contract. The VAPHS will provide two (2) A/C mechanic escorts for the duration of the air flow checks to gain access and operate systems as required.

Question 12: Can you confirm whether all diffusers or registers will be accessible from the ground and are approximately ceiling height (e.g. 15 feet or less)? Will the VA be able to provide access to areas over this height (e.g. ladder or lift)?

Answer 12: There are some diffusers over 15 feet in height. The VAPHS cannot provide any Government Furnished Property (GFP) to perform this contract. The SOW states that the contractor is required to provide all tools and equipment to perform and execute the contract requirements.

Question 13: Does the VA anticipate that all air airflow measurements may be made at the supply diffuser or exhaust grille with a standard air capture hood (i.e. 3’ x 3’ or 1’ x 5’ hood) or will flow measurements require alternate methods such as an in-duct pitot traverse? If pitot traverse is required, will the VA provide duct penetrations for center line velocity of multi-point traverse measurements?

Answer 13: There are some locations that require alternate methods such as air flow monometer using a multiplier. Additional penetrations are not authorized as most areas requiring that option are SS and would affect the integrity of the duct system. The majority of the locations utilize the standard air capture hood.

Question 14: A minimum hourly wage was not provided for a Certified Industrial Hygienist or a Certified Safety in the attachment. Can the VA provide guidance on minimal acceptable hourly wages for these job titles.

Answer 14: The contractor is responsible for correct classification of worker(s) and compliance with all wage and hour laws. The Contracting Officer has determined that the appropriate classification code(s) are contained within the selected DOL Wage Determination for this RFQ and that no additional conformance process is required. It is important to remember that selection of the code/category is made by tasks performed in the classification category. 5. A Statement of Work (SOW) has been revised and is provided as an attachment to this Amendment A00002. Specifically, the following items have been changed:

a. The requirement to have an annual PPD TB test and provide documentation to the COR.

b. Clarification on when work is to be coordinated with COR (normal business hours vs. outside normal business hours).

6. It was requested that a copy of the Schedule of Supplies and Services be provided in WORD format. This is attached. Vendors can submit either the WORD or PDF version with their quote package.

7. The due date for this RFQ is extended as follows:
