Statement of Work James J Peters Veterans Affairs Medical Center Replace Abandoned HVAC on 2B Roof

Background: The Main Building of the James J. Peters Veterans Affairs Medical Center, 130 West Kingsbridge Road, Bronx, New York 10468 is currently seeking a demolition and removal of abandoned HVAC equipment on the roof of the 2B laboratories, plus the repair and replacement of the mushroom cap on the same roof. This work will include a demolition and removal as well as a sealing of the roof.

The intention is to come on-site to remove and demolish the HVAC equipment that has been abandoned for 30+ years. This abandoned equipment often allows water to run inside the building and backwards into the HVAC equipment flooding the 2B laboratories. There is one set of HVAC equipment to be removed, and afterwards to be sealed with rainproofing sealant. This area may need to be capped as the HVAC equipment leads into the building. For now, anything internal to the building will remain abandoned, so the contractor must be sure to cap this area well.

In addition to the removal and sealing of the abandoned HVAC equipment, there is also a mushroom cap that flew off in a recent rainstorm. There was a patch to cover the hole left behind by the removal of this cap, and the intention is to have the cap replaced with a very similar cap that allows for the ventilation function of the 2B laboratories. This includes any additional work associated with reconnecting the cap to ensure the ventilation system works properly.

A water test will be required over the HVAC equipment area to prove the rainproof work was successful. The water test will consist of providing a barrier of some sort, filling the guarded area with at least 1-inch of standing water, and have it stand for 24 hours. 8 of these hours will be watched with a contractor on-site, but the test must be at least 24 hours long. A COR will sign off on a completed water test.

This work will be considered complete when the ventilation system is tested and works successfully.

This work will be considered complete when:

· the ventilation system is tested and works successfully

· The roof is rainproofed, and the laboratories below are no longer leaking

This work is to be completed with a dumpster that will carry away all construction debris off-site. Work is to be considered complete when the COR receives a sanitation ticket.

A. Scope of Work:

1. The contractor shall remove all materials that make up the abandoned HVAC equipment on the 2B laboratory roof. This will include at a minimum: the HVAC system (all parts attached to the roof), the tarp, straps, the cinder blocks, any debris collected on the roof from this work, etc. This work will exclude: any HVAC equipment or material internal to the building.

2. The contractor shall also fill in pitch pockets in surrounding HVAC equipment as shown in the photos below. Perform a water test to ensure the laboratory below does not still see water intrusion. There are about 10 pitch pockets to fill, and they can be seen with cracks and/or plant life growing out of them.

3. The removal process will be thorough, and the contractor must be sure to bring their own dumpsters or other disposal vehicle. Contractors will not be allowed to use the dumpsters stored on-site already from other vendors or that is the property of the James J Peters Medical Center. The materials should be removed off-site and shall include a sanitation ticket to ensure its removal was safe.

4. The mushroom cap will also need to be replaced. Contractor must ensure that the replaced mushroom cap is of exact or extremely similar size and utility. The mushroom cap replaced must be able to function with the laboratory below. There is also a patchwork on the covered vent that must be removed before the start of this mushroom cap phase. The contractor must use the utmost caution to not damage this vent. A test shall be conducted to ensure the mushroom cap functions as normal. The COR must sign off that this ventilation system is functional.

5. The removal process includes some patchwork which the contractor is also responsible for. Once the HVAC system is removed from the roof, it will leave a hole internal to the building, as this is connected to the laboratory and rooms below. This open area must be sealed properly and be rainproofed for this work to be considered complete. The sealant must be approved by the COR with a submittal before it is cleared to be used.

6. A water test must be conducted for the sealant work to be considered complete. The water test will consist of providing a barrier of some sort, filling the guarded area with at least 1-inch of standing water, and have it stand for 24 hours. 8 of these hours will be watched with a contractor on-site, but the test must be at least 24 hours long. A COR will sign off on a completed water test. The COR will be informed at the 0-hour mark and the 24+ hour mark to ensure a proper test.

7. A warranty is required for this work. The warranty for this work shall cover any debris left behind on the construction site, the weatherproofing of the plugs made into roof, any leakage from an improper capping, etc. as per reasonable means of this work. A warranty of 1 year is expected for this work.

8. A man-lift and other heavy machinery may be required for this work. The contractor is responsible for the cleanup of debris and other items that have accumulated from the construction site.

9. A certification of completion and a receipt of services shall be included by the completion of the project. A report on findings may also be included as needed.

10. Technician should have certifications for work to be completed. Technicians should have plumbing/electrical certifications or certifications pertinent to this type of work.

11. A sanitation ticket is needed to complete this work. The contractor may not invoice for this work until a sanitation ticket is submitted to ensure proper disposal of debris.

B. Safety Requirements

The Contracting Officer or his/her designee will notify the contractor of any noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all and any part of the work and hold the Contractor in default.

C. Period of Performance

The period of performance shall be 60 calendar days from the Notice to Proceed.

D. Place of performance

Site for the work to be performed will be the main building (Building 100) in the James J Peters Veterans Affairs Medical Center, on the 2B laboratories’ roof.

E. Inspection

If inspection services are required for equipment or a performed task, then an inspection shall be done by the VA COR/POC upon completion. Upon completion of the maintenance service, the COR/POC will perform the inspection in the presence of the contractor personnel, prior to the contractor personnel leaving the facility. This inspection will ensure that all equipment is returned to full functioning condition No invoices will be processed for payment prior to the completion of POC inspection. If the service is routine and/or performance based, then inspections may not occur for every service date but may occur periodically as part of a Quality Assurance Surveillance Plan (QASP). If this is the case, then a QASP will need to be provided as a separate document.
