Peters VA Medical Center 130 West Kingsbridge Road Bronx, NY 10468-3904 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Items,” as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; [insert quotations are being requested, and a written solicitation document will not be issued.
This solicitation is a request for quotations (RFQ), the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular.
The associated North American Industrial Classification System (NAICS) code for this procurement is 811219, with a small business size standard of 20.5 Million Dollars.
The Northport VA Medical Center is seeking to purchase the Service and Maintenance of the Eaton Model 9315 Uninterruptable Power Supply Source with a Base Plus Four Option Years Contract.

The delivery/task order period of performance is:
Base Year: 7/01/2018 thru 6/30/2019 Option Year 1: 7/01/2019 thru 6/30/2020 Option year 2: 7/01/2020 thru 6/30/2021 Option Year 3: 7/01/2021 thru 6/30/2022 Option Year 4: 7/01/2022 thru 6/30/2023

Award shall be made to the “quoter” r whose quotation offers the best value to the government, considering technical capability, past performance, and price. The government will evaluate information based on the following evaluation criteria:(1) technical capability factor meeting or exceeding the requirement, (2) past performance, and (3) price.

The following solicitation provisions apply to this acquisition:
1. FAR 52.212-1, “Instructions to Offerors–Commercial Items” [insert date of provision]
2. FAR 52.212-3, “Offerors Representations and Certifications–Commercial Items” [insert date of provision]
Offerors must complete annual representations and certifications on-line at http://orca.bpn.gov in accordance with FAR 52.212-3, “Offerors Representations and Certifications–Commercial Items.” If paragraph (j) of the provision is applicable, a written submission is required.
The following contract clauses apply to this acquisition:
· FAR 52.212-4, “Contract Terms and Conditions–Commercial Items” (JAN 2017)
· FAR 52.212-5, “Contract Terms and Conditions Required to Implement Statutes or Executive Orders” (JAN 2017)
All quoters or bidders shall submit the following2 copies of quotes and SDVOSB certification from Department of Veteran Affairs
All "quotations, shall be sent to the Contracting Officer Electronically, ALL MAILED QUOTES WILL NOT BE EVALUATED.
This is an open-market combined synopsis/solicitation for services as defined herein. The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all [insert quotes must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
Submission shall be received not later than June 13, 2018 NLT 3:00PM EST. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). NO faxed QUOTES will be accepted
