QUESTIONS/ANSWERS

1. We noticed that the SF1449 form was not included. Could you kindly provide the SF1449 form or clarify if it is required for submission under this solicitation? SF 1449 is not necessary for the requirement. I attached the Estimate and Pricing template to RFP due to formatting errors of the 1449, and will attach it here. If that is suffice then we are fine to just utilize that.

2. The solicitation outlines quantities for various radiology services, but could you confirm if these are the actual numbers from previous years? Yes, we pulled numbers from the previous years, and then rounded off.

3. The solicitation outlines that radiologists must be licensed and board‐certified, but it does not explicitly mention the submission of CVs or resumes as part of the proposal. Could you please confirm if CVs or resumes of the radiologists are required with our proposal? If so, could you specify the required documentation to be included? Or, is the focus solely on ensuring that radiologists meet the necessary qualifications (such as licensure and board certification) at the time of submission? Please submit CV’s or resumes at the time of submission. CV, resume, licensure, insurance at time of submission.

4. The solicitation outlines three sections for the technical proposal:

a) Technical Capabilities

b) Management Approach/Key Personnel

c) Past Performance of the Organization Could you please confirm whether these three sections should be submitted as a single combined file, or if they should be submitted as separate documents for each section? Any specific names for these files? Volume 1 will consist of all Technical, Management/Key Personnel, and Past Performance. Volume II will consist of Cost/Pricing Proposal.

5. Is there an incumbent contractor? If so, please provide the contract number. Current contractor is Diagnostic Imaging Associates, contract HHSI2462020C0004.

1. Is the 20 minute response time for stroke and trauma patients only, or is this time limitation for all emergency room visits? The STAT within 20 minutes is stroke and trauma, the turn around time for all other STAT’s is 1 hour.

1. Also, from the total volume of imaging projected to be interpreted - what is the estimated volume of emergency room interpretations? 13% across all facilities are ER requests
