Supplement Instructions and Evaluation Information for Offerors

The Offeror’s RFQ shall consist of a cover letter and three (3) separate volumes:

Cover Letter shall include the following:

Small Business Status/Categories as appropriate

Company point of contact with email, phone and facsimile number

Cage Code & DUNS number

Volume I - Technical Capability

Volume II - Past Performance shall include the following:

Client Authorization Letters

Past Performance Questionnaire

Relevant Contracts Reference Sheet

Volume III - Price

Price will be evaluated for reasonableness, balance and completeness.

RFQs shall be submitted on 8 ½” x 11” paper, except for fold-outs used for charts, tables or diagrams, which may not exceed 11” x 17”.

All text shall be single spaced and printed black on white paper; graphics, photos, etc. may be in color.

Printing shall be clear and eligible; 12-pitch type, 10 point proportional spacing using Times New

Roman font is acceptable.

All pages shall be printed on one side only.

Elaborate formats, bindings or color presentations are not desired or required.

Offerors will be evaluated based on a Performance Price Tradeoff (PPT) methodology. The Government will select the best value in which competing offerors’ Technical Capability, Past Performance and Price will be evaluated. All factors other than price are more important than price when combined.

FACTOR 1 – TECHNICAL CAPABILITY

1. Subfactor 1, Technical Approach – The approach the Offeror proposes to meet the government’s requirement.

a. Subfactor 1.1, Subject Knowledge. The Offeror shall describe how their approach benefits the

Government with respect to their demonstrated knowledge, experience, and expertise of the subject matter and how the Offeror intends to leverage this knowledge, experience, and expertise to provide the best possible service to the Government.

2. Subfactor 2, Personnel Management Plan – The process the Offeror proposes to validate and maintain credentials of the proposed HIPAA Privacy Policy Services.

b. Subfactor 2.1, Staffing of Personnel. The Offeror shall describe their approach to staffing this requirement with qualified candidates. Process details shall be provided as to how the Project

Manager will be identified, screened, placed, and monitored, as needed, to perform the requirements.

The candidate’s education, experience, and expertise shall also be provided.

c. Subfactor 2.2, Maintenance of Personnel Qualifications. The Offeror shall describe their methodology to ensure that the Contractor’s qualifications are maintained throughout the contract’s period of performance. Offeror’s process shall detail how the Project Manager will be have the licensures, credentials and/or certifications necessary to perform in accordance with the approach proposed and without oversight by, additional costs to, or intervention by the Government.

d. Subfactor 2.3, Risk Mitigation. The Offeror shall describe their approach to mitigating the effects of potential issues. Offeror’s approach shall demonstrate their knowledge, expertise, and experience with typical issues which arise in providing this type of service and how the Offeror intends to deal with these issues to provide the best possible service to the Government.

FACTOR 2 – PAST PERFORMANCE

The Contractor shall provide a detailed description of two (2) efforts similar in scope, magnitude and complexity to the requirements as outlined in the PWS. The Contrator must also identify whether their role in the cited project was as a prime contractor or as a sub-contractor, as well as, the extent of support provided. Past performance will be evaluated based on the following:

o Recency, is relevant past performance within the last five (5) years.

o Relevancy, is a measure of the extent of similarity between the service/support effort, complexity, dollar value, contract type, and the Offeror’s role as a subcontractor and/or teaming partner or other comparable attributes of past performance examples and the source selection requirements; and a measure of the likelihood that the past performance is an indicator of future performance.

o Clear demonstration of the contractor’s ability to obtain certified SMEs of a specialized nature.

FACTOR 3 – PRICE

1. Sub-Factor 3.1: Price will be evaluated for reasonableness and completeness.
