Quality Assurance Surveillance Plan (QASP) Respond to the following tasks according to the support agreement:

MEASUREMENT GROUP
Web Applications and Websites must be delivered on time
Performance Measure
Project Delivery
Objective
Measurement of the performance of the contractor in delivering developed web applications and websites with acceptable time frame.
Deliverable
Website and Web App
AQL
Acceptable - All project deliverables are provided within 10% variance of the date scheduled and agreed upon between the Web Services Manager and Project Manager.

Exceeded - All official reports are delivered more than 1 day in advance of the date scheduled.

The project scheduled delivery date compared to the actual completed report deliver date.

Frequency
At project delivery

Customer Satisfaction with Scheduled Reports is reported in the "Final Project Summary Report".

Surveillance
Project plan will include a Gantt chart for project schedule
Notes:
Delays not under contractor control are not included in Quality Level calculation.
MEASUREMENT GROUP
System Documentation must be delivered in complete form
Performance Measure
System Documentation (Technical Guides, User Manuals and Plans)
Objective
Measurement of the performance of the contractor in delivering required internal documentation to include system technical guides, user manuals and plans.
Deliverable
Technical Guides, User Manuals and System Plans
AQL
Acceptable - 95% of maintained system documentation is reviewed, modified and delivered within 7 work days of its scheduled due date; no scheduled documentation updates are delivered more than 14 work days after its scheduled due date.

Exceeded – 95% of maintained system documentation is reviewed, modified and delivered by their due date; no scheduled reports are delivered more than 7 work days after due date.

Frequency
Review due to Contracting Officer Representative (COR) Annually

All updates to documents posted within 30 days of significant changes

Surveillance
A formal review by the COR at the time each scheduled document is delivered and approved.

Documentation required and outlined in the PWS tasks.
MEASUREMENT GROUP
Program / Project Management must receive acceptable deliverables
Performance Measure
Program / Project Management – Contractual Deliverables
Objective
Measurement of the performance of the IHS contractor in delivering required contractual deliverables to the Program Manager and IHS customers as scheduled with acceptable quality (as determined by the Service Level Agreement (SLA)). The SLA is the document that defines what is expected in terms of each web element that might require assistance, ranging from web services request submission to a minor content change.

Acceptable - 95% of deliverable requirements are met within 5 work days of scheduled date and are acceptable to the Program Manager.

Exceeded - 98% of deliverable requirements are met within 5 work days of scheduled date and are acceptable to the Program Manager.

Frequency
Review due to COR Annually

All updates to documents posted within 30 days of significant changes

Surveillance
For each deliverable, the actual completion date is compared to the due date and accepted by the Program Manager. The Actual completion date is also logged with the number of elapsed days (if any) after the scheduled due date and reasons for exception.

Schedule and PMB for all DME projects Periodic Operational Status / Progress Report System Performance Reports Periodic Problems/Issues reports Operation Cost and Earned Value Reports Other information for the CPIC, HHS, OMB 53 & 300 processes Program Change Management Plan (As directed by COR and scheduled in the Project Management Plan) Risk Management Plan (As directed by COR and scheduled in the Project Management Plan) Communications Plan (As directed by COR and scheduled in the Project Management Plan) Current versions of all documentation both hardcopy and online Technical documentation is updated within 5 days of a minor change to IHS.gov infrastructure or within 30 days of a new or significant change to IHS.gov infrastructure.

System Security must be maintained by the contractor

Compliance with Security Requirements
Objective

Security Continuous Monitored Certification

Acceptable Quality Level (AQL) Acceptable - 95% of Security requirements are met within 5 work days of agreed upon date with OIT Security and are acceptable.

Exceeded - 98% of Security requirements are met within 5 work days of agreed upon date with OIT Security and are acceptable

Delivered to the COR

Surveillance
For each deliverable, the due date (Information Systems Security Officer (ISSO) approved) is monitored against the actual completion date to note the number of elapsed days (if any) after the due date and reasons for delays or ISSO non- acceptance.
