If your company has a "doing business as" name reflected in you SAM registration, include this name as well in Block 14. Ensure the address typed in block 14 matches the address in your SAM registration.

• PAST PERFORMANCE SUBMISSION: Please ensure the Cover letter is reviewed and complied with its entirety. References and banking submissions must be submitted in advance to the bid opening directly to Ms. Rios via email.

• COMMODITY OR SERVICES SCHEDULE (PAGE 4): Complete the Unit Price (JB = job) and Amount blocks (these amounts will be the same). Note: The amounts listed in this section should be the same as the amount listed in block 17 of the SF-1442.

• BID GUARANTEE: See Page 8 of the solicitation. A bid bond must be submitted in original (ink signatures) with your solicitation. The penal sum of the bond must be at least 20% of the bid price, not to exceed $3 million, whichever is less. A blank bid bond was included in the solicitation. The bid bond must be from a Government approved surety. A list of approved sureties can be located on the Department of Treasury, Bureau of the Fiscal Service website.

• REPRESENTATIONS AND CERTIFICATIONS: All contractors submitting a bid must be registered in the System for Award Management (SAM) at www.sam.gov with the North American Industry Classification Standard code (NAICS) 238220 which is applicable to this acquisition.

• FAR PROVISION 52.204-8: Pursuant to the section (b) (1) of this provision, provision 52.204- 7 is included in this solicitation. Bidders should not mark either of the two options under section

(b) (2). Leave these options unmarked.

2. Other general items to be addressed:

• CONSTRUCTION WAGE RATE: This contract is subject to the Construction Wage Decision, LA2020051 is applicable for this area. All applicable workers must be paid according to the terms of the wage decision.

• LICENSING AND PERMITS: All required licensing and permits are required to be obtained by the contractor who receives contract award .

• WORK HOURS: Work hours within the secure perimeter shall be between 7:30 am and 4:00 pm Monday through Friday excluding weekends and federal holidays.

• COR: The contracting Officer's Representative (COR) is Mr. Brad Breland, Engineering Technician at FCC Pollock.

• PERIOD OF PERFORMANCE: The completion period will be determined at award.

• TIME EXTENSIONS: Time extensions for inclement weather will not be granted unless the weather is unusually severe and abnormal in comparison to prior years.

• PRE-CONSTRUCTION CONFERENCE: A Pre-Construction conference is required after the contract is awarded and will be scheduled accordingly.

• PROGRESS SCHEDULE: A copy of the progress schedule is to be provided to the Contracting Officer and Contract Monitor as soon as possible. Each time the progress schedule is changed, it must be provided to both the Contracting Officer and COR. It is suggested an updated document be provided by the Prime Contractor at the mandatory monthly progress meeting.

• PROGRESS MEETINGS: Progress meetings are mandatory and will be scheduled monthly.

• PAYMENT REQUESTS: Application for payments are to be submitted on AIA G702(Continuation) or the contractors own form as long as the information is similar to the G702. Contactors can get these forms from the following website www.aiabookstore.com

• ACCESS TO SITE/PARKING OF VEHICLES: Parking will be provided for employee vehicles and contractor workvehicles.

• STORAGE OF MATERIALS: Storage of materials will be coordinated with the Contract Monitor.

• CONTRACTOR TEMPORARY OFFICE: No temporary office space will be provided.

• INSPECTIONS: "Punch List" items generated by inspections will be corrected and discrepancy items re-inspected and approved before contract closure takes place. Should any work be covered without proper notification, the contractor shall uncover that work for inspection at his own expense.

3. SAFETY POINTS:

• The contractor is responsible for providing required protective gear to all contractor workers, should it be required. Contractor will ensure that all workers have proper safety gear at all times.

• Contractor is responsible for the prevention of accidents on the project site and has spill kit onsite and containment procedures. OSHA requirements will be adhered to.

• The Contractor will have a Safety Representative on site at all times during the completion of this project.

• Proper storage of any chemicals in lay down yard with SOS present.

4. SECURITY

• ADVERSE WEATHER: Depending upon the nature of the emergency, this normally will not affect any construction outside of the secure perimeter. Contractor is reminded that fog days will be treated like rain days. The performance period will be determined at award to allow adequate time to complete the job, and no consideration will be allowed for normal rain days and normal fog days. The normal rain, snow or fog days will be based on the average over the last ten (10) years for the local community.

• SECURITY CLEARANCES: Contract workers are subject to security investigative procedures.

Please refer to the contract for details. Please refer to Page 16 of the contract.

• EMPLOYEE IDENTIFICATION: Contractor personnel working on project site must bring valid photo identification to the institution each day.

• TOOL ACCOUNTABILITY: Strict accountability and control of all contractor tools and hazardous materials must be maintained at all times. The contractor should provide for enough time at the conclusion of the work day for the return and accounting of all tools issued during the work day.

Lost or missing tools must be reported to the COR and the Captain immediately.

• ILLEGAL ITEMS ON FEDERAL PROPERTY: Firearms, ammunition, knives, other weapons, drugs, narcotics, and alcoholic beverages are notpermitted inside/outside the secure perimeter or anywhere else on Federal property at any time. This includes the private vehicles of contract workers. Violators will be subject to criminal prosecution. All persons, vehicles, and equipment are subject to search by institution authorities at any time.

• NO CONTACT WITH INMATES: Contract workers are not to have any contact with inmates at

any time. No photographs may be taken of inmates.

• CONTRACTOR EMPLOYEE WORK DRESS: Green or khaki-colored clothing is not allowed, as inmates wear these colors.

• PHOTOGRAPHS: The institution's Facilities Department maintains a digital camera that will be utilized by FBOP staff to document the project work. The Contractor may request copies of these photos, and may request that photos be taken for specific conditions. Cellular telephones equipped with photographic capabilities are prohibited on the project construction site.

VIII. ADDITIONAL NOTES:

Questions and Answer from Pre-Bid Site Visit Held on 9/8/2020 at FCC Pollock

Q1. Upon reviewing the SOW document identifying "provide all labor and material for the installation of a new fire pump and fire suppression system" and then reviewing the photo's and drawing included in the controlled document titled same, we are curious are there any components beyond the fire pump such as system risers, new mains, or new sprinkler heads to be addressed in this project? Or is just all items specifically identified normally associated with a fire pump?

Answer: Only parts associated with the Fire Pump will be required by the contractor. There will be no change in the system risers, new mains, or new sprinkler heads.

Q2. If there are additional fire sprinkler system components, will there be additional drawings to come?

Answer: There will be no changes to the fire sprinkler system components in this project. The contractor is only changing the fire pump skid.

Q3. Will the awardee have to provide notice for shutting off the utilities, have to provide a schedule or have any type of back up plan in place if they are to be shut down for an extended period?

Answer: We are modifying the SOW of work to include this. FCC Pollock would require a 2 hour notice prior to shutting off any utility services. Additionally, the contractor should provide a proposed and pre-approved plan of action, 10 Working Days in Advance, to the COR and Facilities Manager on the specifics of expected time lines, down times in utilities, and potential delays in the project. This is so FCC Pollock can make provisions and have contingency plans and equipment in place to minimize any disruption of services provided to the inmate population and fire protection response.

SUMMARY OF WORK

The Federal Correctional Complex at Pollock, located at 1000 Airbase Road, Pollock, LA 71467 has a requirement for the replacement of a skid mounted fire suppression pump system at the United States Penitentiary. The fire pump at USP has been identified that there are serious engineering design flaws within the system and was rated “Poor” on its last inspection report. If activated the current system would be required to pump water several hundred yards and up four stories; while maintaining at least 35 psi of water pressure at the highest sprinkler head/point. In its current state the fire pump would activate and fall short of maintaining this critical pressure. As well as, there is significant concern that the 150 psi surge would cause ruptures in the fire suppression piping.

Another determinant to the 35 psi setting is that there are systems within the prison that require much higher pressure to operate. Original drawings indicated that a pressure of 112 psi is required to operate these systems properly. Currently there is only one city tank pressure of approximately 50 psi; which is too low to operate these systems. It has been proven by flowing 2 inch drains that the fire main pressure will not drop low enough to activate the fire pump. Ultimately, the upper level systems will never see the pressures in excess of 112 psi.

Bidders shall provide a lump sum bid based on the requirements of the solicitation, and provide all labor and material for the installation of a new fire pump and fire suppression system, coupled with an independent fire pump soft start, via Variable Frequency Drive, for surge protection and ensure that the Pressure Reducing Valves (PRV) on the domestic side are operating properly. The domestic water pumps (jockey pumps) would be modernized and sequenced with the existing fire pump controller. This upgrade would maintain a constant 125-150 psi on the loop, regardless of domestic demand. Ultimately, the fire suppression system will operate to its original design intent.

All work described in these documents and all work necessary to complete the work as described or shown is to be executed in a thoroughly substantial and workmanlike manner. All work shall be done by persons who are thoroughly experienced in their particular trade or craft. The work includes, but is not necessarily limited to the following description:

1. INTRODUCTION

1.1 The Contractor is strongly encouraged to attend the pre-bid site visit identified in the solicitation, to satisfy themselves regarding all existing conditions that may affect performance of contract work. The contractor is solely responsible for ensuring that all measurements, labor and required material are accurate and covered in their initial bid of the project and will adhere to all All FBOP Technical Design Guidelines, seismic zone requirements, environmental compliances, Life Safety, NFPA, ADA-ABA, and Federal, State and Local codes.

MATERIALS AND METHODS:

2 DESCRIPTION OF WORK

2.1 The contractor shall remove the existing fire pump skid mounted unit and install an upgraded skid mounted unit with a new fire pump and fire suppression system, coupled with an independent fire pump soft start, via Variable Frequency Drive, for surge protection and ensure that the Pressure Reducing Valves (PRV) on the domestic side are operating properly. The domestic water pumps (jockey pumps) would be modernized and sequenced with the existing fire pump controller.

2.2 The contractor shall provide dumpsters or approved equal for demolition disposal.

2.3 The contractor shall verify all existing measurements & conditions prior to ordering materials.

2.4 Contractor shall ensure that all materials and installations are within guidelines for manufacturer specifications.

2.5 Contractor shall anchor the newly installed skid mounted system to an existing concrete pad in accordance with the manufacturer specifications and all FBOP Technical Design Guidelines, seismic zone requirements, environmental compliances, Life Safety, NFPA, ADA-ABA, and Federal, State and Local codes.

2.6 FCC Pollock will provide staff escorts for all work done inside the secure perimeter of the institution. The contractor will give as much advance notice as possible, but no less than two working days, as to when they will be working, so that scheduling arrangement can be made for by FCC Pollock staff.

2.7 Normal working hours are Monday through Friday, 7:30 am to 4:00 pm excluding Federal Holidays. In the event of weather conditions that present security problems, work will be delayed until Correctional Services authority decides the conditions have cleared. If similar weather conditions arise during the work day, work will be stopped and all contractors and equipment will be escorted out until conditions have cleared. All work will be coordinated with the COR (Contracting Officer Representative) and approved by the Facilities Manager prior to scheduling the work.

2.8 The Contractor shall provide FCC Pollock with a 2 hour notice prior to shutting off any utility services. Additionally, the contractor shall provide a proposed and pre-approved plan of action, 10 Working Days in Advance, to the COR and the Facilities Manager on the specifics of expected time lines, down times in utilities, and potential delays in the project. This is so FCC Pollock can make provisions and have contingency plans and equipment in place to minimize any disruption of services provided to the inmate population and fire protection response.

3 CONTARCTOR USE OF PREMISES

3.1 The contractor shall limit its use of the premise to the work indicated, so as to allow for Government occupancy and use. During the entire construction period, the contractor shall coordinate use of the premises for construction operations with the FBOP COR.

1. All visitors to the construction site shall register with FBOP site officials.

The contractor shall not allow employees to have personal visitors on site.

2. The project site will be open to the contractor Monday through Friday, with the exception of federal holiday, for the duration of the project. The project schedule is to be based on a normal 40 hours’ work week. The contractor shall establish a normal week schedule for the duration of the project:

a. Work outside the secure perimeter institution will be allowed between the hours of 7:00 AM to 4:30 PM. This will allow the Contractor’s work force to arrive on-site in the morning to prepare for entering the secure perimeter of the institution. It will allow the Contractor’s work force to button-up their laydown / staging area after exiting the secure perimeter at the end of the work day. Work conducted on site will be under constant escort from FBOP staff.

3. Contraband: The contractor shall inform his/ her employee(s) that intoxicating beverages, drug, weapons, etc., will not be allowed on the site.

Introduction of contraband shall be subject to the penalties prescribed by Title 18

USC 1791.

4. Explosives: The use of explosives are forbidden without written authorization from the FBOP Contracting Officer upon no less than thirty (30) days advance written request.

5. Communication Equipment: cell phones, pagers, and radios are not allowed within the secure perimeter.

6. Parking: parking areas, either on or off-site will be furnished for workmen by the contractor at a location approved by the FBOP COR.

7. Temporary Office: Any temporary field office used by contractor shall be located in an area approved by the FBOP COR.

8. The Contractor must submit his or her layout plan for staging, stockpiles, etc. to FBOP COR. The contractor shall maintain all staging and construction area in a neat and orderly fashion.

9. The Contractor shall provide protection of the work against weather, rain, wind, storms, frost, cold and or heat so as to maintain all work materials, apparatus’, and fixtures free from damage.

CONTRACTOR PERSONNEL AND EQUIPEMNT SECURITY

A. Contract employees working at FCC Pollock facility will be subject to a background investigation after award of the contract. Contractor employees must complete the applicable forms provided by the FBOP COR. Following the investigation, the FBOP will provide the contractor with a list of employees cleared to work at the institution.

If a contractor’s employee is not cleared, that fact only will be communicated to the contractor. No additional justification will be released. The following investigative procedures will be applied to contractor under the terms of the proposed contract:

A negative response will preclude an individual employee of the contractor from working within the institution.

B. Contractor employee’s working within the secure perimeter shall wear a contractor badge issued by the institution at all times, and shall be escorted by an institutional staff member.

C. Tools brought into the institution’s secure perimeter will be inventoried at the beginning and end of each work day by FBOP staff.

1. All tools used by the Contractor working within the secure perimeter shall be under constant supervision. Loss of a tool shall be reported immediately to the escorting FBOP staff.

2. No explosive action tools shall be used on the property at any time.

3. Material brought inside the secure perimeter of the institution shall be limited to a one day supply. Surplus materials that are not used/installed on work day must be moved outside the secure perimeter at the end of the day.

D. Contractor employees working on the FBOP site shall be required to undergo a general institution orientation on responsibilities, procedures, and laws governing conduct when working at an FBOP institution. These rules include, but not limited to the following:

1. Weapons and ammunition of any kind are not allowed on the Federal property, including the parking lot.

2. Do not give, or offer to give, anything to an inmate.

3. Do not bring any correspondence into or out of the institution.

4. Do not accept gifts or favors from inmates.

5. Leave personal items locked in your vehicle or secure them at the contractor’s lay-down area.

6. A form of photo ID is required to enter the facility.

7. Revealing clothing is not acceptable attire and work attire is required at all times.

8. Conduct yourself in a responsible manner.

9. Report any unusual actions(s) or behavior to FBOP staff.

E. Contractor shall have access to onsite restrooms and utilities.

EXECUTION OF WORK AND CLARIFICATION OF INFORMATION

A. It is the intention of the Construction Documents to provide a complete project. The Contractor shall be responsible for this result. The Contractor shall verify all measurements and be responsible for the correctness of the same. The Contractor shall layout their work and be responsible for all lines, elevations, and measurements of building, utilities, and other work executed by them. This includes verification of existing conditions, if any, that affect their work or to which their works to be fitted.

B. The Contractor shall carefully study and compare Contract Documents before proceeding with fabrication and installation of work. Coordinate scheduling, submittals, and work to ensure efficient and orderly sequence of installation.

C. Should it appear that the work intended to be described is not sufficiently detailed or explained by the Construction Document, the Contractor shall issue a Request for Information (RFI) to the FBOP Contracting Officer. RFI’s shall be used by the Contractor to clarify information in the Construction Documents only. Each RFI shall be numbered sequentially, dated and indicate a reasonable date for the Contracting Officer’s written reply. All RFI questions shall be thoroughly researched by the Contractor prior to presenting them as RFI’s. In no case shall any work proceed in uncertainty.

D. A pre-construction conference will be held at the jobsite before installation of the roofing system begins and will include the following:

1. The conference shall include the FBOP site Contracting Officer, COR, Contract Monitor, Safety Manger, Facilities Manager, Complex Captain and Contractor.

2. Discuss the methods and procedure of the roofing installation including the manufactures written instructions.

3. Review flashing, special roof details, drainage, roof penetrations, equipment curbs and any other conditions impacting roof repair.

4. Discuss methods of temporary protection during construction.

5. Discuss general roof inspection procedures.

6. Discuss procedures for testing and leak repair.

7. Review material list.

QUALITY ASSURANCE

A. Source Limitations: To the greatest extent possible for each unit of Work, and subject to restrictions of the Buy American Act, provide products, materials or equipment of a singular generic kind from a single source

B. Compatibility of Options: Where more than one choice is available for contractor’s selection of a product or material; options should be reviewed and coordinated through the COR, Facilities Manager and contracting officer for prior approval. Total compatibility among options is not assured by limitations within contract document, but must be provided by contractor. Compatibility is a basic general requirement of product/ material selections.

C. The components furnished shall be a product manufactured by a firm commercially producing a similar product for at least five (5) years, and shall meet All FBOP Technical Design Guidelines, seismic zone requirements, environmental compliances, Life Safety, NFPA, ADA-ABA, and Federal, State and Local codes.

The skilled workers shall have the required qualification, experience and certification as applicable.

PRODUCT DELIVERY/ STORAGE / HANDLING

Deliver, handle, and store product in accordance with manufacture’s recommendations and by the methods and means that will prevent damage, deterioration, and loss, including theft, and protect against damage from climatic conditions. Control delivery schedules to minimize long term storage of products at site and overcrowding of construction spaces. In particular, provide delivery/ installation coordination to ensure minimum holding or storage times for products recognized to be flammable, hazardous, easily damaged, or sensitive to deterioration, theft, and sources of loss. Damaged or defective items, based on the opinion of the Contracting Officer and the COR shall be replaced at no cost to the government.

WARRANTIES (GUARANTEES)

A. The warranty and guarantee provisions of the Contract clauses apply to all work of the contract. In accordance with FAR 52.246-21, Warranty of Construction, and in addition to any other warranties in this contract, the Contractor warrants that work performed under this contact conforms to workmanship performed by the contractor or any subcontractor or supplier at any time.

B. The Contractor will provide a no dollar limit warranty for minimum for thirty (30) years from date of Project Substantial Completion.

SUBMITTALS

A. Submit the required information of the persons who would perform the work inside the institution for background investigation. This includes Federal NCIC Background checks in compliance with H.S.P.D 12 requirements. Upon award the COR shall provide all required documentation to the project manager or contractors designated point of contact.

B. Submit manufacturer’s brochures, details, drawings, source certification for all material and equipment.

PROJECT CLOSEOUT

A. General: When the contractor considers the work substantially complete, submit the following to the Contractor Officer:

1. A written notice that work, or designated portion therefore is substantially complete, and request an inspection by the FBOP COR and the Contracting Officer.

2. Prior to the contractor requesting substantial completion, the contractor shall develop a punch list of incomplete and non-compliant items.

3. Within 30 days after receipt of such notice, the Contracting Officer and FBOP COR shall make an inspection to determine the status of substantial completion.

4. If the Contracting Officer determines that the work is not substantially complete:

a. Contracting Officer will promptly notify the contractor in writing, giving reasons therefore.

b. The contractor shall remedy deficiencies in the work and send a second written notice of substantial completion to the Contract Officer.

c. The Contracting Officer and the FBOP COR will re-inspect the work.

d. The contractor shall be responsible any additional inspections by the government that are not fault of the government.

5. When the Contracting Office determines that the work is substantially complete, the Contracting Officer will:

a. Request the FBOP COR will verify the contractor’s punch list.

The Contractor’s punch list may amended by the FBOP Contract Monitor with any open issues and any remaining items from the incomplete/non-compliant list.

b. Request the contractor to prepare a Certificate Substantial Completion on a form approved by the Contracting Officer, accompanied by the punch list.

This form will be used to record the established start dates for the warranties and guaranties. This document provides for agreement as to the time allowed for completion or correction of work remaining, the date upon which the FBOP institution will assume possession of the work. If issued after the contract completion date, the certificate establishes the date upon which liquidated damages cease.

B. Final Inspection and Acceptance: The Contractor shall diligently complete all punch list items before a Final Inspection is scheduled.

1. When the contractor considers the work complete, he/she shall submit a written certification to the Contracting Officer stating the work has been completed and inspected for compliance with the contract document; and the work is ready for final inspection. The contractor shall give the Contracting Officer a minimum of ten (10) work days advance written notice of the date on which the work will be fully completed and ready for government’s final inspection.

2. The Contracting Officer and the FBOP COR shall make an inspection to verify the status of completion with reasonable promptness after receipt of such certification.

3. Should the Contracting Officer consider that the work is incomplete or defective:

a. The Contracting Officer will promptly notify the Contractor in writing, listing the incomplete or defective work.

b. The Contractor shall take immediate steps to remedy the stated deficiencies and send a second written certification to the Contracting Officer when the work is completed.

c. The Contracting Officer and the FBOP COR will re-inspect work.

d. The contractor shall be responsible for any additional inspections by the government that are not the fault of the government.

e. The Contracting Officer will assign monetary value to all punch list items not completed, and to all required submittals not received, as of the date of final acceptance. The total value of those items shall be retained and/ or deducted from the contractor’s final payment until the contractor demonstrates to the Contracting Officer satisfaction that such items have been completed or corrected.

4. When the Contracting Officer finds that work is acceptable under the contract documents, the Contracting Officer will request the contractor to complete all remaining closeout submittals and contract requirements, including but not limited to; operating and maintenance manuals, record documents, warranties, guarantees, certificates, clearances, and bonds.

5. When all work is acceptable, the Contracting Officer will issue a Certificate of Contract Completion, GSA Form 218, or other approved form to the Contractor for signature.

C. Final Application for Payment: After the work is fully accepted by the Contracting Officer, the Contractor shall submit the final application for payment in accordance with procedures and requirement stated in the Contract requirements. As required by the Contracting Officer, the final application for payment shall include affidavits that all bills have been paid. The Contractor’s submission of the final application for payment shall signify that the work is complete, and the Contractor has vacated the project work site.
