About this file
This document is a federal contract amendment (Amendment 0001) and a past performance questionnaire for solicitation 140P5125Q0024 for asbestos abatement services at Cape Hatteras National Seashore. The amendment, effective 06/02/2025, revises the past performance questionnaire (provided as attachment 0008) and establishes a performance period from 07/01/2025 to 10/30/2025.
The project requires removing loose lead-based paint, restoring exterior and interior building finishes, removing asbestos flooring and particle board subflooring, installing new plywood subflooring, and moving building contents to and from temporary storage. The past performance questionnaire is designed for references to evaluate the contractor's performance across dimensions such as overall performance, timeliness, ability to resolve problems, quality of work, and overall rating. The questionnaire uses a rating scale from Exceptional to Unsatisfactory and provides space for detailed comments from previous contract references.
