STATEMENT OF WORK

Occupational Therapy/Speech – Language Therapy

1.0 BACKGROUND/PURPOSE

The Cheyenne – Eagle Butte School is a Bureaus of Indian Education (BIE) operated school located in Eagle Butte, South Dakota. As a BIE operated school, Cheyenne – Eagle Butte School is responsible for providing a high quality general education and Special Education programming. The Cheyenne – Eagle Butte School Special Education Department is seeking both a State of South Dakota and Nationally certified Occupational Therapist and a State of South Dakota Board certified Speech – Language Therapist. The purpose is to provide continuing uninterrupted therapy to students already identified as needing occupational therapy or speech – language therapy.

1.1 SCOPE OF WORK

The service provider shall provide all personnel, supervision, materials and other items and services necessary to effectively evaluate and provide therapy to the students at the Cheyenne – Eagle Butte School Special Education Department. In addition the service providers will have the necessary medical licensure as required by the State of South Dakota. In addition the Occupational Therapist must hold National Certification from the Osteopathic Medical Examiners. It is imperative that the Occupational Therapist and Speech Language Therapist is well versed in the State of South Dakota and Federal Special Education laws, rules, regulations and eligibility requirements.

1.2 SERVICE PROVEDER PERSONNEL

The purpose of this contract is to acquire the services of both a South Dakota and Nationally certified Occupational Therapist and a South Dakota certified Speech – Language Therapist. It is imperative that each individual possesses both state and/or nation certification at the time of application. The Occupational Therapist must be licensed through the National Board of Osteopathic Medical Examiners. The therapists should have experience working in reservation schools. The Speech – Language Therapist will need to be highly qualified within the State of South Dakota and possess the proper licensure/state certification. The service providers must be able to provide 1 day per week on site when school is in session. It is imperative the contractors be able to work collaboratively with the Special Education Coordinator/School Supervisor, school staff, students and parents. Upon notification from school required to be available for consultation when not on – site. Speech/Language services are a highly sensitive area and the need of continuity of services is critical.

1.3 GOAL

The overarching goal is to conduct comprehensive occupational and speech – language evaluations as required by the Individuals with Disabilities Education Improvement Act of 2004 by a highly qualified State and/or National Certified therapists. In addition the speech – language provider will complete all required State of South Dakota evaluations. All evaluations will be completed within established State and Federal timelines for the Cheyenne – Eagle Butte School to fulfill all legal requirements and remain in compliance. In addition to the evaluations individual student therapy must be consistently provided based on individual student needs.

1.4 OBJECTIVES

To provide individual occupational and speech – language therapy to special education students as required through their Individual Education Plan (IEP) and mandated under the Individual Disabilities Education Improvement Act of 2004.

1.5 TASKS

· Provide ongoing therapy to special education students currently in treatment as identified by their Individual Education Plan (IEP)

· Provide comprehensive Occupational Therapy Evaluations as designated by Special Education Coordinator/School Supervisor

· Provide comprehensive Speech – Language Evaluations as designed by Special Education Coordinator/School Supervisor

· Maintain Individual Education Plans according to the Local and State regulations

· Provide 1 day per week of on – site therapy for Occupational Therapy and Speech – Language each

· Consultation with Teacher Assistance Team/problem solving team as requested by Special Education Coordinator/School Supervisor

· Provide prompt completion and delivery of evaluation reports

· If requested to will attend all Initial Eligibility meetings to review evaluation results with parents and staff

· All evaluations will be completed within 25 school days as required by South Dakota State law

· All evaluation reports will be completed with 30 calendar days as required by South Dakota State Law

· Provide on – site training to school staff and/or parents on topics related to area of expertise upon request from Special Education Coordinator/School Supervisor

· Willing to attend and participate in trainings and workshops requested by Special Education Coordinator/School Supervisor, pending schedule and availability

· Recommend therapeutic equipment to be used with individual students based on individual student therapy needs

1.6 DELIVERY

To provide 1 day per week of on – site occupational therapy and speech – language therapy services; each, during the school year when school is in session as determined to be the need by the school.

1.7 PLACE OF PERFORMANCE

Contractor’s work is to take place on the Cheyenne – Eagle Butte School campus, contractor’s office, or other location as determine and approved by the Special Education Coordinator/School Supervisor.

1.8 PERIOD OF PERFORMANCE

Contractor is expected to complete all defined tasks between the Cheyenne – Eagle Butte School calendar for SY 18 – 19; August 2018 – August 2019.

1.9 SECURITY REQUIREMENTS

Contractor’s staff is required to check in with Special Education Coordinator/Administrative Office prior to entering the school campus. The contractor’s staff will be required to adhere to local requirements regarding computer/internet use.
