EXHIBIT B - PAST PERFORMANCE REFERENCE LETTER AND QUESTIONNAIRE
Prepare and send the following reference survey for each project listed in Part I of the Performance Relevancy Questionnaire. Be sure to include the name of your company at the top of each page of the survey. For Government contracts, send to Contracting Officer or Technical Representative. For commercial references send to personnel with duties similar to those for Government contracts. It is your responsibility to follow-up and to encourage your references to send in their survey. If you have multiple references at one location, send one cover letter and survey for each contract you want a reference for.

We have listed your firm as a reference for the work we have performed for you as listed below. Our firm has submitted a proposal for a project advertised by the USDA Forest Service. In accordance with Federal Acquisition Regulations (FAR), they will evaluate our firm's past performance. Your candid response to the attached survey will assist the evaluation team in this process. We understand that you have a busy schedule, and your participation in this evaluation is greatly appreciated. Please complete the enclosed survey as thoroughly as possible. Space is provided for comments. Understand that while the responses to this survey may be released to the offeror, FAR 15.306 (e)(4) prohibits the release of the names of the persons providing the responses. Complete confidentiality will be maintained. Only one response from each office is required.
Please send your completed survey to the following address to arrive NOT LATER THAN 2 P.M., MAR 22, 2019. DO NOT RETURN THEM TO OUR COMPANY.

E EXCEPTIONAL: Performance met contractual requirements and substantially exceeded most (requirements). Any problems encountered resulted in corrective actions taken by the contractor which exceeded expectations and were highly effective. Contractor consistently performed at the highest level.
V VERY GOOD: Performance met contractual requirements and exceeded some
(requirements). Any problems encountered resulted in corrective actions taken by the contractor which were effective.
S SATISFACTORY: Performance met all minimum requirements. Any problems encountered resulted in corrective actions taken by the contractor which appear or were satisfactory.
M MARGINAL: Contractor met contract requirements with minor government agency resource oversight or assistance. Performance appeared weak in meeting all minimum contractual requirements.
P POOR: Performance may not have met minimum contractual requirements or nonconformance jeopardized the achievement of contract requirements. Performance necessitated major government agency oversight or assistance.
N NEUTRAL: Relevant past performance does not exist or information is not available.
Offeror is not evaluated favorably or unfavorably.
Using the codes above, circle the appropriate letter for each item on the questionnaire and record any comments.
QUALITY OF WORKMANSHIP
• Rate the contractor's compliance with contract terms and conditions and statement of work.

• Did the contractor provide adequate, competent and qualified management, key personnel and technical personnel capable of meeting contract requirements throughout the performance period of the contract?
• How well did the contractor work independent of agency guidance, oversight and assistance?
• How effective was the contractor in meeting Cost/Price performance targets and controlling costs (i.e. changes, etc.)? Did they demonstrate reasonableness in modifications scope and costs?
• Were subcontractors/tradesmen adequately managed and coordinated?
Explain any subcontracting issues (positive or negative) that impacted the performance of your contract(s).
CUSTOMER SATISFACTION
• How reasonable and cooperative was the contractor during performance?
• How committed was the contractor to customer satisfaction?
TIMELINESS OF PERFORMANCE
• How well did the contractor adhere to the agreed-to schedule?
