1. FAR 52.204-24 states “The Offeror shall not complete the representation at paragraph (d)(1) of this provision if the Offeror has represented that it "does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument" in paragraph (c)(1) in the provision at 52.204-26, Covered Telecommunications Equipment or Services—Representation, or in paragraph (v)(2)(i) of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items. The Offeror shall not complete the representation in paragraph (d)(2) of this provision if the Offeror has represented that it "does not use covered telecommunications equipment or services, or any equipment, system, or service that uses covered telecommunications equipment or services" in paragraph (c)(2) of the provision at 52.204-26, or in paragraph (v)(2)(ii) of the provision at 52.212-3.”

The document (see attachments) talks about what is in the FAR 52.204-24. Number one, FAR 52.204-24 is a required document because it is new. All other clauses are considered part of the solicitation and will be art of the contract. At this time no other documents are required to be filled out. Vendors should certify their products.
2. Please confirm, that as referred to FAR 52.212-1, offer may be submitted on company letterhead.
Offers can but do not have to be submitted on company letterhead.
3. On the link provided to the SAM website, under “DESCRIPTION”, it indicates “Each offer must show a breakdown giving price of each element, provide descriptive literature or information showing that the offered service meets all statement of work.” Please define element. Does “element” refer to each level of service to be provided per year, or to simply each year’s total price?
Elements are represented by the specifications and/or statement of work. If it is not clearly defined, then the vendor should present something that they believe fits the specifications and/or statement of work. Unless we specifically identify something, it is left up to the vendor to determine. It is always best to spell out as much as they can to ensure they are presenting a thorough estimate.
