Attachment 001 Performance Work Statement and QASP
Project: Annual Service Contract and Preventative
Maintenance for Laboratory Equipment
GENERAL INFORMATION
1. Scope of Work: This Statement of Work is to for an annual service plan and an initial preventative maintenance visit for two pieces of laboratory equipment currently at the
National Bio and Agro-Defense Facility in Manhattan, Kansas. The pieces of equipment included are – nCounter Pro, DA_2210E0076 + PS_2205F1070
2. Background: The Agricultural Research Services at the National Bio and Agro-Defense
Facility (NBAF) requires a Device Service Plan. The National Bio and Agro-Defense
Facility is a USDA facility currently operating at BSL-1 and BSL-2. The equipment is currently located in a laboratory operating at a BSL-1 level. Security clearances will be required for all personnel arriving onsite to provide preventative maintenance.
CONTRACTOR REQUIREMENTS
3. Technical Requirements/Tasks:
3.1 Provide an initial preventative maintenance visit for a nCounter Pro, DA_2210E0076 +
PS_2205F1070 at the National Bio and Agro-Defense Facility. This visit only occurs in the base period and shall serve as the annual preventative maintenance visit requirement indicated in section 3.2.1. for the base period.
3.2 Provide annual service plan for all CLIN periods that includes:
3.2.1 One annual preventative maintenance visit.
3.2.2 Plan shall include any and all repair coverage including parts, travel, labor, S&H, and any other expenses for the repair.
3.2.3 Plan shall cover replacement reagents for any reagents lost due to an instrument issue.
3.3 Service report for all service visits to be delivered to the USDA technical point of contact within five business days of any inspection visit.
4. Government Furnished: The vendor will work with the Technical POC to determine equipment and tools needs and determine what is readily available on site at the National
Bio and Agro-Defense Facility (NBAF), and what the vendor will need to supply.
5. Travel: The contractor will be required to travel to NBAF. The facility is currently operating at BSL-1 and BSL-2 levels and all necessary protocols will apply to all contractor personnel while on site. Required personnel protective equipment (PPE) will be provided to contractor personnel while on site.
6. Contractor’s Key Personnel: The contractor shall provide a repair service technician that meets security requirements detailed in section 7 below and is authorized by the OEM manufacturer of the serial numbered equipment detailed in Section 3.1 above to perform and document the service requirements.
7. Security Requirements: The service technician shall undergo a background security check to be completed at least 7 days prior to arriving at the NBAF. NBAF will fund and conduct the background check. PII required of the technician for the background check: SSN, credit history, birthday, age, home address, and proof of USA citizenship. Technicians will be escorted at all times while on site.
8. Quality Assurance Surveillance Plan (QASP). The Government shall establish a QASP as a means of evaluating whether the Contractor is meeting the performance standards/quality levels identified in the PWS to ensure that the government pays only for the level of services received.
