Performance Work Statement
Flooring Removal and Installation Services
DM
DAVIS-MONTHAN AFB, 85707
1.1. Description of Services. The contractor shall furnish all supplies, equipment, materials, tools, personnel, labor, transportation, supervision, and management necessary to remove existing linoleum, carpet, padding and wall base and replace with new resilient vinyl tile and new carpet, padding, to include transitions and wall base at the DM on Davis-Monthan Air Force Base (DMAFB), AZ in accordance with manufacturer’s specifications, industry standards, and business practices. Measurements provided by the Government are estimated and the contractor is ultimately responsible for determining exact measurements. The contractor is responsible for verifying material quantities and office layouts.
1.2. Schedule. Upon contract award, the contractor coordinate the date and time for flooring installation with the Government Point of Contact (POC) and Contracting Officer. Otherwise agreed upon by the contracting officer. The Contractor shall provide a project schedule showing all tasks and proposed duration at time of quote submission. A detailed project schedule shall be submitted five (5) business days from award specifying specify demolition and installation by task and room number/location. The schedule shall be approved by the Contracting Officer and the Government POC. Work shall be completed within a timeframe agreed upon by the Contractor and Contracting Officer. The contractor shall complete the installation flooring on weekends to have the least amount of impact to the program. No more than two rooms shall be out of service at one time while completing work.
1.3 Working Days/Hours. The Contractor shall perform services between the hours of 7:30 a.m. - 4:30 p.m., Saturday – Sunday only. A employee will be on site during weekend work performance. Weekday work will only be authorized may be considered on a case-by-case basis upon approval from Contracting Officer and in coordination with the Government POC, as long as it is at no cost to the Government.
1.4 Waste Removal. The Contractor shall remove all waste and debris from the work site and place it in Contractor provided disposal containers. The Contractor shall dispose of waste at a permitted waste site. Upon completion of services, floor and surrounding area shall be completely free of all waste. No waste shall be disposed of on DMAFB

The contractor shall remove and dispose of all existing linoleum, carpet, padding, wall base and rubber flooring, ensuring not to disturb adjoining flooring. Square footage for each room is outlined below for removal and disposal of existing linoleum, carpet, padding and wall base and rubber flooring. The contractor shall provide temporary movable storage on site.

1.2. Installation of Linoleum, Carpet, Padding, Transitions, Wall base and Rubber flooring: The contractor shall prepare floor surfaces to ensure proper installation of the new material. Square footage of carpet, padding, linoleum, wall base and rubber flooring is to be identical in all rooms with the exception of INF-B Room 130-B, PTA Room 126-A, PTB Room 126-B, PTC Room 120-A, and PTD Room 120-B. Square footage of carpet, padding, wall base, rubber flooring and linoleum should be followed according to diagrams in Appendix 1 “Reconfigured Flooring”. Carpet for activity rooms must meet guidance 01-10-10, 9.4.13 Child activity rooms; general requirements for child activity rooms are as follows; soft flooring surfaces such as carpet from safelandings shell be specified.

1.2.1 Skim Coating. The contractor shall skim coat the entire floor with cementitious floor patch to provide barrier from any remaining abatement chemicals and to give the floor the smoothness required for carpet installation.
1.3. Project Specifics. Carpet Padding. The contractor shall remove and dispose of all existing carpet padding and install new carpet padding. The contractor is responsible for confirming all measurements and for verifying critical fall height has been met. Edges should be ramped in such a way as to ensure there are no tripping hazards created with the new carpet and the existing linoleum. New carpet system shall: meet CPSC Guidelines; Meet ASTM F 1292 and ASTM F 2373 at minimum 3ft critical fall height; Meets ABA requirements; Impact-absorbing mat/padding; Contains Anti-microbial properties; Solution dyed; Integrated moisture barrier;
CRI Green Label; Contains Zero VOC’s; Conventional Cleaning Process; Integral wall base system. As outlined in the 1-10-10 and the UFC 740-14
Soft flooring surfaces shall be a shock-absorbing carpet system with integral moisture barrier and base. The contractor is responsible for confirming all measurements and for verifying critical fall height has been met. The contractor shall include the testing requirement with a Triax and produce the certification that carpet was tested after installation and that it meets the 3’ critical fall height. Edges should be ramped in such a way as to ensure there are no tripping hazards created with the new carpet and the existing linoleum. New carpet system shall: meet CPSC Guidelines; Meet ASTM F 1292 and ASTM F 2373 at minimum 3ft critical fall height; Meets ABA requirements; Impact-absorbing mat/padding; Contains Anti-microbial properties; Solution dyed; Integrated moisture barrier; CRI Green Label; Contains Zero VOC’s; Conventional Cleaning Process; Integral wall base system. Colors shall be one from the Southwest Color Pallet referenced in Appendix A from the 1-10-10 (pending receipt of Appendix A from SVI HQ). As outlined in the 1-10-10 and the UFC 740-14
Rubber Tile Flooring (Corridors, Toileting and other high traffic areas requiring resilient flooring)

Contractor shall provide appropriate sample per Carpet, Vinyl, Tile, and Rubber Flooring Requirements. 01-10-10 Section 9.3.1 Structural Interior Design (Section 01-10-10 supplements UFC 4- 740-14).
Adhesives. Adhesives utilized by the contractor shall not seep upon installation or throughout the life of the new flooring and wall base.
must be performed on the weekends for which rooms will be ready for operation on Monday morning. Areas that have been recently painted, carpeted, tiled, or otherwise renovated shall be well ventilated before they are used by children. Additional time may be available during the weekday with advance coordination for an exact schedule with Government personnel to confirm access to areas with children present, for building access, and to arrange for Government escorts if required. Upon contract award, the Government will provide the contractor with procedures to access facilities. Contractors are required to sign in and out of the facility every entry and exit.
All contractors and personnel involved with the installation will be required to pass a criminal background check and be granted access to the base
1.6. Government Notification. Upon arrival to DMAFB to perform services, the contractor shall call the Security Forces Control Center (SFCC) at (520) 228-3200 to announce their arrival and report in which building they will be performing services. At the end of each day, and before departing DMAFB, the contractor shall call the SFCC to report their departure. The contractor shall follow these procedures each time they arrive and depart DMAFB to perform services. The contractor shall immediately call the SFCC in the event the contractor accidentally sets off an alarm.
1.7. Building Access. A building custodian or 355th Civil Engineer Squadron (355 CES) personnel will open and close the facilities for contractor access and remain with the contractor if work is to be performed on the weekends.
1.8. Work Practices. All flooring shall be installed with the colors, patterns, textures, and other manners in accordance with the measurements taken by the contractor and the instructions furnished by the Government and verified by the contractor. Installed products shall follow all manufacturers' specifications, recommendations, industry standards, and business practices necessary to ensure flooring is level, even, and/or consistent with the system being installed.
1.9. Manual and/or Warrantee Documents. Upon completion of carpet and tile installation, the contractor shall provide the COR with two copies of all necessary maintenance manuals and warrantee documents.
2.0. General Information
2.1. Contract Manager (CM). The Contractor shall designate a contract manager (CM), who will be responsible for contractor-performed services. Upon contract award, the Contractor shall submit the name and telephone number of the CM, in writing, to the Contracting Officer. The CM shall have full authority to act for the Contractor on all matters relating to daily execution of the contract and must be available by phone to respond to Government service calls, during regular business hours.
2.2. Safety. The contractor shall comply with all applicable laws and safety regulations.
Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor's compliance or noncompliance with such responsibilities. The contractor is responsible for providing contractor employees with all necessary and required personal protective clothing and equipment.
2.2.1. Fire Prevention and Safety Standards. Contractor employees shall comply with the Occupational Safety and Health Act (OSHA), Davis-Monthan AFB Fire Protection Program, Air Force Occupational Safety, and the Fire Prevention and Health Program.
2.2.2. Work Area. The contractor shall, at all times, keep work areas free from accumulations of waste material, rubbish, tools, scaffolding, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition, and shall not leave any tools, materials, or equipment in the The Government will not provide the contractor with any storage space.
2.2.3. Smoking In AF Facilities. Contractors are advised that the AF has placed restrictions on the smoking of tobacco products in AF facilities. AFI 40-102, Tobacco Use in the Air Force, outlines the procedures used by the commander to control smoking in our facilities. Contractor employees and visitors are subject to the same restrictions as DMAFB personnel. Smoking is permitted only in designated smoking areas.
2.3. Illegal Substances. Illegal substance possession and/or use will not be tolerated on DMAFB, including medical marijuana. The contractor shall ensure that contractor employees who have base access do not possess and/or use illegal substances. All contractors and subcontractors are required to adhere to all local, state and federal laws concerning possession and/or use of illegal substances. Contractors found to be in possession of illegal substances will be barred from the installation, and subject to arrest and prosecution by local authorities.
2.4. Contractor Employee Restrictions. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the Contracting Officer as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
2.4.1. Illegal Aliens. In accordance with Air Combat Command (ACC) supplement to AFI 31- 101, The Air Force Installation Security Program (FOUO), illegal aliens are unauthorized on DMAFB. The contractor shall not hire illegal aliens to perform contract services on DMAFB, nor attempt to bring illegal aliens on the installation. Corrective/punitive action will be taken against the contractor and the illegal alien if the contractor hires illegal aliens to perform contract services on DMAFB.
2.4.2. Contractor Travel on DMAFB. Contractor employees shall not loiter in any working area before or after performing services. All contractor employees shall limit their travel on DMAFB only to the specific areas required for performance of services or in travel directly to and from these locations. Upon completion of contract services, contractor employees shall promptly depart DMAFB.
2.4.3. Contractor Employee Conduct. The Government reserves the right to require removal from the installation any contractor employee who endangers persons or property, whose actions are inconsistent with professional conduct, whose continued employment is inconsistent with the interest of military security or who is found to be incapacitated or under the influence of alcohol, drugs, or other substances. Removal of employees for any reason does not relieve the contractor of the requirement to perform services specified herein.
2.5. Security Requirements
2.5.1. List of Employees. Upon contract award, the contractor shall submit to the Contracting Officer a list of contractor employee names, social security numbers, and driver's license numbers, or other means of positive identification of all employees performing services on DMAFB. Foremen, supervisors, and superintendents must be identified on the list of employees.
The contractor shall submit an updated list when an employee's status or information changes.
The name of any contractor employee that is terminated during the period of the contract must be reported to the Contracting Officer, in writing, within one workday of the employee's termination.
2.5.2. Safeguarding of Classified and Other Information. The contract does not involve the disclosure of classified information, however, any information of a confidential nature, records or employees files that may be observable during performance of services shall not be perused by contractor employees for any reason. The contractor shall ensure that contractor employees not notify, repeat, comment upon, disclose, or use in any way information seen, observed, or derived from documents or conversations of a sensitive or confidential nature overheard by them.
The contractor shall comply with and assume responsibility for compliance of this provision by its employees.
2.5.3. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance on DMAFB are obtained for employees and non- Government owned vehicles.
2.5.4. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires;
e.g. terminated for cause, retirement, etc., or upon contract completion. The contractor shall notify the Contracting Officer in writing when an employee departs, and return the employee's identification to the office that issued it.
2.5.5. Traffic Laws. Contractor employees shall comply with all DMAFB traffic regulations and state traffic laws. Contractor employees are subject to random vehicle speed control checks.
Failure to adhere to base traffic regulations may result in the loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones is strictly prohibited while driving on DMAFB, unless the phone is hands free. Seat belt use is mandatory for all drivers and passengers of vehicles.
2.5.6. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base. Denial of entry to contractor personnel who refuse to be searched, or detention of personnel found to be in possession of contraband, may result in loss of base driving privileges, debarment from the base, or other administrative action. Detention of contractor employees for any reason does not relieve the contractor of the requirement to perform contract services.
2.5.7. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.
2.5.8. Reporting Requirements. Contractor employees shall immediately report to an appropriate DMAFB authority any information or circumstances of which they are aware that may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.
2.5.9. Physical Security. Contractor employees are responsible for safeguarding all Government property and controlled forms provided for the contractor's use. At the end of each work period, the contractor shall secure all DMAFB facilities, equipment, and materials.
2.5.10. Contract Authority. No person other than the Government Contracting Officer has the authority to make any changes to the contract. Authority from the Contracting Officer to the contractor to make changes will be in the form of an official, signed contract modification. If the contractor is asked to make any changes to the contract terms and conditions, the contractor shall notify the Contracting Officer immediately.
2.6. Environmental Protection Compliance
2.6.1. Adherence to the Davis-Monthan Air Force Base Contractor Environmental Guide.
Adherence to this guide will help increase your company's awareness of the installation's Environmental Management System (EMS), and the environmental regulatory requirements that impact DMAFB.
2.6.1.1. Environmental Management System (EMS) Awareness Training. The contractor shall comply with the DMAFB Contractor Environmental Guide in completing the required EMS Awareness Training prior to start of work. The Contractor Environmental Guide will be made available by the Contracting Officer.
2.6.1.2. Environmental Laws and Regulations. The contractor shall comply with all applicable Federal, State, and local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations/requirements change during the term of the contract, the contractor shall comply with such law/regulation as changed. The contractor is responsible for any penalties, fines, etc. resulting from violations. The contractor shall immediately notify the COR when violations occur, and cooperate fully with fire department personnel and Government investigators.
2.6.1.3. Environmental Permits. Contractors shall comply with all regulatory requirements to include permits if applicable. Pima County Department of Environmental Quality (PDEQ) and Arizona Department of Environmental Quality (ADEQ) require permits for asbestos removal or demolition as well as projects that require earth-moving or soil disturbance. These requirements may include storm protections. The contractor shall treat areas subject to dust producing activities with liquid palliatives which will not harm regrowth of vegetation, or other methods of dust control. The contractor shall also ensure methods to protect native plants, birds and wildlife are included in projects where required. All permit costs shall be considered as having been included as a factor in the contractor's bid price.
2.6.1.4. Hazardous Materials/Waste. The contractor shall be responsible for the proper management of both hazardous and non-hazardous waste. The contractor shall segregate hazardous or regulated materials and non-hazardous or non-regulated materials and handle, transport, store, and dispose of those materials, in accordance with the Resource Conservation and Recovery Act, all applicable federal, state, and local statutes and regulations, and IAW Appendix C, Hazardous, PCB, and Universal Waste: Proper Management and Disposal, of the Contractor Environmental Guide. The contractor shall coordinate all hazardous shipments with the 355 Civil Engineer Squadron Hazardous Waste Program Manager (355 CES/CEIE) at 228-4840.
2.6.1.5. Hazardous Material (HAZMAT) Handling. The contractor shall have approval from Hazardous Waste Program Manager (355 CES/CEIE) at 228-4840, in accordance with the Contractor Environmental Guide, prior to handling, using, and storing any chemicals, solvents, lubricants, and other products that require Safety Data Sheet (SDS) or Material Safety Data Sheet (MSDS) on the installation. The contractor shall plan, fill out, and submit a HAZMAT worksheet to the COR prior to bringing HAZMAT on the base - see Appendix A, Guidance For Submission Of Safety Data Sheets For Hazardous Material (HAZMAT) and Reporting of Hazmat Usage, and Attachment(s) A-1 to A-3 of the Contractor Environmental Guide for details. The contractor shall maintain one copy of the MSDS's for each HAZMAT line item used within the work center.
2.6.1.6. Prohibited Substances/Chemicals. The contractor shall not use products that are, or contain Extremely Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio-accumulative and Toxic (PBT) chemicals. Any HAZMAT containing one of these banned substances will not be allowed on base. Contact the environmental office (355 CES/CEIE) at 228-7201 if you have any questions on materials.
2.6.1.7. Hazardous Materials/Waste Spills. The contractor shall take every reasonable precaution to prevent spills of HAZMAT, and immediately report any HAZMAT spills to the base fire department by calling 228-3333, in accordance with Appendix J, HAZMAT Incident Response Procedures, of the DMAFB Spill Prevention, Control, and Countermeasure (SPCC) Plan. The contractor shall comply with this plan when handling and storing all oils and fuels. The contractor shall clean and dispose of all HAZMAT spills in accordance with local, state, and federal laws and regulations and the Contractor Environmental Guide. The contractor shall bear full financial obligation incurred from cleaning and disposing of HAZMAT spills, or release of HAZMAT within the confines of DMAFB by contractor employees.
2.6.1.7.1. Spill Containment Kits. The contractor shall ensure that all vehicles used for transport of HAZMAT on DMAFB, carry a spill containment kit capable of containing a spill, until first responders arrive. The contractor shall make spill containment kits available for inspection by Government personnel, upon request.
3.0. Government Provided Services
3.1. Emergency Services. DMAFB will provide security forces services (call 228-4444, for emergencies only), and fire protection services (call 228-3333). There are no emergency medical services available on Davis-Monthan AFB, therefore the contractor must dial 911 for emergency medical services.

4.1. Appendix 1 - Rooms/Area Sizes. The following rooms/areas sizes are estimates only, and the contractor is ultimately responsible for exact measurements.

Moving of office and classroom furnishings, Single mobilization. Rooms and storage sheds not being utilized at the time of installation will be used for temporary location of office and classroom furnishings.
