Field Service and Installation Terms and Conditions Statement

SWS VIDMARLISTA

Field Assembly: (9) sections of extended height 2K Adjustable Racking as listed on quote Q-155552 and shown on drawing DPS- 13155699.

Labor: Regular Wage Mechanical Erection Normal Working Hours, 5 days / week, Sat/Sun and Holidays Excluded

Field assembly start date to begin at a mutually agreed upon time.

Stanley Vidmar Responsibilities:

▪ Field Assembly will be performed or arranged by SWS VidmarLista

▪ Attend any required Safety Meeting – Not to exceed 1 hour

▪ Provide Forklift and Scissor Lift

▪ Unload, stage and store equipment inside building, within 100’ of erection site inside building (same level)

▪ Unpack equipment & take inventory of shipped parts – inform home office immediately of missing parts

▪ Check floor variation prior to start of field assembly to determine if additional shims are required

▪ Evaluate floor - level & plumb proposed equipment

▪ Provide anchoring hardware and shims for variance not exceeding 1”

▪ Provide mechanical erection of the proposed equipment

▪ Consolidate all debris to pre-determined location on site

▪ Assemble per manufacturer’s recommended assembly instructions

▪ Arrange customer inspection of material and obtain signed completion acceptance sheet prior to leaving job site.

▪ Provide appropriate PPE

Customer Responsibilities:

▪ Provide clean, clear erection site with necessary utilities

▪ Provide designated POC to assist with questions regarding install as necessary

▪ Purchase shims for variance in excess of 1”

▪ Locate pallets, pallet structures and/or brackets into racking

▪ Assemble accessories including tub kits, partitions, dividers, rear lip features, etc.

▪ Inspect assembly (immediately upon completion) & sign completion/acceptance form while crew is on site

▪ If required, prepare background checks with information provided on the technician

▪ Remove all debris off site

▪ Building permit fees, plan check fees and fees for professional engineer's seal on drawings and calculations are not included in the preceding system price and are the responsibility of the customer.

▪ Appropriate site conditions including necessary utilities to perform the work, accessible paved roads, appropriate dock availability, regular and uninterrupted access to the erection site during normal project site hours. Area must be vacated of all other trades.

Field Assembly: This is firm fixed price based on an unrestricted, continuous operation, free of work stoppages. All parts must be available and in good condition. If a work stoppage occurs due to the customer or additional parts are required to finish the field assembly, additional charges may apply and a formal purchase order will be required for the work to continue.

NOTE: If new construction or renovation, building must be complete and area must be vacated of all trades for field assembly to start

NOTE: Any revisions that are required after receipt of the purchase order as a result of information not provided in the quoting or proposal stage such as but not limited to; doors, columns, electrical panels, low ceilings or any other obstructions, will result in additional costs that will be passed on to the customer/dealer.

Equipment is to be assembled in strict accordance with the manufacturer’s assembly instructions. Any deviation may void the warranty.

Installation price is based on SWS VidmarLista recommended anchoring procedures. Any additional costs associated with anchoring, labor, and material outside manufacturer’s recommended methods may apply. (Engineered analysis, local jurisdiction requirements etc..)

THIS QUOTATION IS FOR YOUR COMPANY, NOT YOUR GENERAL CONTRACTOR, END USER OR OTHER THIRD PARTY.

PRICE QUOTED WILL ONLY BE VALID AND ACCEPTED ON PURCHASE ORDER FROM A QUOTED PARTY. THIRD PARTY PURCHASE ORDERS WILL NOT BE ACCEPTED. THIRD PARTY COMPANIES MUST REQUEST A NEW QUOTE.
