About this file
The document is an Affirmation and Disclosure Form, revision dated March 2022, used by state government contractors to affirm compliance with executive orders regarding offshore services and data locations. The form requires contractors to disclose the principal business location, service performance locations, and specific locations where state data will be stored, both for the primary contractor and any subcontractors. Contractors must provide detailed address information and are obligated to immediately notify the state of any changes in service or data location.
The form emphasizes strict restrictions on offshore services, particularly those involving Russian institutions or companies. Contractors must affirm understanding that the state retains the right to immediately terminate the contract if services are performed outside the United States without a formal signed waiver. The form serves as a critical compliance mechanism, requiring authorized contractor representatives to acknowledge and sign, thereby incorporating these location and service disclosure requirements into the potential contract with the state.
