Statement Of Work Design and Installation of Commercially Available and/or Engineered Portable Confined Space and Fall Protection Systems Associated with Subteranian Seawater Pump Station, Aeration Vaults and Cooling

Tower Make-up Water Vaults Northeast Fisheries Science Center Sandy Hook Laboratory

74 Magruder Road, Highlands, NJ

PART 1 GENERAL

1.01 DESCRIPTION

A. The Northeast Fisheries Science Center (NEFSC) Sandy Hook Laboratory is located at 74 Magruder Road, Highlands NJ 07732.

B. Work includes but is not limited to the installation of properly engineered, rated and equipment mounts/anchor points certified as compatible by the manufacturer of the portable confined space/fall protection davit system(s) and fall arrest posts. In order to provide maximum flexibility of use at the facility’s varied confined spaces, the portable systems shall include one and two piece adjustable masts with differing and variable offsets. A rail-based ladder climbing safety system will also be required. All installations must be approved to local standards by a licensed engineer. All installations shall enable the portable space/fall protection/fall arrest systems to be properly used in accordance with the manufacturer’s instructions.

C. The Contractor and/or subcontractor shall be normally engaged in the design and installation of fully OSHA-compliant confined space and fall protection safety systems including rated concrete work and anchor points, emergency retrieval equipment, ladder climbing safety kits, and fall restraint devices, and shall have a minimum of five (5) years verifiable experience related to this type of work. Contractor shall supply all labor, engineering, engineering approvals, equipment, tools and materials required as described in the contract documents, drawings, required by law and regulation and as necessary to complete the project.

D. The National Marine Fisheries Service (NMFS), a part of the National Oceanic and Atmospheric Administration (NOAA) of the Department of Commerce (DOC), is a governmental agency charged with the mission of stewardship for living marine resources.

1.02 EXTENT OF WORK

A. Provide all engineering, labor, material, tools, equipment, qualified engineer approvals and supervision necessary to complete the work described or shown in the drawings, specifications, and accordance with the manufacturer’s most current specifications and details to ensure the requirements in the contract documents are met supporting the best practices as recommended by the manufacture, Government Agencies, Professional Associations, Councils, Institutes, Manufacture, Codes and Standards. These include but are not limited to:

B. The contractor shall be fully knowledgeable of all requirements of the contract documents and shall make themselves aware of all job site conditions that will affect their work.

1.03 SUBMITTALS

A. The contractor shall submit the following :

1. Product data (5 days after NTP)

2. Schedule (5 days after NTP)

3. Contractor Confined Space Plan (5 days after NTP)

4. Contractor Fall Protection Plan (5 days after NTP)

5. Safety Data Sheets (SDS) as applicable (5 days after NTP)

6. Safety Plan (5 days after NTP)

1.04 PRODUCT DELIVERY, STORAGE AND HANDLING

A. Deliver materials to the job site in the manufacturer’s original, unopened containers or wrappings with the manufacturer’s name, brand name and installation instructions intact and legible. Deliver in sufficient quantity to permit work to continue without interruption.

B. Comply with the manufacturer’s written instructions for proper material storage.

C. Any materials which are found to be damaged shall be removed and replaced at the contractor’s expense.

1.05 WORK SEQUENCE

A. Care shall be exercised to provide protection of surrounding structures, parking lot, and hallways and buildings structure.

1.06 USE OF THE PREMISES

A. Before beginning work, the contractor shall secure approval from the building owner’s representative for the following:

1. Areas permitted for personnel parking.

2. Access to the site.

3. Areas permitted for storage of materials and debris.

4. Security: Obey the owner’s requirements for personnel identification, inspection and other security measures.

B. The NEFSC is a smoke free campus. The Contractor and its employees shall adhere to all applicable rules and regulations regarding maintenance of a smoke free environment on the job site.

C. Use Of Government Facilities

1. Water and electricity for construction purposes are available at the work site.

2. Toilet facilities shall be provided by the Contractor. Location of temporary facilities shall be coordinated with owner’s representative or the COR.

3. Contractor shall provide storage for tools and materials. The Government is not responsible for providing storage or security of materials, tools, or equipment.

4. Contractor has the option at his/her own cost to provide a temporary job office. Location to be determined and approved by the COR.

D. The premises will be occupied during the performance of the work. Contractor to coordinate work with the COR to accommodate the work schedule, continuing access and completion date.

E. Working hours shall be 8 Am – 4:30 PM M-F. Contractor shall observe all federal holidays.

1.07 EXISTING CONDITIONS

A. If discrepancies are discovered between the existing conditions and those noted on the drawings, immediately notify the COR.

1. Upon receipt of Bidding Documents verify documents are complete. Notify COR if documents are incomplete. Immediately notify COR upon finding discrepancies or omissions in Bidding Documents.

1.08 PRE-BID SITE VISIT

A. Prior to bid submittal, the contractor shall schedule a job site inspection to observe actual conditions and verify all dimensions. For access to the site prior to the closing Bid Date the contractor must contact the owner’s representative. Coordinate site visits at least 48 (two working days) hours in advance of intended site visit.

1.09 FACILITIES AND CONTROLS

A. The contractor shall remove and dispose of all debris and trash from the job site in a timely and legally acceptable manner so as to not detract from the aesthetics or the functions of the building or worksite.

B. Contractor shall comply with all applicable Federal, State, and local laws and regulations concerning environmental pollution prevention control and waste disposal.

C. Contractor shall immediately inform the COR of any environmentally hazardous situations that may arise as well as contact required regulatory agencies.

1.10 JOB SITE PROTECTION

A. The contractor shall adequately protect the site from damage while performing the required work.

Provide canvas, boards, barriers, signs, cones, and other devices as necessary for protection of equipment, facilities, personnel and the public. Remove protection materials, devices at the completion of the project. The contractor shall repair or be responsible for costs to repair all property damaged during the performance of work.

B. Protect against fire and flame spread. Maintain proper and adequate fire extinguishers within fifty

(50) feet of any open flames and welding. If any heated or flamed application is utilized a 15 lb or larger fire extinguisher shall be within 25’ reach or closer at all times. Contractor to supply two 15 lb or larger fire extinguisher to the job site

C. Comply with all requirements mandated by OSHA, EPA, NJ DEP and other regulatory agencies during the course of the project.

D. Best Management Practices (BMP) shall be followed and exercised to provide protection during the course of the project.

E. The contractor shall take precautions to prevent the spread of debris.

1.11 SAFETY

A. The contractor shall be responsible for all means and methods as they relate to safety and shall comply with all applicable OSHA, NEC, NFPA, and EPA, local, state and federal requirements.

Safety shall be the responsibility of the general contractor. All related personnel shall be instructed daily to be mindful of the full time requirement to maintain a safe environment for the facility’s occupants including staff, visitors, customers and the occurrence of the general public on or near the site.

B. The Contractor shall comply with the requirements of 29 CFR, Part 1910: Occupational Safety and

Health Administration (OSHA) General Industry and Health Standards and Health Standards and 29 CFR, part 1926 Construction Standards. The Contractor shall promptly correct any non-complying work or conditions. Contractor shall be responsible to ensure its work does not affect the safety of the public or employees of the Government. Neither the Government nor its representatives shall bear any responsibility for safety precautions at the project work site. If, however, in the opinion of the COR a life-threatening condition exists, the COR will orally (to be confirmed in writing) require the Contractor to immediately suspend work in the affected area until the condition is corrected.

Whenever Contractor’s operations may create a hazardous condition, it shall at its sole expense, furnish, install and maintain such guards, fences, barricades, railing, light, signs, or other devices necessary to prevent accident, injury or damage to people or equipment.

C. Safety plan. To include but not limited to fall protection, confined space, open flames, welding, debris, cleanup, work place safety methods, and emergency procedures. The Safety Plan must be available on site at all times and must be reviewed with all subcontractors working under this contract for the duration of the contract project.

F. It is the contractor’s responsibility to ensure all employees, sub-contractors, venders, and other company representative adhere to all safety rules, regulations, policies and practices.

G. The contractor shall keep the project site in a neat and orderly condition at all times.

1.12 WORKMANSHIP

A. All work shall be of highest quality and in strict accordance with the manufacturer’s published specifications, most recent codes and standards (including OSHA), and to the facility owner’s satisfaction

1.13 QUALITY ASSURANCE

A. Unless otherwise noted in the contract documents, contractor shall comply with the manufacturer’s current specifications and details.

B. Provide adequate number of experienced workmen regularly engaged in this type of work who are skilled in the application techniques of the materials specified. Provide at least one thoroughly trained and experienced superintendent on the job at all times when work is in progress.

C. There shall be no deviations made from this specification or the approved shop drawings without the prior written approval of the COR. Any deviation from the manufacturer’s installation procedures must be supported by a written certification on the manufacturer’s letterhead and presented for the specifier’s consideration.

D. All substitutions requests for products, equipment, materials and methods described within the contract documents shall be submitted to the COR. Substitutions request are not permitted prior to the Notice to Proceed. Substitute products will be considered when the request is submitted in writing to the COR. The COR will have seven (7) working days to approve / disapprove substitution request. Substitution request (approved or denied) shall not result in schedule extensions.

1.14 JOB CONDITIONS, CAUTIONS AND WARNINGS

A. Secure, isolate, monitor, test, warning postings, and other requirements per regulations and law for confined space entry.

B. The Contractor shall not cause or permit any hazardous material to be brought upon, kept or used in or about the premises by its agents, employees, or subcontractors without the prior consent of the COR. Hazardous materials that are necessary to the performance of this contract shall be used, kept and stored in a manner that complies with all laws regulating any such hazardous material brought upon or used or kept at this site. Contractor shall provide Safety Data Sheets to the COR for any hazardous materials used in the performance of this contract.

C. As used herein, the term “Hazardous Material” means any hazardous or toxic substance, material, or waste, including but not limited to, those substances, materials, and wastes listed in the Department of Transportation Hazardous Material Table (49 CFR 172.101) or by the Environmental Protection Agency as hazardous substances (40 CFR Part 68.130) and amendments thereto, petroleum products, or such other substances, material, and wastes that are or become regulated under any applicable local, state, or Federal law.

D. Contractor is encouraged to visit the site to view, measure the working space.

1.15 WARRANTY

A. Provide five (5) year warranty covering both labor and material with no dollar limitation.

PART 3 EXECUTION

3.01 GENERAL

A. Comply with the manufacturer’s published instructions for the installation of specified products, and installation requirements per the contract documents.

B. The Contractor is responsible for his/her own security arrangements including subcontractor’s, material, tools, equipment, and personnel.

C. Work in Progress: The Contractor shall be responsible for the protection and safe keeping of all work in progress prior to occupancy or final acceptance by the Government.

D. All methods, materials and workmanship provided by the Contractor under this contract shall, as a minimum, be performed in accordance with professional industry standards and shall comply with applicable codes for construction. Field measurements are the responsibility of the contractor.

3.02 SPECIFIC TASK

A. Improvements :

1. Install new anchor flush core or floor mounts for Confined Space/Fall Protection Systems:

a. Seawater Pump Station Mechanical Room Access: 2

b. Seawater Pump Station Aeration Vault #1: 1

c. Seawater Pump Station Aeration Vault #2: 1

d. Cooling Tower Make-up Water Vault #1: 1

e. Cooling Tower Make-up Water Vault #2: 1

f. Cooling Tower Make-up Water Vault#3: 1

2. Install new Bolt-on Anchor Plate for Fall Arrest Posts:

a. Seawater Pump Station Mechanical Room Access: 2

b. Seawater Pump Station Aeration Vault #1: 1

c. Seawater Pump Station Aeration Vault #2: 1

d. Cooling Tower Make-up Water Vault #1: 1

e. Cooling Tower Make-up Water Vault #2: 1

f. Cooling Tower Make-up Water Vault#3: 1

3. Install new Ladder Climbing Safety Kit (rail)

4. Install new fixed storage units for Portanle Confined Space and Fall Proteteciton Safety

System components.

a. Adjacent to Seawater Pump Station Mechanical Room access: 1

b. Adjacent to Cooling Tower Make-up Water Vault area: 1

5. Perform all concrete coring operations as required,

6. Perform all concrete work associated (forms, compact subgrade, anchors, and the like) with any required new footings to facilitate anchor points. Footing shall be 4’ X 8’ X 8” .

WWF 6X6-(D/W) 2.1X (D/W) 2.1 {6 X – 8 X 8} comforms to ASTM A1064. Minimum 3500 PSI. Concrete shall be sufficiently thick or have sufficient underlying structure to support a 90,000 inch-pounds momentary load and a 5000 lbs vertical load. Installation must be approved to local standards by a qualified engineer.

7. Contractor shall verify all connections requirements and supports prior to bid submission.

8. Contractor shall verify system installation(s) in accordance with manufacturer’s instructions.

9. Contractor shall provide documentation of compatability/concurrence related to the use of any non-system components.

10. Contractor shall ensure functionality of all system installations and components.

11. Contractor shall provide documentation of approval of all installations to local standards by a qualified engineer.

12. Install all required and stated accessories and other work necessary to complete the stated scope of work.

13. Location of all equipment shall be approved by COR prior to installation.

B. Access to Work Site:

1. All persons employed within the boundaries of the property or restricted-access areas therein, and all persons permitted to enter such property and areas shall comply with the security regulations for the NEFSC Sandy Hook Laboratory.

2. The Government reserves the right to close down the job site and order Contractor personnel off the premises in the event of a national emergency or a shut-down, for as long as security problems persist. The Contractor may only return to the site with verbal approval from the Contracting Officer or COR.

3. The Government reserves the right to exclude or remove from the site or facility any employee of the Contractor or a subcontractor whom the Government deems incompetent, careless, insubordinate or otherwise objectionable, or whose continued employment on the work is deemed by the Government to be contrary to the public interest.

4. Personnel may be subject to inspection of their personal effects when entering and leaving the facility. In addition, unscheduled inspections of personnel may be made when entering the facility or while on site.

C. Commercially Available Portable Confined Space and Fall Protection Systems:

1. Miller DuraHoist Confined Space Safety System, including permanent core or flush floor mount.

a. Or approved equal

2. Miller DuraHoist Fall Protection Posts/Accessories, including fixed anchor base.

a. Or approved equal

3. Miller MightEvac Self Retracting Lifeline (SRL) with Emergency Retrieval Hoist and complete mounting bracket assembly

a. Or approved equal

5. Miller GlideLoc Ladder Climbing Safety System Kit/Accessories (including stops and fall arrestors)

* Due to extreme environmental conditions of worksite, all instances where parts components have the option for aluminum, zinc-plated mild steel, galvanized or stainless steel, the stainless steel option shall be selected.

D. Final Acceptance and Inspections:

1. Inspection of work to be performed by the NEFSCS facilities maintenance supervisor, safety officer and COR. Any corrective action required as a result of the inspection will be accomplished prior to final monthly billing.

2. The Contractor may be notified of deficiencies in performance in writing by the COR. Oral deficiency notifications will be confirmed in writing. Prompt corrective action shall be taken by the contractor upon notice of deficiency.

3. The contractor shall advised the Government when all work is completed. The Government will not accept the work until it determines that all work has been satisfactorily completed within the terms of the contract.

3.09 CLEAN UP

A. Perform daily clean-up to collect all wrappings, empty containers, paper, and other debris from the project site. Upon completion, all debris must be disposed of in a legally acceptable manner. The Contractor shall ensure that the project area and areas adjacent to the project area are maintained in a clean and safe condition
