Performance Work Statement For

Turnkey Containerized Showers

1. DESCRIPTION OF SERVICES/GENERAL INFORMATION. This requirement is to provide turnkey containerized showers at the Basic Officer Leader Course (BOLC), Medical Training Facility located in Training Area 22 (TA22) on Joint Base San Antonio (JBSA)-Camp Bullis. Period of performance shall be from Base Year 1 Apr 2017 thru 31 Mar 2018, with four one-year options. Work shall be accomplished to support three week training periods six times per year as directed by the Government. The showers shall provide field sanitation facilities for trainees using the facility. Three (3) containerized showers (18 heads) must be provided. Showers shall be configured so that separate facilities for males and females must be provided. All work shall be accomplished in a manner consistent with industry standards. The contractor shall provide all equipment, supplies, parts, tools, maintenance, supervision and labor to deliver and install containerized showers required under this performance work statement (PWS) in order to provide showers, potable water delivery, and janitorial services. Any special tools or equipment required for installation shall be solely the responsibility of the contractor. The vendor shall be responsible for the tear down and removal of the equipment at conclusion of the contract. All work should be accomplished by a licensed and qualified contractor and shall comply with 29 CFR 1910, Occupational Safety and Health Standards for General Industry and 29 CFR 1926 Safety and Health Regulations for Construction.

1.1. DESCRIPTION OF SERVICES:

1.1.2. Contractor shall provide.

A. Contractor shall provide portable water supply/resupply to support three week training periods six times per year as directed by the Government.

B. Contractor shall dispose waste water at the Camp Bullis sewer manhole to support Three week training periods six times per year as directed by the Government.

C. Contractor shall remove all trash/debris from the facility daily and properly disposed of it off-base to support three week training periods six times per year as directed by the Government.

D. Contractor shall provide janitorial service to clean and disinfect the showers after each use to support three week training periods six times per year.

E. Contractor shall be responsible for 24-hour maintenance of the shower facilities plumbing, lighting, HVAC and power generation units to support three week training periods six times per year as directed by the Government.

F. Contractor shall be responsible for repairing and replacing any damages to

Government property caused as a result of the delivery, setup, and testing of showers.

G. Contractor shall ensure that propane is available, propane lines be high pressure

(120 psi) and shall be protected from all pedestrian and vehicle traffic.

H. Contractor shall connect the shower panel box to the existing electrical pole in coordination with Camp Bullis Department of Public Works (DPW) Electrical Dept.

I. Contractor shall ensure the installation of showers meet all required Federal, State, and Local codes, National Fire and Protection Codes (NFPA).

1.1.3. Specifications for Containerized Showers.

Specifications for Showers:

A. Showers shall be modular and conform to ISO Standard Container sizing of 8’ x 8’6”x 20’ exterior dimensions with electricity for lighting, outlets, exhaust fan, and air-conditioning.

B. Showers shall be engineered manufactured units. Tented Showers are NOT authorized.

C. Showers shall have individual shower stalls with hot and cold water. Showers shall have an integral changing area for each stall.

D. Showers shall have Anti-Scald Valves for user safety.

E. Showers shall have an internal exhaust system to properly remove interior moisture.

F. Showers shall have HVAC for each containerized unit.

G. Showers shall have interior lights.

H. Showers shall have interior sinks with hot and cold water. Sinks shall be 1 sink per 3 showers.

I. Shower units shall have mirrors located above sinks.

J. Showers shall be propane fired water heaters. Water Heaters shall be enclosed within a separate mechanical area for safety and security.

K. Showers shall have 10 # ABC Fire Extinguisher for each unit.

L. Showers shall have a smoke detector within each mechanical area.

1.2. GENERAL INFORMATION.

1.2.1. Quality Control. The contractor shall be responsible for quality control for all work accomplished during the performance of the task. The contractor shall provide and maintain a Quality Control Plan (QCP) which ensures the requirements in this PWS are met. The QCP will be considered acceptable and approved unless the contractor is notified by the Contracting Officer (CO) prior to award. The QCP shall be included in the technical proposal for evaluation and acceptance. As a minimum, the contractor shall develop quality control procedures that address the areas identified in section 2, Service Delivery Summary.

1.2.2. Quality Assurance. The government will periodically evaluate the contractor’s performance by appointing a Contracting Officer Representative (COR)(s) to monitor performance to ensure services are received. The COR will evaluate the contractor’s performance in accordance with the Quality Assurance Surveillance Plan (QASP) for this requirement.

1.2.3. Government Remedies. The contracting officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items (Jan 2017), for contractor’s failure to correct nonconforming services.

1.2.4. Hours and Days of Operation.

A. Normal Operation Hours 8:00 AM to 5:00 PM . Services shall be performed as indicated in the description of the services, Monday through Friday.

A list of recognized Federal Government holidays is as follow:

B. Emergency Services after Normal Business Hours. The Government may request service at any time, on any day of the year. Emergency service requests require a response time of one hour, 24 hours a day during the period of performance.

C. Installation Closures. In the event of unplanned facility closure due to natural disaster, military emergency, severe weather, or facility-related problem that prevents the contractor’s personnel from performing; personnel shall follow the same departure and reporting directions given to Government personnel.

D. Meetings. The Contractor's representative may be required to meet with the Contracting Officer as determined necessary. The Government will schedule meetings, but the Contractor may request a meeting be held when there is a discrepancy between the Contractor and the Contracting Officers Representative (COR) regarding performance.

E. Accident Reporting. The contractor shall maintain an accurate record of accidents resulting in traumatic injury or death and accidents resulting in damage to Government property, supplies and equipment. In the event of an accident incurred on a Government installation while performing work specified in this contract, the Contractor shall create a report to be rendered immediately to the Range Control,

(210) 295-7510 or the Camp Bullis Security Forces, (210) 295-7557. Report shall include, but not necessarily be limited to place, time, extent of damage, and injury to personnel, etc. This provision is not intended to compromise and does not compromise any State or Local Government requirement for reporting motor vehicle accidents.

1.2.5. Security. The Department of Defense (DoD) requires installations to appropriately vet all contractors providing service to JBSA Fort Sam Houston prior to granting access. Vetting is an evaluation of an applicant’s character and conduct for approval, acceptance, or denial for the issuance of an access control credential or physical access. Contractors must submit written requests to the Contracting Officer (CO) on company letterhead. The CO will verify, sign, and forward the list to the Security Forces Visitor Center no later than ten working days prior to the requested access date.

Work under this agreement is UNCLASSIFIED. The Contractor and its service providers shall comply with all applicable DoD security regulations and procedures during the performance of this contract. The Contractor shall not disclose and must safeguard procurement sensitive information, computer systems and data, Privacy Act data, and Government personnel work products that are obtained or generated in the performance of this contract. This includes dissemination of protocols and papers not generally available through the public literature.

1.2.6. Special Qualifications. Contractor shall provide a licensed driver with the proper certification by state and federal laws to dispose the waste water.

1.2.7. Performance of Services During Crisis Declared By The National Command Authority or Overseas Combatant Commander. Services shall be provided 24-7 unless otherwise directed by the CO. Should a crisis occur, the Contractor will be notified by the

COR/CO.

1.2.8. Records Management. The Contractor shall manage all data created for Government use or legally controlled by the Government, in support of the functional activity or required by AF publication, IAW with the AF records management procedures in Air Force Instruction (AFI) 33

1.2.9. Air Force Antiterrorism (AT) Program. The AT program seeks to deter or limit the effects of terrorist acts against the AF by giving guidance on collecting and disseminating timely threat information, providing training to all AF members, developing comprehensive plans to deter and counter terrorist incidents, allocating funds and personnel and implementing AT measures. In accordance with AFI 10-245 paragraphs 2.17 and paragraph 2.25, DOD contractors and sub-contractor personnel shall be offered Level 1 – AT Awareness Training. Compliance with the training will be monitored by the contracting officer representative.

1.2.10. Contractor Full-Time Equivalent Reporting. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the government via a secure data collection site. The contractor is required to completely fill in all required data fields at https://afcmra.hqda.pentagon.mil/login.aspx.

Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

Reporting Period: Contractors are required to input data by 31 October of each year.

Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.

3. GOVERNMENT FURNISHED PROPERTY.

A. Potable Fresh Water Supply – fire hydrant or similar.

B. Waste Water discharge point – sewer manhole or similar.

C. Power Supply - 220V/100AMP/Single Phase Disconnect mounted to existing pole or panel or similar.

D. Fresh Water Holding Tank (water Buffalo), 3500 gallon.

E. The electrical connections shall be done by the contractor and coordinated with

AMEDDC&S Engineering and Facilities personnel, prior the performance (210-221- 7394 or 210-221-6356). All work shall be accomplished by a licensed and qualified contractor and shall comply with 29 CFR 1910, Occupational Safety and Health Standards for General Industry and 29 CFR 1926 Safety and Health Regulations for Construction.

4. CONTRACTOR FURNISHED EQUIPMENT.

A. Three (3) Mobile shower units – 18 heads, and one fire extinguisher per container.

B. Potable water supply system with pressure pump manufactured with National Sanitation Foundation (NSF 61) Rated Equipment and pressures shall be and pressures shall be between 38-42 Pounds per Square Inch (PSI).

C. Backflow Preventer and Reduced Pressure Zone (RPZ) installed between the pressure pump and the showers.

D. Waste water holding tank tight, leak proof – 3500 gallon capacity.

E. Waste water collection system with grinder pumps.

F. Propane tank and distribution lines In Accordance With (IAW), National Fire and

Protection Act (NFPA) 58 and local codes and Camp Bullis/FSH Safety Guidelines.

G. 200767Potable Water Piping shall be Polyvinyl Chloride (PVC), Standard Dimension Ratio (SDR) 35 and rated for 120 (PSI) operating pressure.

H. Potable Water Delivery Truck with a secure and tight waste water collection system.
