The research investigates how Medicare Advantage provider networks and reimbursement practices affect access to acute hospital care and clinical outcomes in rural areas, where MA enrollment quadrupled from 10 percent in 2010 to 40 percent in 2022. Primary outcomes include 30-day mortality and clinical markers of illness severity among hospitalized patients (intubation, intensive care unit admission, dialysis, and blood transfusion), with travel distance to in-network hospitals examined as a secondary mechanistic outcome. The study addresses a knowledge gap regarding the extent to which rural MA enrollees have timely access to in-network acute hospital care, particularly given that 141 rural hospitals closed between 2010 and 2022 with disproportionate impact on Black, Hispanic, and American Indian communities. The research approach includes in-depth, semi-structured interviews with Medicare Advantage and rural hospital representatives to characterize the quality implications of MA on hospital care for rural populations. Performance of the award occurs in Rhode Island and extends through April 30, 2031. This is a Project Grant assistance type.
