The National Center for Complementary and Integrative Health (NCCIH) awarded a $817,330 Project Grant (CFDA 93.213 - Research and Training in Complementary and Integrative Health) to the University of Virginia to investigate an adaptive intervention to improve pain outcomes in patients with comorbid chronic low back pain and insomnia. The project will first enroll 300 patients in a cognitive behavioral therapy for insomnia (CBTI) program, then randomize non-responders to receive either a telehealth-delivered positive affect enhancing intervention (savoring meditation) or an attention control condition. The primary outcome will be pain severity, tracked for 12 months to assess durability of effects. The project aims to identify predictors of pain response and explore whether changes in insomnia severity moderate the effects of the savoring meditation intervention. This research seeks to develop a mechanistically informed approach to improve pain outcomes for patients who do not respond sufficiently to CBTI alone.
