RISE 2.0 (Indiana Youth and Family Recovery Services Expansion) will deliver integrated, family-centered, trauma-informed, evidence-based care across Indiana, serving a minimum of 25 unduplicated adolescents and young adults in year one and at least 80 annually in years two through five, totaling a minimum of 345 over the five-year period. Services include medication management, care coordination, recovery supports, aftercare, caregiver services, statewide teleconsultation, and workforce training. Service delivery encompasses developmentally appropriate assessment, motivational enhancement, cognitive-behavioral and family-based treatment, medication evaluation, care coordination, and recovery support. Caregivers and family members participate through family-based treatment, CRAFT groups, navigation, referral support, recovery planning, and aftercare. The award period runs through September 29, 2031, with performance centered in Indianapolis, Indiana.
