The Department of Health and Human Services Administration for Community Living awarded the State of Alaska Department of Health $303,569 on October 1, 2025, under the Special Programs for the Aging, Title III, Part D, Disease Prevention and Health Promotion Services program (CFDA 93.043).
The award funds evidence-based disease prevention and health promotion services for older Americans, including programs related to physical fitness, medication management, chronic disease self-management education, psychosocial behavioral health intervention, HIV, arthritis, brain health, diabetes, falls prevention, depression, and chronic pain. The State of Alaska Department of Health, acting as the State Agency on Aging, receives the award and passes funds through to substate-level organizations, including Area Agencies on Aging and community-based service providers, which deliver direct services to eligible populations across Alaska.
The period of performance runs from October 1, 2025, through September 30, 2027. Work is performed throughout Alaska. This is a formula grant awarded on a non-competitive basis to the state under statutory entitlement; the state approved its allocation through a State Plan submitted to the Administration on Aging.
