The U.S. Centers for Medicare and Medicaid Services (CMS) awarded a $2,887,052 Cooperative Agreement to the District of Columbia Department of Health Care Finance (DHCF) to develop operational capabilities to promote continuity of care for justice-involved individuals, particularly youth, under the Section 206 Consolidated Appropriations Act, 2024: State Planning Grants to Promote Continuity of Care for Medicaid & CHIP Beneficiaries (CFDA 93.694) program. The 4-year grant, effective January 13, 2025, will support DHCF in improving Medicaid eligibility and enrollment processes, building technological infrastructure for information sharing, and enhancing staff capacity to deliver person-centered services for individuals transitioning from secure detention facilities back to the community. DHCF will collaborate closely with the District's Department of Corrections (DOC) and Department of Youth and Rehabilitation Services (DYRS), which will be sub-awardees under this grant.
