Added: Jun 09, 2017 5:01 pm This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.(ii) The Navajo Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Proposal (RFP) KSU-KG17-0002. This solicitation is issued on a SMALL BUSINESS SET-ASIDE. (iii) The solicitation documents and incorporated provisions and clauses are those in effect tough Federal Acquisition Circular 2005-16. (iv) The associated North American Industry Classification System code is 238220 and the small business size standard is $15.0 million. (v) PRICE/COST SCHEDULE - Contractor shall include all costs on fee schedule to complete services.(vi) PURPOSE OF THE CONTRACT: Task Description: Contractor shall provide all labor, material, equipment, etc. to remove old chiller and install new chiller at the Kayenta Health Center. SEE RFP.(vii) Date and Place of Delivery: Kayenta Health Center, U.S. Highway 160, South of Mile Post 394.3, Kayenta, AZ, 86033.(viii) Specifications: See RFP.(ix) Evaluation Factors: The Government reserves the right to issue a single award or multiple awards to the schedule contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following evaluation factors will be considered: 1. Past performance2. Qualifications of each proposed candidate3. Technical Capability4. Logistics5. Socioeconomic Status These evaluation factors will be weighted more than price. Price will be utilized as a tiebreaker in case of equally acceptable proposals.CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer.AWARD DECISION: The Government will award a contract, resulting from this solicitation, to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Added: Mar 07, 2017 5:02 pm This Sources Sought Notice is for the Navajo Area Indian Health Service, Kayenta Service Unit (KSU) issued in accordance with Federal Acquisition Regulation (FAR) Subpart 5.101 & 5.207 for chiller removal, installation, testing and turn-up at the Kayenta Health Center, Kayenta, AZ. The purpose of this notice is to identify potential sources for providing this service.This Chiller Replacement project will be within the Navajo Reservation at Kayenta, Arizona. The Hospital is located off US Highway 160, South of MP 394.3. This is a new hospital facility with two existing chillers in place. This project requires vendor's expertise for the replacement of an unrepairable 300-ton chiller at the Kayenta Health Care Center.General requirements:1. The Contractor shall replace of York 300-ton chiller, all hardware, software, associated piping equipment, chill water piping, electrical work, crane service, DDC control connection, shipping and startup service.2. The Contractor shall replace chiller and it must be controlled with existing DDC control system.3. Contractor shall provide technical support for maintenance, on-call service and emergency repairs.The Government does not intend to make an award on the basis of the Sources Sought Notice or otherwise pay for the information provided herein.The Anticipated Award Date: 04/14/2017.Submission Instructions: Interested parties who consider themselves qualified to perform the above-listed services are invited to submit a response to this Sources Sought Notice by 03/15/2017, 12:00 Mountain Time.
