Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This procurement is for a Toyota Sienna XLE Wheelchair Accessible Minivan with required components used during Driver Training IAW VHA Directive 1171.16 Appendix B. The current Driver Training mini- van is equipped with out-of-date technology/equipment that no longer meet the needs of our veteran population, for training and products required. The current vehicle limits safety, training, mobility, access, and quality of care to our veterans and results in denial, costly referrals or travel to another VA and delays in care. Estimated Amount: $161,624.00 Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Driver Rehabilitation program is a very unique and specialized program that requires specific equipment to properly train and treat patients. The specialized vehicle requested allows for evaluation and treatment for both driver and passenger needs. It provides training for in/egress of a vehicle with adaptive equipment as well as behind the wheel training with specialized adaptive equipment seated and secured from wheelchair level. Only certain resources could be considered and were considered for optimal vehicle of choice to support our specialized veteran population. The vehicle requested by Driver Rehabilitation meets the recommendations set forth in the VHA Directive 1173.16 (Driver Rehabilitation for Veterans with Disabilities Program). Patients referred to the VA Driver Rehabilitation service present with a wide range of physical disabilities. Basic instructional techniques in driver education and training are common in many disabilities. Specific and isolated disabilities require special evaluation and adaptations (i.e. spinal cord injury, amputation, traumatic brain injury, stroke) that are needed for safe and innovative quality patient care. The adaptations include wheelchair accessibility in the vehicle (i.e. Lowered floor conversion, power ramp entry, wheelchair secure, modified seating), special equipment to training for return to driving (i.e. hand controls, spinner knob, reduced effort steering, secondary control adaptations). Description of market research conducted and results or statement why it was not conducted: Market Research was conducted, and several Vendors do have FSS contract for conversion type vans. However, with this being a highly specialized/unique training conversion van used to train Saint Louis, MO. VAMC Veterans IAW VHA Directive 1171.16 Appendix B market research determined only one local certified EMC Installer source (United Access of Saint Louis, MO.) The price was determined fair & reasonable from present and past purchases from other VAMCs. GSA Management Fleet (Washington, D.C.} has approved this one time purchase with waiver (Attached}.
