The Department of Veterans Affairs (VA) has an ongoing program to furnish artificial limbs and related services to those veterans that require them and are eligible for assistance. This requirement is for beneficiaries of the James E VanZandt VA Medical Center (JEVZ VAMC), 2907 Pleasant Valley Blvd, Altoona, PA 16602. It is VA's intention to solicit proposals and award a firm fixed price requirements type contract on a competitive basis under the authority of Title 38 U.S.C. Section 8123. The contractor must be Joint Commission (JC) or equivalent accredited and must maintain accreditation for the duration of the contract. The government contemplates award of one Firm-Fixed Priced, Requirements type contract. Contract term will be for the base period of one year from the date of award, with four (4) 1- year renewal options.



The Contractor shall fabricate all artificial limbs in strict conformance to the prosthetic prescription that has been provided by the VA Amputee Clinic Team. Contractors shall not add or substitute components or alter the beneficiary's limb prescription in any way without prior approval and written authority from the VA Amputee Clinic Team Prosthetic Representative or designee.

Area of coverage for the James E VanZandt VA Medical Center: The contractor shall provide services to the area of coverage as specified below for the VA Medical Center, 2907 Pleasant Valley Blvd, Altoona, PA 16602.
