The Department of Veterans Affairs VISN 15 has a need for trash and recycling service for one base year with the option to extend up to four additional years for a total of five years.
Contractor will provide one, 30+cubic yard, dry-waste compactor with container and cart tipper, and removal of non-hazardous waste and recyclable waste from the Great Lakes Consolidated Mail Outpatient Pharmacy facility located at 5th and Roosevelt, Building 37 NW Dock 18, Hines, IL 60141-3030. Recyclable materials are to be sold and revenue returned the Great Lakes CMOP. Non-hazardous waste and recyclable waste is to be removed at regular intervals as described in the Statement of Work, and disposed at an EPA-approved landfill in accordance with all federal, state, local and commercial standards.
A MANDATORY SITE VISIT will be held on 20 August. Bidders should attend to fully understand the nature of the work and conditions under which the work is to be performed. Failure to attend the site visit will not relieve the successful offeror from completing the work in strict compliance with the true intent and meaning of the specifications. Offerors shall meet at the Great Lakes CMOP facility located at 5th and Roosevelt, Building 37 NW Dock 18, Hines, IL 60141-3030. Contact James Hinspeter, (708) 786-7541, to arrange a time.
Contractor shall sign a Business Associate Agreement (BAA) in accordance with the Health Insurance Portability and Accountability Act (HIPAA), the Health Information Technology for Economic and Clinical Health Act (HITECH) Act, and the HIPAA Privacy, Security, Breach Notification, and Enforcement Rules ("HIPAA Rules"), 45 C.F.R. Parts 160 and 164, for the Use and Disclosure of Protected Health Information (PHI).
These requirements shall be performed in accordance with all federal, state, local, and commercial standards and the Specifications. The solicitation will be made available on or about 14 August 2015, and will be distributed solely through the Federal Business Opportunities (FBO) website. Hard copies of the solicitation will not be available. The website provides all downloading instructions to obtain a copy of the solicitation. All future information concerning the acquisition, including solicitation and amendments, will be distributed solely through the FBO website. Interested parties are responsible for monitoring the site to ensure they have the most up-to-date information about the acquisition.
THIS PROCURMENT IS UNDER NAICS CODE 562111 WITH A SIZE STANDARD OF $ 38.5 Million, PSC CODE S205. This solicitation will be set-aside for Small Businesses.
