This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and supplemented with additional information included in this notice. The Department of Veterans Affairs 646 is considering whether or not to set aside an acquisition for Small Businesses. The solicitation number is VA244-12-Q-0291 and it is issued as a Request for Quotation (RFQ).This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued.
The NAICS code is 541380
Successful offer will be the one that provides the Lowest Price Technically Acceptable.
The award will be for 12 months of service (base year) and four option years.
The VA Pittsburgh Healthcare System 646 has a requirement for the services listed below.
All work will be performed at our facilities located in Pittsburgh, Pennsylvania.
All communication regarding quotes, questions, etc. shall be submitted by email to John Barton (Johnny.Barton@VA.Gov). Phone calls will not be accepted. Quotes are due by 2/21/12 at 09:00 A.M. Eastern Standard Time.

SCOPE OF WORK

8.1 Minor reasonable repairs can be made within the scope of testing/preventative maintenance as certified repair technician of the subject equipment.
8.2 Critical immediate repairs will be requested per approval from Contract Office at contracted hourly rate.
9.0 Notifications
9.1 All machine leaks which exceed the NIOSH maximum limit of 25 ppm for nitrous oxid3e must be reported by the vendor to Safety Department, Supervisor of Biomed Shop, or designated hospital representative prior to the company representative leaving the medical center.
10.0 Scheduling
10.1 Contac the COTR, Christian Lewis, at or Supervisor of Biomed Shop to schedule the testing service.
11.0 Pharmacy Clean Room
11.1 Certification of Laminar Air Flow Hoods (LAFH)
11.1.1 Particle counts for verification of ISO class 5 for all 7 hoods
11.1.1.1 5 Horizontal LAFH
11.1.1.2 1 Biological Safety Cabinet (Chemo Hood)
11.1.1.3 1 compounding aseptic barrier isolator or CAI (PharmaGuard)
11.1.2 Minimum of 1 microbial surface sampling of each LAFH (5 total)
