This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for Solicitation #FA4484-15-R-0036. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84
The associated NAICS code is 339999 with a size standard of 500.
The contract line item breakout is as follows:
0001 TOM4-000C ToMManikin GEN4, (2) each.
0002 TOM4-4104 Burn Arm (right arm) (2) each.
0003 TOM-4205 Closed fracture arm (left arm) (2) each
0004 WW3-902 Packable evisceration abdomen overlay (2) each
0005 ITTS-PMS Patient Monitoring Suite (2) each
All items are brand name or equal in accordance with FAR 52.211 6. The government will inspect the equipment before acceptance.
The required delivery date is 60 days after receipt of an order and shall be FOB Destination, Joint Base McGuire-Dix-Lakehurst, 08641
PROVISIONS/CLAUSES: The following clauses apply to this acquisition: FAR 52.402-7 Central Contractor Registration, FAR Clause 52.212-1 - Instructions to Offerors Commercial Items; FAR Clause 52.212-3 - Offeror Representations and Certifications Commercial Items; FAR Clause 52-212-4 - Contract Term and Conditions Commercial Items; FAR 52.211-6 Brand Name or Equal; FAR Clause 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Item following are incorporated references: (b) FAR 52.225-1- Buy American Act-Supplies, FAR 52.232-33 - Payment for Electric Funds Transfer-Central Contractor Registration, FAR 52.222-50 Combat Trafficking in Persons, FAR 52.223-18 Encouraging Contractor Policy to Ban Text Messaging While Driving. The following DFARS clauses apply to this acquisition; 252.203-7000 Requirements Relating to Compensation of Former DOD Officials; 252.209-7995, Representation Regarding Conviction of a Felony Criminal Violation under any Federal or State Law (DEV); 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisition of Commercial Items; 52.225-7000, Buy American Act-- Balance of Payments Program Certificate; 252.225-7001 Buy American Act and Balance of Payments Program; 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports. Federal Acquisition Regulation clauses and provisions are available in the internet at: http://farsite.hill.af.mil or may be requested from the Contracting Officer.
INSTRUCTIONS TO OFFERORS: All quotes must be emailed to POC; Mrs. Zaquette Everett, zaquette.everett@us.af.mil. All contractors must be registered in the System for Award Management website at http://www.sam.gov database prior to any contract award. Register, or renew registration, with the Online Representations and Certifications Application https://orca.bpn.gov/ as soon as possible, to expedite contract award. Wide Area Workflow https://wawf.eb.mil/ will be the method of submitting invoice after receipt of supplies or services. Please submit the following information with each quote: Cage Code, DUNS number, Tax Identification Number, Prompt Payment Terms, Delivery Time, GSA/VA/Government Contract No. (If applicable), Date offer expires, warranty, Line item unit price, and Total cost.
EVALUATION OF QUOTATIONS/OFFERS: Quotes/offers will be evaluated IAW 13.106 2. Award will be made to the responsible offeror whose quote/offer represents the lowest price. All offers quoted as equal must provide additional information for evaluation or will not be considered. The Government intends to evaluate proposals and make award without discussions. Quotes must be signed, dated and submitted by 14 September 2015, 03:30pm EST to zaquette.everett@us.af.mil via PDF or WORD DOCUMENT to the Attention of Mrs. Zaquette Everett.
LATE OFFERS: Offerors are reminded that e mail transmission of quote/offer will not constitute a timely submission of quote/offer unless it is received by the government prior to the closing date and time.
Quotations/offers or modifications of quotes/offers received at the address specified for the receipt of offers after the exact time specified for receipt of offers WILL NOT be considered (ref. FAR 52.212 1(f)).
