Added: Aug 01, 2017 11:10 am This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quote (RFQ) for non-personal orthopedic physician services.

The solicitation is issued as Request for Quotes (RFQ) 246-17-Q-0043.


This procurement is 100% small business set-aside under NAICS code 621111 with a standard business size of $11.0M. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-86-2, effective February 1, 2016.


Contractor shall provide a firm, fixed-price hourly rate for Orthopedic Clinic services. The period of performance for this requirement is approximately one year.


Estimated start date is September 8, 2017. The closing date for receipt of quotes is August 25, 2017 Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m. CST, on August 25, 2017. The offer may be submitted by email or mail, addressed to this office, showing the solicitation number, and your name and address. Qualified vendors will review the Statement of Work and following information then submit applicable information. No phone inquiries!


The Indian Health Service (IHS) intends to award a firm-fixed price purchase order. The Government may make an award without discussion therefore, the quote should contain the best price and shall be determined to be the best overall values and most advantageous to the Government.

The provisions of 52.212-1, Instructions to Offerors-Commercial Items (Jan 2017), applies to this acquisition. In addition to the information required in FAR 52.212-1, Contractors shall provide their Dun and Bradstreet Number (note: Contractors must be fully registered in the System for Award Management (SAM) database to be eligible for award. This can be done at https://www.sam.gov).


The provisions of 52.212-2, Evaluation - Commercial Items (Oct 2014) applies to this acquisition. The Government intends to award one from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Price; (2) Qualifications; (3) Past Performance. All factors are equal in importance.


Quotes shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (Jan 2017) OR indicate certifications in ORCA at https://www.acquisition.gov.

CRITERIA REQUIREMENTS
The due date for this requirement is September 25, 2017 4:30 PM CT. THE INDIAN HEALTH SERVICE WILL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO RESPOND TO THE FOLLOWING EVALUATION CRITERIA:
A) Propose and provide an all-inclusive hourly rate for services.
B) Documentation of two past references.
C) Qualifications, shall include copy of degree from accredited medical school and proof of completed approved residency
D) Registration in the System for Award Management (SAM) database (reference https://www.sam.gov Please note that all parties interested in doing business with the Indian Health Services must be registered in the SAM database.
D) Certifications in ORCA at https://www.acquisition.gov.
E) Current license as an Orthopedic/Medical Doctor (any U.S. state or territory).
F) Proof of current medical insurance in the appropriate amount of $1,000,000 per occurrence and $3,000,000 aggregate.
G) Board Certification


EVALUATION RATING INFORMATION
Rating:
Five: Outstanding-Proposal meets requirements and indicates an exceptional approach and understanding of the requirements. Strengths far outweigh any weaknesses. Significantly exceeded performance or capability standard. Risk of unsuccessful performance is very low.
Four: Good-Proposal meets requirements and indicates a thorough approach and understanding of the requirements. Exceeded some performance or capability standards. Proposal contains strengths, which outweigh any weaknesses. Risk of unsuccessful performance is low.
Three: Acceptable- Proposal meets requirements and indicates an adequate approach and understanding of the requirements. Strengths and weaknesses are offsetting or will have little or no impact on contract performance. Risk of unsuccessful performance is no worse than moderate.
Two: Marginal-Proposal does not clearly meet requirements and has not demonstrated an adequate approach and understanding of the requirements. The proposal has one or more weaknesses, which are not offset by strengths. Risk of unsuccessful performance is high.
One: Unacceptable- Proposal does not meet requirements and contains one or more deficiencies. Proposal is un-awardable.
