This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

The solicitation is issued as Request for Quotes (RFQ) 246-22-Q-0044

NAICS Code: 334510, Size Standard: 1,250

Set Aside: 100% Small Business

Contractor shall provide a firm, fixed-price quote to provide and deliver Optometry Equipment for the Haskell Indian Health Center, Lawrence, KS. Delivery shall be completed within 60 days from date of award.

The closing date for receipt of quotes is February 4, 2022 at 4PM CST. Offers shall be submitted to the Oklahoma Area Indian Health Service, Division of Acquisition Management, 701 Market Drive, Oklahoma City, Oklahoma 73114 or emailed to Teihahnietuh.McCargo@ihs.gov . The offer shall be addressed to Teihahnietuh McCargo, Contract Specialist, showing the solicitation number, and your name and address. No phone inquiries!

TELEPHONE INQUIRIES SHALL NOT BE ACCEPTED AND TELEPHONE CALLS SHALL NOT BE RETURNED.

The provisions of 52.212-1, Instructions to Offerors—Commercial Products and Commercial Services (November 2021), applies to this acquisition. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: (1) Firm, fixed-price quote, (2) Specifications that are being quoted and (3) past-performance. Award decision shall be based on lowest technical accepted price. Vendor is still required to meet all specifications in the Scope of Work

The provision at FAR 52.212-2, Evaluation—Commercial Products and Commercial Services (November 2021), applies to this acquisition. The contract shall be awarded to the Lowest Priced Technically Acceptable (LPTA) offeror. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions.

The clause at FAR 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services (November 2021), applies to this acquisition. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website located at https://www.sam.gov

THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING:
a) Firm, Fixed Price Quote
b) Technical specifications (submittals) of your proposal along with Country of Origin
c) (2) Verifiable, recent Past Performances that include similar experience in deploying hardware similar in SOW. Include Contracting Organization, Contract POC (phone number and email), and Contract amount.

Proposals shall be evaluated on an acceptable/non-acceptable basis.

Evaluation shall proceed as follows:
1. Quotes shall be ranked by price, from low to high
2. Quotes shall then be evaluated for technical acceptability and past-performances beginning with the lowest priced quote
3. This process shall proceed until a technical acceptable quote is identified

THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS OF THE SCOPE OF WORK.

CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR PROVIDING THE LOWEST REASONABLE PRICE, MEETING ALL TECHNICAL SPECIFICATIONS AND PAST-PERFORMANCES.

The Technical Evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions.
