Added: May 01, 2015 7:09 pm This notice is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.

This announcement constitutes the only solicitation that will be posted. This notice is being issued as a Request for Quotations (RFQ) for Occupational Health Services (OHS).

The provisions and clauses in the RFQ are those in effect through FAC 2005-81.

This procurement is a total small business set-aside.

The NAICS Code and the small business size standard for this procurement are 621111 and $11M, respectively. The offeror shall state in their offer their size status for this procurement. All responsible sources may submit an offer which shall be considered by the agency.

Offers for the requirements described in the solicitation are due by June 3, 2015 to NASA Armstrong Flight Research Center, Attn: Shari Trigg, Contract Specialist, Solicitation Number: NND15531905Q, 2825 East Avenue P, Building 703, M/S S323, Palmdale, CA 93550 and must include all elements as instructed in the solicitation.
