INTENT TO SOLE SOURCE
FAR 13.106(B)
This is a notice of intent of The Louis Stokes Cleveland Department of Veterans Affairs Medical Center, 10701 East Boulevard, Cleveland, Ohio 44106 to negotiate sole source with PARKS MEDICAL ELECTRONIC INC. for supplying The Department of Veterans Affairs Medical Center, Cleveland Ohio, Medical Service with the following
FLO?LAB MODEL 2100 SX?2 with touch screen monitors 3 EA
DICOM COMPLIANCE PACKAGE WITH MODALITY WORKLIST 3 EA
Background
The Louis Stokes Cleveland VA Medical Center Vascular Lab section is requesting three peripheral machines. These are used to assess patients healing and perfusion status in the upper and lower extremities. One machine will be located in the Youngstown CBOC to accommodate patients within the demographic. Two will be used at Wade Park in the Vascular Lab clinic. One is replacing a current machine and the additional will be used in the expanding Vascular Lab clinic. This allows for the Wade Park clinic to provide appointments in a timelier manner. Each year vascular lab increases the number of exams requiring the need for an additional machine.
Scope
The contractor shall provide all resources necessary to support the Vascular Lab section requirement at the Louis Stokes Cleveland VA Medical Center.
" The vendor shall deliver the above items to the Cleveland VAMC/ Youngstown CBOC within 1-2 weeks or order placement.
" Installation and assembly of the equipment shall be completed by the contractor. The work will be inspected and accepted by the medical staff and biomed.
" Minimum warranty shall be Six month on Doppler probes and PPG sensor, 90 days on cuff bladders, 24 month mail back warranty on parts and labor on remainder of unit.
The Government believes that only one business can satisfy the agency requirements and intends to negotiate with only one source under the authority of FAR 13.106(b) - access to proprietary information. Other persons may identify their interest and capability to respond to this requirement by Thursday September 02, 2016 at 3PM EST. Such capabilities information will be used solely for the purpose of determining whether or not to conduct this procurement on a competitive basis. All information shall be furnished at no cost or obligation to the Government. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government.
