The Gulf Coast Veterans Health Care System (GCVHCS), Biloxi, MS, is issuing a combined synopsis/ solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is being issued. The solicitation number 36C25618U0296 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99 effective July 17, 2018. The NAICS code is 515210 and the size standard is $38.5 million. This requirement is set aside as open and unrestricted and will be firm-fixed-price. The closing date for this notice is 12:00 PM CST September 12, 2018. The requirement is for the following:
Contractor shall provide Cable TV (no satellite) along with two modems for the Gulf Coast Veterans Health Care System, Joint Ambulatory Care Center (JACC), 790 Veterans Way, Pensacola, FL 32507. This shall include all labor, parts, and travel required to provide 24/7 full service activation and coverage for various buildings and numerous drops as identified below and in accordance with the Scope of Work (SOW).
Contractor to provide services in accordance with terms and conditions of the contract, for the period of October 1, 2018 through September 30, 2019, with four (4) twelve (12) month Option Periods to be exercised at the discretion of the government.
To be eligible for award of a contract, vendor must be actively registered in the SAM database (http://www.sam.gov). Vendor is also required to complete representations and certifications found in the provision at 52.212-3, Offeror Representations and Certifications - - Commercial Items.

Scope of Work
Contractor to provide Cable TV services (no satellite) along with 2 modems for the Gulf Coast Veterans Health Care System, Joint Ambulatory Center (JACC), 790 Veterans Way, Pensacola, Florida 32507, to include all labor, parts, materials, and travel required to provide 24/7 full service activation/coverage for various buildings and numerous drops. There are 60 outlets. The internet has 300MB (megabyte) download speed and 20MB (megabyte) upload speed.

Cable Outage Response Time: Within one (1) day or 24-hours following notice of a service call.
VA authorized personnel who may contact the contractor for repair service (Note: these names are subject to change):

Darrel Maples, Supervisory General Engineer, Engineering, and/or
Jay Tripp, Chief Engineer.
Contractor shall be responsible for troubleshooting all equipment failures and shall notify the Contracting Officer for any disruption of cable service, which may exceed 48-hours from the original service call. Contractor must notify and provide verbal or detailed written service report to the Contracting Officer for any disruption of service, which exceeds 48-hours from the original service call receipt.
Any defective cabling found outside the buildings shall be repaired or replaced by the cabling Contractor.
Any defective cabling found within the buildings shall be repaired or replaced by the Government.
All parts and labor are covered under this plan, as stated above.
If field technicians are unable to repair faulty equipment outside the buildings, and the contractor is non-responsive within 72-hours from original service call, the Government reserves the right to contract for repair service with another provider. Any repair service fees charged by an outside provider will be passed along to the contractor and shall be deducted from their monthly invoice (outside charges shall be deducted appropriately within the timeframe service was disrupted)..
