This amendment is hereby issued as follows:

1. Include Preconstruction Risk Assessment form.

2. Answers to contractor questions:



1. Shall a camera be installed and what are the specifications of the required equipment?

VA Response: Yes, the camera is a Panasonic Model # WV-SF332.



2. Does the sprinkler in 107A need to be replaced with an anti-ligature sprinkler head?

VA Response: Refer to the specifications, Sprinkler System: The contractor shall install a new anti-ligature sprinkler system in room 107 and 107A. If the current sprinkler system in room 107A meets the anti-ligature code, then the sprinkler system in that room can stay. The contractor shall test new sprinkler system.



3. Where can we acquire the specifications for all of the proposed equipment (ie. card reader, nurse call button, anti-ligature fixtures etc.?

VA Response: Specifications are in the manuals cited in the statement of work. The contractor that we use for the camera and the card reader is M3TCorporation.com. The camera is a Panasonic Model # WV-SF332. The card reader is a XceedID Corporation Model # MT15. The contractor that we use for the nurse call station is Kratosdefense.com. The nurse call station is a Rauland Borg Model# R4KPC11 (pull cord) and R4KPB12 (push button)



4. What is the load rating for the existing HVAC unit?

VA Response: The manufacturers website will decipher what this model number means. HVAC unit shall be Trane model FCEB0401 JAAD0A20BF2M0000D3GEEF4000000000000 OR EQUIVALENT in piping or drainage entry and exit and be compatible with existing systems and architecture. Reference Trane Manual UNT-SVX07D-EN.

5. Is ceiling mounted patient lifts included in this project?

VA Response: There will be no need for any patient lift apparatus within the scope of this project.



6. Can you provide a manufacturer and model number for the existing security camera, so that we can match it with a compatible security camera?

VA Response: The camera is a Panasonic Model # WV-SF332. The contractor that we use for the camera and the card reader is M3TCorporation.com



7. Can you provide a "basis of design" type or model for the anti- ligature light fixtures?

VA Response: Existing 4 foot lighting is: Cooper model# FMR D 24 UNV 80/86 MF24-4FT. Efforts shall be made to acquire an LED equivalent even if available in a 2 x 2 equivalent in lumens.

8. Can you provide the name of the company who manages the nurse call system?

VA Response: The nurse call station is a Rauland Borg Model# R4KPC11 (pull cord) and R4KPB12 (push button). The contractor that we use for the nurse call station is Kratosdefense.com.

This amendment is hereby issued as follows

Patch and seal linoleum sheet flooring next to the existing HVAC unit and in the vicinity of the original entry door.



In picture number 2 of the photographs, change 107A closet door to 107B closet door



All existing drywall is impact resistant. All new drywall shall be impact resistant. All work shall be completely finished and painted to acceptable standards.



Walls are to have Acrovyn sheet goods covering the lower four feet of the wall to include the appropriate trim pieces throughout all sections of the main holding room. Wall covering is complete in the bathroom.

52.228-13 ALTERNATIVE PAYMENT PROTECTIONS (JULY 2000)

(a) The Contractor shall submit one of the following payment protections:

An Irrevocable letter of credit (ILC)

Certificate of Deposit



(b) The amount of the payment protection shall be 100 percent of the contract price.

(c) The submission of the payment protection is required within 10 days of contract award.

(d) The payment protection shall provide protection for the full contract performance period plus a one-year period.

(e) Except for escrow agreements and payment bonds, which provide their own protection procedures, the Contracting Officer is authorized to access funds under the payment protection when it has been alleged in writing by a supplier of labor or material that a nonpayment has occurred, and to withhold such funds pending resolution by administrative or judicial proceedings or mutual agreement of the parties.

(f) When a tripartite escrow agreement is used, the Contractor shall utilize only suppliers of labor and material that signed the escrow agreement.

AMENDMENT NO. 1 VA244-17-B-0178

This amendment is hereby issued as follows:

1. Correct the solicitation number on various pages of the solicitation to VA244-17-B-0178.

2. Include an additional site visit to be held, November 30, 2016, 10:00 a.m., VAMC Coatesville, PA. Contractors are to meet at building 19 S.

3. Correction: Door installation is to consist of a frame and a 4 foot and a 2 foot door within the frame. Two foot door is to have flush bolts through the floor and frame top. Door kit is to match existing Sally Port doors within that room.

4. Include photos.

The Coatesville Veterans Affairs Medical Center intends to issue a solicitation for Building 59A Holding Room, Project No. 542-16-202. This project will entail converting Room 107 (130 sq. ft.), 107A (39 sq. ft.), and 107B (9 sq. ft.) in Building 59A on the first floor into a patient holding room. The design of Rooms 107 and 107A will be based on the Office of Construction and Facilities Management guidelines for a holding room and patient bathroom in a Mental Health Clinic environment. Room 107B will continue to be a closet and will have the hinges removed and replaced with the piano hinge on the existing entry door which is slated for removal. The main doorway and frame into Room 107 will be removed its piano hinge is to be reused on the closet door. The opening will be properly sealed and a new doorway entrance will be installed opening into Room 105 with left hand piano hinge from room 107. All hardware must be tamper-proof and meet Mental Health Clinic safety requirements per Holding Rooms and match existing CVAMC hardware. All walls, floors, ceilings, electrical outlets, lightning, HVAC, plumbing, toiletry appliances, and security equipment and devices shall comply with the Office of Construction and Facilities Management guidelines for a holding room and patient bathroom in a Mental Health Clinic environment.



The Solicitation documents and drawings will be available for download from the website https://www.fbo.gov on or about November 21, 2016. Potential bidders are responsible for monitoring this site for the release of the solicitation package and any other pertinent information. The Government will not provide paper copies of the solicitation. Bidders shall be responsible for downloading their own copy of the solicitation package, drawings, and subsequent amendments, if any. Telephone, written, facsimile, or e-mail requests for the solicitation will NOT be honored. It is essential that interested parties register on the bidders mailing list at https://www.fbo.gov, "Interested Vendor List". Individual copies of the Bidder's Mailing List WILL NOT be prepared nor distributed by the Contracting Officer. The solicitation amendments issued to the IFB will be posted to the website and bidders are advised it is their responsibility to obtain and acknowledge all amendments. Paper copies of amendments will NOT be individually mailed.



The only mandatory pre-bid site visit will be held on or about November 28, 2016 and will be conducted with potential bidders in order to visually compare existing room with future project, visually confirm appliances and materials to be used in comparison with existing appliances and materials in order to avoid mistaken installation practices. The referenced materials listed will be cited and confirmed on site with all perspective bidders. Bidders will receive a briefing on parking, facilities, and patient safety prior to submitting a bid in the event that the proposal will be affected by hospital procedures. The exact date/time/location of the Site Visit will be specified in the solicitation. Note: All prospective bidders shall be registered and certified in VETBIZ as a Certified SDVOSB in order to be considered for award (Class Deviation VAAR 804.1102 VIP Database).



This acquisition is being issued in accordance with FAR Part 13 Simplified Acquisition Procedures. In accordance with VAAR 836.204, the magnitude of this construction project is between $25,000 and $100,000. The maximum time for completion of the project is sixty (60) days after receipt of Notice to Proceed.



Set-Aside Information: This opportunity is available only to Service-Disabled Veteran Owned Small Businesses. The bidder shall be small business under relevant North American Industrial Classification Standard (NAICS) code and shall be registered and current in the following websites: www.sam.gov and www.vetbiz.gov. Bidder shall ensure registrations in these websites are not expired and are correct, current and valid. The NAICS code for this project is 236220 Commercial and Institutional Building Construction and the small business size standard $36.5 Million.
