INSTRUCTIONS FOR VENDORS
1. This Request for Quotation (RFQ) is not an order. If your quotation is accepted WDFW will issue a Field Order or other order document that will indicate what items have been accepted. WDFW may accept or reject quotations by each item separately or as a whole.
2. Please provide a price quote via fax, email or regular mail for the item(s) specified in this RFQ.
3. Please complete the VENDOR QUOTATION, VENDOR INFORMATION and VENDOR CERTIFICATION EXECUTIVE ORDER 18-03 pages of this form to respond to this request. Failure to complete and use this form may result in rejection of a vendor’s quote.
4. Responses for the items indicated must be returned by no later than the date and time indicated.
5. Unless otherwise indicated, all prices offered must be FOB Destination, with all transportation and handling charges paid by the Vendor.
6. All quotations provided by the Vendor are good for a minimum of 60 calendar days from the Quotation Required By date/time on Page 1 of this document.
7. Vendors must be registered in the Statewide Vendor Payment System prior to submitting any requests for payment. Note: Polychlorinated biphenyls, commonly known as PCBs, have adverse effects on human health and the environment. Accordingly, the State of Washington through its procurement of goods is trying to minimize the purchase of products with PCBS and to incentive its vendors to sell products and products in packaging without them. WDFW respectfully requests any packaging be without color or dyes.
For Contracts and Awards with an Estimate Value under $150,000 The Purchaser is issuing this Solicitation pursuant to RCW 39.26.
