SCOPE OF WORK

Whiteriver Health Center Whiteriver, Arizona

Landscaping Improvements and Grounds Maintenance Service

1. SCOPE:

The Contractor shall provide landscaping improvements fo r the Whiteriver IHS Service Unit located at Whiteriver Arizona and at the Cibecue Clinic in Cibecue Arizona. This contract will be for a one-time initial landscape improvement project.

The Contractor is encouraged to visit the sites prior to the submission of a proposal. Failure to do so will not relieve the Contractor from fulfilling the requirements of the Contract. A pre-bid site visit will be scheduled.

The Contractor shall provide all labor, materials, tools, equipment, and transportation to perform the work as required by this contract.

2. GENERAL:

Maps are included with this Scope of Work.

2.1 There are three general areas where the performance of this contract will occur (see Figure 1 and 3):

2.1.1 Whiteriver Health Center Complex: includes areas around the Hospital building, the physical plant, Garrett Building and business support buildings (5 modulars) at the health center complex.

2.1.2 Medicine City Residential Area: includes a housing area adjacent to the Hospital with 50 single-family homes, two 4-plexes, a RV park, and three hospital support building. Mowing residential yards is not included in this contract except for those listed in section 3.2.1. The improvements in the residential area includes bu t i s no t l im i ted to drainage ditches, hillsides around the helipad, alleyways and other common areas.

2.1.3 The Cibecue Clinic includes approximately one acre of landscaped property.

2.2 The Contractor may encounter rocks and other items that are detrimental to mowing equipment.

Thoroughly walking the areas prior to beginning mowing activities is strongly encouraged; especially in alleyways and drainage ditches. The Contractor shall be responsible for repairing damage caused to vehicles or property because of negligence during maintenance activities.

2.3 All grass cuttings around the main hospital building, support buildings and medicine city residential area shall be bagged, hauled off si te and disposed of properly by contractor.

2.4 All weed and brush cuttings shall be hauled off site and disposed of properly by contractor.

2.5 Pick up and dispose of nuisance trash around the property when encountered. The intent of this provision is aesthetics. When the Contractor encounters nuisance trash or debris they are to remove and dispose of that trash and not simply just "mow through it" scattering it around. If the Contractor inadvertently mows through trash it shall be cleaned up before moving on to other areas.

2.6 Health Center and residential dumpsters or waste containers are not for the Contractor's use.

2.7 Services shall be in accordance with all Federal, State, and Tribal Government regulation, Environmental Protection Agency (EPA); Occupational Safety and Health Association (OSHA); US Department of Agriculture (USDA) and other applicable regulations. This includes ensuring patients, staff and visitors at the Health Center or in the residential areas are not harmed by grounds maintenance activities. The Contractor is responsible for providing personal protective equipment to employees engaged in landscaping activities. The Contractor shall ensure liability insurance is in good standing.

2. 8 The Contractor shall be responsible for acquiring all necessary Tribal work permits and meeting Tribal TERO requirements. Any costs associated with work permits and TERO shall be borne by the Contractor.

The TERO office is located on 209 E Fatco Road, Whiteriver, Arizona 85941. The telephone number for the Office is 928-338-1012.

3. LANDSCAPE IMPROVEMENTS:

Initial improvements shall include but are not limited to tree trimming, tree removal, stump removal, clearing and grubbing, mowing, weed control and installation of decorative landscape rock at designated areas.

3.1 Around the health care buildings:

3.1.1 Grass Mowing

All lawn type areas around the health care buildings shall be cut to a height of two to th ree i nches. In surrounding areas where native grasses are present the grass shall be cut to a height not exceeding six inches.

3.1.2 Edging

Edge along grass edges, sidewalks or areas where decorative landscape is present. The edges of lawn areas shall be trimmed and neatly edged.

3.1.3 Tree Removal

Remove and dispose of any dead trees, remove starter trees around the bases of existing trees or adjacent to buildings or structures. Remove trees in lawn areas that have not been intentionally planted. Remove trees from fence lines, or any that will interfere with fencing in the future.

3.1.4 Stump Removal

Remove and dispose of stumps identified on the site plan that accompanies this Scope of Work. The Contractor shall grind (or remove) the stumps to below ground level.

3.1.5 Tree Trimming

Trees along walkways, adjacent to sidewalks or in parking areas shall be trimmed so that no branches impeded pedestrian passage. Branches in those areas shall be a minimum of 7-feet above the passage surface. Tree trimming includes the removal of dead or damaged branches. Other trees, such as firs or pine trees shall be trimmed to ensure an attractive, healthy, and safe adult tree. Trees in contact with building structures shall be trimmed to prevent future contact with building. All tree trimmings shall be hauled off site and disposed of properly by contractor.

3.1.6 Hedges

Hedges shall be professionally shaped and trimmed. Care shall be taken in maintaining hedges adjacent to sidewalks and walk ways to ensure passage of pedestrians; i.e. no branches overhanging the sidewalks or walkways. Hedges adjacent to the buildings shall be trimmed such that they do not come in contact with the structure. This is primarily for the purpose of vector control. All hedge trimmings shall be hauled off site and disposed of properly by contractor.

3.1.7 Landscaped Areas

In areas where decorative landscape rock is present the Contractor shall remove and haul existing landscape rock to the RV Lot which is located on the Whiteriver Service Units Campus (just north of main Hospital) and install new decorative rock as shown on the plan included with this scope (Figure 2).

3.1.8 Weed Control

After the initial mowing, the Contractor shall provide chemical weed control along streets, parking areas and all fence lines only in the health center complex (hospital, physical plant, Garrett Building and 5 modulars). The weed control shall be applied a minimum of 2-feet from the curb or edge of the road and 2-feet on each side of the fences.

3.1.9 Power Washing

Clean and power-wash all sidewalks, the loading dock, and other concrete areas around the hospital.

The Contractor shall submit MSDA sheets for any chemicals used in the power washing procedure. The Facility Manager shall have approval authority for chemical use.

• Power cleaning of the loading dock, ambulance bay, and at the Emergency Department entrance shall be scheduled with the Facility Manager. The intent is to accomplish this task at a time when it will not negatively impact the E m e r g e n c y Department.

• Power cleaning shall be accomplished with high pressure water. The Contractor shall provide the hose, pressure pump, and other tools necessary to remove all deleterious material from the concrete surface.

• The contractor shall instruct employees to stop activities when ambulances are arriving or departing.

3.2 Medicine City residential areas

3.2.1 Mow front and back yards at units 24, 28, 42, 47, 50 (See Figure 4) and vacant properties (Check with facilities for list of current vacant properties, on the average there will be 7 vacant properties at any given time).

3.2.2 Rough-cut mow all alleyways and common areas along the streets. This includes hand trimming around structures such as clothe line posts, storage sheds, fire hydrants, etc., when needed.

3.2.3 Clean and grub all drainage ditches.

3.2.4 Remove dead trees and nuisance trees along streets, in fence lines, and close to buildings and structures.

3.2.5 Trim trees in yards and common areas to ensure attractive, healthy, and safe adult trees. Trees shall be trimmed so they will not cause damage to houses. Trees below utility lines shall be trimmed so they do not interfere with the utility lines. Tree trimming includes the removal of dead or damaged branches.

3.2.6 Bushes and hedges shall be trimmed to ensure a professionally maintained appearance.

3.2.7 The Contractor shall cut weeds and grass inside the propane tank enclosure.

3.2.8 All fence lines (yards and enclosures) shall be cleared of vegetation.

3.3 Cibecue

3.3.1 Grass Mowing

All lawn type areas around the clinic shall be cut to a height not exceeding two inches. In surrounding areas where native grasses are present the grass shall be cut to a height not exceeding six inches.

3.3.2 Edging

4. IRRIGATION SYSTEM

If irrigation system is damaged by contractor in performance of duties, the contractor is obligated to repair at own expense.

5. SCHEDULE

a. Contractor hours of operation at the Whiteriver Health Center and the Cibecue Clinic shall commence no earlier than 8:00 a.m. (MST) and continue no later than 5:00 p.m., Monday through Friday (excluding federally recognized holidays), or as agreed upon between the contractor and the Facility Manager.

b. The Contractor shall provide a detailed schedule for performance of grounds maintenance. It is important for the Contractor to maintain a consistent schedule of performance. This allows for notification of staff and residences of upcoming yard maintenance. Any changes to the set schedule must be submitted to the Facility Manager for approval a minimum of one week before the proposed changes.

6. PERIOD OF PERFORMANCE

a. All work associated with the initial landscaping included in this scope shall be completed within 90 days of the issuance of a Purchase Order.

7. Work Oversight and requirements prior to Commencement of Services.

a. Provide a listing of on-site personnel. The contractor shall ensure a supervisor or foreman is designated and remains on-site any time work is being performed.

b. The on-site supervisor or foreman shall be responsible for quality control. Coordination shall occur daily with the Government representative and on-site Contracting Officer’s Representative.

c. Provide proof of liability insurance shall be required for work; and shall be provided prior to commencement of work.

d. The contractor shall submit Mate r ia l Sa fe ty Da ta Sheets ( MSDA) sheets for any chemicals used in the power washing procedure and chemical weed control. The Facility Manager (Contracting Officer’s Representative) shall have approval authority for chemical use. Because this is a health care facility it is critical that the Contractor adhere to the application of only approved chemicals.
