The following have been added by full text:

The purpose of this amendment is as follow:

a) to change Block #28 of Standard Form 1449 to show that only one (1) copy is needed;

b) to change Addendum to 52.212-1, paragraph III "Documentation to be submitted", by adding:

- Two (2) Past Performance Questionnaire per 52.212-2 Evaluation and by correcting the office of the Point of Contact, and

c) to change FAR 52.212-2 "Evaluation -- Commercial Items (Oct 2014)", Factor III - Past Performance by replacing the sentence "As such, the offeror must submit two (2) completed forms on the Attachment titled EXPERIENCE QUESTIONNAIRE - in English - as specified below" with " As such, the offeror must submit two (2) completed forms on the Attachment titled PAST PERFORMANCE QUESTIONNAIRE - in English - as specified below".

The following have been modified:

ADDENDUM TO 52.212-1

ADDENDUM

I. PRICE, SOLICITATION OFFER AND AWARD DOCUMENT, AND CERTIFICATIONS /

REPRESENTATIONS.

A. Each offeror shall provide a lump sum price in Euro currency and without IVA tax for all contract line item(s) in the bid schedule.

B. An authorized official of the firm shall sign:

- the SF 1449;

- all the certifications/representations; and

- all provisions/clauses with a signature block.

C. To be awarded a government contract you MUST have DUNS (Dun & Bradstreet; tel: 02-28-455-379) and NCAGE (www.dlis.dla.mil/Forms/Form_AC135.asp) numbers and be registered in SAM (https://www.sam.gov).

II. DOCUMENTATION REQUIRED FOR THE DETERMINATION OF RESPONSIBILITY

The U.S. Government highly suggests that the offeror submit the following documentation/certification. This will be required of the winning offer to be satisfied that a prospective contractor currently meets the applicable standards in Federal Acquisition Regulation 9.104 – Standards. Although it is not mandatory, the Government suggests that the following documentation be submitted with the proposal:

CERTIFICATE OF THE CHAMBER OF COMMERCE (CERTIFICATO DI ISCRIZIONE ALLA CAMERA DI

COMMERCIO) (ORDINARIO)

The offeror shall provide with the proposal a copy certified and NOT older than two (2) months at the time set forth for the receipt of the offer of the Chamber of Commerce Certificate of Membership (Certificato di Iscrizione alla Camera di Commercio). The certificate must include the following statement otherwise the proposal will be rejected. "NULLA OSTA ai fini dell’articolo 10 della Legge 31 Maggio 1965, n. 575 e successive modificazioni. La presente certificazione e’ emessa dalla C.C.I.A.A. utilizzando il collegamento telematico con il sistema informativo utilizzato dalla Prefettura di Roma."

Proposal Copies - One (1) paper and one (1) electronic (CD-R or DVD-R) copy of the proposal shall be submitted.

This includes one original dated offer signed by an official fully authorized to bind the offeror. The CD-ROM or DVD shall be "Read Only" and all documents shall be submitted in Microsoft Office (version 2007 compatible) formats or Adobe Acrobat (PDF). No e-mail submissions will be accepted.

IV. No Site Visit will be held. Requests for Information (RFIs) will be accepted until 12:00 p.m. (noon, GMT+1, Rome), one week before the Soliciation Due in Date.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Factor i - Personnel Resume(s); Factor II - Past Experience;

Factor III - Past Performance and Factor IV - Price.

In order to be considered awardable, there must be an acceptable rating in every non- price factor.

The offeror is responsible for submitting a well-written proposal, with adequately detailed information, which clearly demonstrates compliance with the solicitation requirements.

TECHNICAL FACTOR I – PERSONNEL RESUME(S): This factor will be rated ACCEPTABLE or UNACCEPTABLE based on whether the offeror demonstrates fulfillment of the Performance Work Statement criteria from the Resume Template.

TECHNICAL FACTOR II – PAST EXPERIENCE: This factor will be rated ACCEPTABLE or UNACCEPTABLE based on whether the offeror demonstrates its experience by showing it has performed at least two (2) contracts that are comparable to the requirements of this solicitation (i.e. that in the past the offeror performed contracts that had similar tasks like the ones listed in the Performance Work Statement). The contracts must be current or have been completed within five (5) years from the date of this solicitation or the rating will result UNACCEPTABLE. Less than the required amount of experience will result UNACCEPTABLE.

Offerors must submit two (2) completed forms, in English language, of attachment entitled: EXPERIENCE QUESTIONNAIRE. The Government will consider this information, as well as information obtained from any other sources.

FACTOR III – PAST PERFORMANCE: The offeror must demonstrate it currently has a satisfactory performance record and that it meets the applicable standards of Federal Acquisition Regulation 9.104 – Standards. As such, the offeror must submit two (2) completed forms, in English language, on the Attachment titled PAST PERFORMANCE QUESTIONNAIRE as specified below.

An offeror will be assessed as being ACCEPTABLE or UNACCEPTABLE based on whether the past performance submitted for consideration is relevant and demonstrates successful past performance by the offeror, such as: no terminations for default; no ratings less than satisfactory by its customers/references; whether the quality of its work was satisfactory and completed in a timely manner; and whether the offeror demonstrated reasonableness and cooperation during performance of the contracts. Past failure to apply sufficient tenacity and perseverance to perform acceptably is strong evidence of unacceptable past performance. In addition, failure to meet the quality requirements of a contract is a significant factor to consider in determining unsatisfactory performance.

PAST PERFORMANCE EVALUATION RATINGS:

ACCEPTABLE - Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance is unknown.

UNACCEPTABLE - Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.

For Past Performance to be considered relevant, it must be similar in work, as described in the Performance Work Statement, must be accomplished in the last five (5) years from the date of this solicitation. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror shall be determined to have unknown past performance. Unknown shall be considered ACCEPTABLE.

The Past Performance is considered to be not relevant if it is not similar in work of the attached Performance Work Statement and if it is not accomplished in the last five (5) years from the date of this solicitation.

The Government will conduct a past performance assessment on all contracts submitted by the offeror on the Experience Questionnaire. The Government will consider the currency and relevance of the information, source of the information, context of the data, and general trends in contractor’s performance. The Government will consider the Experience Questionnaire information, as well as information obtained from any other sources.

FACTOR IV – PRICE: Price will be evaluated for fair and reasonableness and for unbalanced pricing.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

As part of price evaluation, the Government will evaluate its option to extend services (see FAR Clause 52.217-8) by adding six (6) months of the offeror's final option period price to the offeror's total price. Offerors are required only to price the base and option periods. Offerors shall not submit a price for the potential six month extension of services period. The Government may choose to exercise the Extension of Services at the end of any performance period (base or option periods), utilizing the rates of that performance period."

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
