1.3 Detailed Requirements: The Contractor shall complete the following tasks:

1.3.1 Development of Job Hazard Analysis (JHA). The Contractor shall coordinate with the

USACE technical POC to develop and provide an individual JHA for each project, including rescue plan, and host a pre-work safety briefing at each project site before inspection work commences. The contractor shall update the template provided in Attachment 2, 3, 4 and 5 and provide any additional information as necessary. The preliminary JHAs for the project shall be developed to the extent possible with information available prior to the inspection and shall be submitted to the USACE as a draft no less than seven (7) days prior to the inspection. The JHA shall be completed on site by the Contractor with USACE and facility POC input, reviewed and signed by all applicable parties at the pre-work safety briefing prior to the inspection. Prior to conducting any rope access work before each shift, the Contractor shall hold documented safety meetings, rescue pre-planning, and complete and provide Job Hazard Analysis forms.

1.3.2 Coordination. The Contractor shall ensure that the inspection is coordinated with the USACE technical POC. The Contractor’s Rope Access Supervisor shall participate in a teleconference

(approximately one (1) hour duration) with members of the inspection team and USACE project personnel at least one (1) week prior to the commencement of the inspections to discuss the rigging plan, equipment needs, and other logistics.

1.3.3 Safety supervision. The Level III Team Leader/Rope Access Supervisor shall provide safety supervision of SPRAT certified USACE inspectors performing visual inspection via rope access. The inspection team will include three (3) USACE inspectors. The safety supervision will include ensuring proper rigging strategies and all actions are in accordance with requirements outlined in SPRAT or IRATA industry standards and as outlined in the USACE

Safety Manual referenced herein. At a minimum, one onsite Contractor shall be certified in basic First Aid and CPR. Current certificates shall be shall be submitted to USACE with the

Contractor’s cost proposal.

1.3.4 Rigging/Access Plan and Equipment. The Contractor shall develop a comprehensive rigging and access plan to allow USACE inspectors to perform hands-on close-up visual inspection of all Tainter gate components, including but not limited to, the upstream skin plate, downstream gate horizontal girders, strut arms, bracing members, trunnion bearings, etc. Contractor shall coordinate with the USACE POC to ensure that sufficient amount of rope and rigging, which is to be provided by the Contractor, is available for all USACE inspectors to be on the ropes at any one time with extra equipment available to facilitate moving to the next inspection point in order to minimize the down time of the USACE inspection team. In addition, the Contractor is responsible for providing his own rope access equipment and any equipment necessary for a potential rescue scenario. The Contractor is also responsible for providing all rope and rigging equipment necessary to facilitate the NDT inspection. USACE inspectors are, at a minimum, SPRAT or IRATA Level I certified and will provide their own personal gear to include multi-purpose harness.

1.3.5 NDT. The Contractor shall perform non-destructive testing (magnetic particle, ultrasonic shearwave, and phased array) of welds. Rope access will be required to access the welds;

therefore the weld inspector shall also be certified at least as a Level I certified Rope Access

Technician. Details of NDT shall be coordinated with the USACE technical POC prior to work.

The general locations of the welds to be tested are on the downstream strut arms at the bracing connections, trunnion to strut arm welds, strut arms to girder flange welds, and at the strut arm splices and on the downstream girders at the girder splices. The welds include full penetration butt/groove welds. See Attachment 6 for general location of NDT. The exact NDT procedure for each location shall be determined by the weld inspector and mutually agreed upon by

USACE POC prior to testing. Additional locations may be identified on site during the inspection.

1.3.5.1 Ultrasonic Testing. All welds shall be inspected using NDT methods in accordance with AWS

D1.1:2015. The NDT technician performing Ultrasonic Testing (UT) shall be qualified to

ASNT Level II or III as specified in subsection 6.14.3. ASNT certifications shall be submitted to USACE with the Contractor’s cost proposal. Acceptability of welds shall be in accordance with AWS D1.1:2015, Table 6.1 and 6.3 for cyclically loaded structures. NDT test reports shall identify the location of flaws that do not meet the acceptance criteria and the actual location of flaws shall be marked on the tested welds using indelible ink visible for photos.

The NDT technician shall photograph ALL tested locations that contain defects. The photos shall clearly show the location of defects and shall identify the gate number and location of the defect on the structure. Location nomenclature shall be coordinated with USACE POC.

Written report shall provide level of failure, including reference level, sound path, indication level, location, etc. and shall include photos of all tested locations that contain defects. Photos shall be clearly labeled for location and cross referenced to the report descriptions of weld defects. Simply providing a "pass/no pass" documentation report of NDT results will not be acceptable. Results must be sufficient to allow analysis of weld deficiency and location of flaw for structural weld repairs. Draft written reports of weld testing shall be provided to the

USACE technical POC on AWS Form L-11 (see Attachment 2) or similar form at the completion of each day of inspection. Final, signed reports shall be provided to the technical

POC within seven (7) calendar days of the completion of the inspection trip. NDT technician shall be available to revise the final NDT report following USACE review and comment.
