3 Pre-Bid Site Visit There will be a scheduled pre-bid site visit for prospective bidders. The date and time of the visit will be posted on FBO. The location of the visit will be William B. Hoyt II Visitor Center, 6103 Visitor Center Road, Mount Morris Dam, Mount Morris, New York 14510. Prospective bidders are urged to attend to gain information that will be necessary in the preparation of bid submittals. Due to maintenance, operation and security requirements, unscheduled visits may not be honored. Following a visit to the project site, a question and answer session will be held. During this session, prospective bidders are asked to direct all questions concerning the project to the Buffalo District Contracting Office (DCO).

5 Material/Equipment Specifications
5.1 Equipment/Conduit Mounting Hardware
All hardware used to mount equipment and conduit shall be Type 303 Stainless Steel.
5.2 Enclosures
As certain areas within the dam are damp/wet environments, when specified under certain bid line items, all equipment and hardware installed under that bid line item shall be rated to IP66 outdoor use. Any enclosures shall be rated to a minimum IP66/NEMA 4X.
Enclosure heaters shall also be installed in all enclosures to prolong equipment life and prevent condensation on electronics. Heaters shall be thermostatically controlled. Include desiccant packs in each enclosure also.
5.3 Cable Connectors
All cable connectors shall be wet rated.
5.4 Uninterruptable Power Supplies (UPS)
The entire system shall be designed and installed to run in the event of a power outage for a minimum of one hour.
Install UPS as necessary. Include surge protection. The UPS inside the head unit shall be rack mounted.
6 Detailed Description of Work
6.1 Detailed Description of Work – Base Bid Item(s)
6.1.1 Base Bid Item 1 – Obtain Approval from Cellular Carriers Mount Morris Dam is a flood control project located in Mount Morris, New York. The dam is of mass concrete construction, approximately 225 feet in height and 1,250 feet in maximum length, located at the base of the Genesee River gorge. Work under this Statement of Work shall consist of the contractor designing, installing, and configuring a Distributed Antenna System (DAS) for re-broadcasting of cellular frequencies inside the Mount Morris Dam.
As this is a complex project in an intricate facility, the contractor is strongly urged to attend the pre-bid site visit in order to better assemble a more accurate bid. Although fair cell phone voice and data signals are attainable at the location the outdoor antenna is specified to be installed, the government will not list signal strengths per carriers and it is strongly recommended the contractor verify signal strengths at the pre-bid site visit. No exceptions will be granted to contractors who bid on the project without fully understanding the outdoor/indoor RF qualities of the site.
This base bid line item is for obtaining approval and Base Bid Line Item 2 is for design of the system and installation of the headend. Approval shall be obtained through the either the prime contractor or DAS equipment manufacturer to the cellular carriers. The contractor shall install the system to certain areas within the dam under Base Bid Item’s 3 and 4. Coverage to other areas may be awarded under optional bid line items, and may be awarded up to one-hundred (180) days past the initial award. Refer to Section 7.2 for more information. There is no guarantee the government will award any optional line items.
The system shall be designed to operate on the Verizon Wireless and AT&T cellular networks, with the requirement that it may be easily upgraded to support T-Mobile and Sprint in the future with only limited hardware changes at the headend.
Per Federal Communications Commission (FCC) Report and Order FCC 13-21, the contractor may be required to obtain a license from the FCC and authorization from cellular carriers. Prior to work beginning, the contractor is required to obtain written authorization from both AT&T and Verizon Wireless for permissions to re-broadcast licensed frequencies on all applicable bands utilized in the region. The request to the carriers shall come from the DAS equipment manufacturer. The hardware shall support capabilities to implement the following bands, including all standards within said band: 750 MHz, 800 MHz, 850 MHz, 1900 MHz, 1700 MHz, 1900 MHz, and 2100 MHz. The hardware shall also support adding T-Mobile and Sprint carriers into the system in the future, outside of the scope of this contract. All antennas inside the dam shall be capable of all frequencies and standards within those frequency bands. A wireless survey and design for the system shall be completed by an experienced Radio Frequency (RF) Engineer whom holds a certification by the DAS manufacturer for the product line to be installed. Include product datasheets for the DAS manufacturer as a bid submittal, which are contractually binding. Failure to include or fully complete this submittal along with the bid may be grounds for bid qualification and the government may move onto the next lowest bidder.
All electronic equipment for the DAS system shall be manufactured by CommScope, Corning, JMA Wireless, or approved equal. Or approved equal includes DAS manufacturers whom have product lines meeting the requirement in this Statement of Work. The contractor shall include along with the pre-bid submittal the manufacturer and product line of DAS system which shall be installed.
Prior to submission of the formal request to both carriers or application for a license or registration with the FCC, the contractor shall submit in a pre-work submittal all documents which shall be submitted to the carriers and the FCC, including but not limited to DAS manufacturer datasheets for products to potentially be installed under any optional bid items, and all paperwork which shall be submitted for the formal request. The contractor shall not submit any documents to the FCC or cellular carriers until approved by the government’s project manager.
6.1.2 Base Bid Item 2 – Design and Install Distributed Antenna System Head Unit As the dam is of mass concrete construction, all floors, walls, ceilings, partitions, etc., should be assumed to block all RF frequencies from penetrating the facility, with the exception of the main lobby and office on Level 7. There are several galleries in the dam, better described as narrow corridors several hundred feet in length, with a typical cross section of 5 x 7 feet.
This Statement of Work consists of several line items, used to extend coverage to various parts of the dam as base or optional bid items. Under this line item, the system shall have a headend unit installed which is capable of expansion to support all remaining line items. This base unit shall be installed in the kiosk at the north side of the dam. As this is an unconditioned space, the headend shall be installed in an enclosed server cabinet with heating and cooling built-in.
The contractor shall install the base unit for DAS cellular phone coverage, which shall allow service inside the dam to at minimum AT&T and Verizon, as stated above in Base Bid Line Item 1. The hardware shall also support adding T-Mobile and Sprint carriers into the system in the future, outside of the scope of this contract. Table #1 below lists the following areas of the dam which may have coverage awarded under either a base or an optional bid item, and are shown in the elevation drawing of the dam in Attachment #1. The areas are either classified as an open space, a linear gallery, or a vertical shaft. Antenna coverage to these areas shall not be installed under this line item. All areas listed in Table #1, other than the last row, are either in a base or option line item. It is up to the discretion of the government for which, if any, line items are awarded.

The system shall comply with the FCC 13-21 order and shall use only certified equipment which shall be installed by a qualified installer. Prior to ordering supplies and materials, the contractor is required to get written approval from all carriers for authority to broadcast on the applicable signals, as specified in Base Bid Item 1.
The contractor shall submit at minimum a bi-weekly status updates throughout the life of the design phase of this line item. The contractor will be provided with as-built drawings of the facility, similar to Attachment #1, for which the contractor shall locate all equipment to be installed. The contractor shall submit a 50%, and 90% design review. The contractor shall provide the government’s project manager with the reviews, allowing comment up to seven (7) calendar days after receipt of the submission.
The contractor shall place an outdoor directional antenna at the top of the kiosk, which is only accessible by ladder from the dam’s parapet, on a pole if necessary, aimed at the nearest cell tower. The system shall not be hardwired to any carrier’s base stations.
The contractor shall install a system which gives a minimum -90 dB signal strength throughout any areas installed under optional bid line items. The contractor is required to design the system and place antennas at spacing to meet this requirement. A walkthrough inspection shall be completed after each level or area is added to confirm these minimum requirements. If the minimum signal strength is not met, the contractor is required to modify the system to meet compliance. As the system is used for emergency 911 communications, no exceptions shall be granted.

All equipment and hardware installed under this line item, other than within the elevator shaft, shall be rated to IP66 outdoor use. Any enclosures shall be rated to a minimum IP66/NEMA 4X. All wiring and connectors shall be wet rated.
7 Project Information
7.1 Basis for Award
The government shall award the contract to the offeror who provides the lowest priced technically acceptable offer.
The evaluated price shall include the base work plus all options. In order to be considered technically acceptable, the contractor shall:
• Have a minimum of (5) years experience on projects of a similar nature. The contractor shall supply the names, locations, and client contact information for two (2) projects of similar size and scope that have been accomplished within the previous five (5) years as part of the bid submittals. Similar projects include the design and installation of a commercial or enterprise size DAS.
7.1.1 Award Period for Options
The government reserves the right to award any and/or all optional bid items within one-hundred eighty (180) days following the initial award.
7.2 Period of Performance
The project completion date is set at one-hundred eighty (180) days following the initial award of the contract for the base bid line items. For any and/or all optional bid items awarded within one-hundred eighty (180) days following the initial award, the completion date for said bid items is set at ninety (90) days following the award of the bid item, or the initial period of performance, whichever is later. In the event the contractor is unable to meet the project completion date, requests for extensions must be made in writing. The request must explain, in detail, why the original project completion date will not be met. The request shall be sent to the DCO along with an informational copy to the Project Manager (PM). The request shall be made as soon as possible, but not later than ten (10) days prior to the original completion date. This request shall not negate penalties for liquidated damages unless an extension is granted by the DCO in writing.
7.2.1 Pre-Construction Submittals
All pre-construction submittals, as stated in Section 20.2 of this SOW, shall be provided to the government within fourteen (14) days following the award of the contract. The contractor shall allow up to twenty-one (21) days from time of receipt for government review of submittals. Work may not begin until government approval of all submittals required in Section 20.2 of this Statement of Work.
7.2.2 Design Submittals
All design submittals, as stated in Section 20.3, including the design and layout of all hardware/equipment, electrical schematics, etc., shall be provided to the government within sixty (60) days following the award of the contract. The contractor shall allow up to twenty-one (21) days from time of receipt for government review of submittals. Work may not begin until government approval of all submittals required in Section 20.3 of this Statement of Work.
7.2.2.1 Project Timeline
As part of the pre-construction submittals, the contractor shall supply a timeline or schedule for accomplishing the work showing the following minimum milestones and completion dates: design start date, 50% design, 90% design, 100% design, carrier approval, equipment order, equipment delivery, construction start, headend installation, construction completion.
7.3 Pre-Construction Meeting
Following the award of the contract and review and acceptance of the contractor’s pre-construction submittals, the prime contractor must attend a mandatory pre-work conference. The meeting shall be scheduled by the government, and conducted on-site, at the Mount Morris Dam, at a time and date to be announced following the award of the contract.
7.4 Work Schedule
All scheduled work is to be performed Monday through Friday 0800-1630 hours, excluding federal holidays. If the contractor desires to work beyond the previously stated hours and days, written permission to do so shall be obtained from the government. Schedule changes shall be requested at minimum 24 hours in advance, unless there is an emergency situation.
7.5 Material/Equipment Information
Contractor shall provide all labor, tools, ladders, scaffolding, equipment, supplies and materials needed to complete the work described herein, unless otherwise noted herein that any of these items will be provided by the government.
7.6 Dimension/Quantity Verification
Contractor shall independently measure and verify all dimensions and quantities given herein prior to ordering materials and supplies for the project. This information shall be provided to the government QA person as part of the pre-work submittals.
7.7 Material Storage
If temporary storage of project material is requested, the contractor shall make a written request to the government’s QA person and receive written approval from the government. If material is found to be hazardous, flammable, combustible, etc, the contractor will be required to provide the appropriate hazardous material storage locker. Storage locations will be discussed at the pre-bid site visit and during the pre-con meeting.
7.8 Site Cleanup
All excess, waste, and removed material related to this project shall become the contractor’s property and shall be promptly removed from the site and disposed of or reused/recycled in accordance with applicable local, state, and federal regulations. Documentation describing disposal of material shall be furnished to the government as part of the project closeout files. The contractor shall not dispose of any project generated waste in government receptacles or on government grounds.
8 Quality Assurance (QA)
8.1 Overview
8.1.1 Government Project Manager (PM)
The government has appointed a Project Manager (PM) for this project. The PM shall ensure all questions of a technical nature concerning the contractual work are answered.
8.1.2 Government Quality Assurance (QA)
The government will appoint a Quality Assurance (QA) person to ensure all work is accomplished in accordance with contractual documents. The QA person for this contract will be identified after the contract is awarded. The QA person serves as the liaison between the contractor, PM, and the DCO. The QA person does not have the authority to modify contractual agreements.
8.1.3 Government Inspections
Contractor shall allow inspection by the government PM, QA person, DCO personnel, and District Safety and Health personnel, at any time during the life cycle of the project. The contractor shall provide safe access for government personnel visiting the project.
8.1.4 Product Data Sheets
The contractor shall provide to the government product data sheets from the manufacturer for all materials used on this job, as part of the pre-construction submittals. All materials shall be delivered to the project site in new condition, in their original containers and wrapping material, with labels clearly showing all product information.
9 Communication
During the execution of the contract, the PM shall receive weekly status reports from the contractor on issues such as milestones met and/or missed, and issues standing in the way of project completion. The DCO shall be the communication hub for the government QA Person, PM, and the contractor regarding clarification of contractual issues. By adhering to the established lines of communication, it will reduce or avoid generation of duplicate requests for information, which may inadvertently result the government or the contractor to the acceptance of contractual changes without official approval by the DCO.
10 Repair/Restoration of Government Property Contractor, at no expense to the government, is required to protect all government facilities, grounds, equipment, sidewalks, and pavement during construction. All government structures, grounds, and equipment disturbed by contractor during the performance of work shall be returned to a pre-work condition prior to request of final payment.
Damages to government property that were caused by contractor and/or sub-contractor will be repaired at no expense to the government by the prime contractor.
11 Work Stoppage, Contract Cancellation Actions The Mount Morris Dam Park Manager, PM, or government QA person may issue temporary “Stop Work” notification if they find activities or job site conditions are in violation of Federal, Department of Defense, State or local health, safety and/or environmental standards, laws and regulations. The District Safety Officer (DSO), the DCO, and the PM shall be contacted immediately after a Stop Work notification order is issued or rescinded. Violation of health, safety or environmental regulations committed by the contractor shall not be grounds for compensation by the government to the contractor. Refer to contractual documents provided by the District Contracting Office regarding additional work stoppage and/or contract cancellation actions.
12 Material Change Orders Once equipment and materials are approved and accepted by the government, the contractor shall not substitute equipment or materials without providing a detailed explanation for the substitution. Changes in materials will require full justification and approval from the DCO. Modifications to contractual documents must be reviewed by the contractor, government QA person and PM, and approved by the government’s DCO. Requests for modifications must be in writing explaining the reasons for the modification, detailed description of the modification and what the benefits are and an estimated cost broken down by materials, labor and equipment.
13 Liquidated Damages Liquidated damages of $150 per work day will be assessed if the work is not completed within the time period described in Section 7.2 of this Statement of Work. Liquidated damages shall be assessed beginning the day following the end of the initial period.
14 Inspection/Testing
14.1 Pre-Final/Final Inspection
Upon completion of the work, the government will perform a pre-final inspection to verify that the project is complete. A government pre-final punch list may be developed as a result of this inspection. If contractual requirements are not met, this list shall be provided to the DCO, who will bring them to the attention of the contractor.
The contractor’s QC supervisor shall ensure that all items on this list have been corrected in a timely manner before notifying the government, so that a final inspection with the government and contractor can be scheduled. The contractor’s QC supervisor, PM, DCO representative, and QA person shall be in attendance at the final acceptance inspection.
15 Completion The contract will not be deemed fulfilled until all submittals, products, services, equipment, material, installation, construction work, and final inspection is provided by the contractor under the terms of the contractual documents and found satisfactory to the DCO.
16 Payment The project will be paid for on a lump sum basis, the price for which includes all labor, materials, equipment, mobilization/demobilization, disposal, and other costs required to complete the work. Partial payment(s) may be made; this will be discussed at the pre-construction meeting.
17 Warranty
17.1 Warranty Period
Contractor shall, at a minimum, warranty all workmanship, equipment, and materials against deficiencies for a period of three (3) year(s) from the date of acceptance of the work. The contractor shall also provide the manufacturer’s warranty for newly installed equipment and/or materials produced by said manufacturer. The period of warranty shall commence at time of government's acceptance of the work. If within the warranty period the installed equipment/materials incur breakdowns, excessive wear, or become unreliable, the costs to repair or replace installed equipment and materials shall be borne by the prime contractor. The warranty shall bear the name of the Buffalo District, U.S. Army Corps of Engineers as the recipient of the warranty and display the period of coverage. All warranties shall be submitted prior to request of final payment on this contract.
17.2 Repairs
When repairs are required because of defective workmanship during the warranty period, contractor shall make such repairs within 72 hours of notification. When repairs are not performed within the specified time, emergency repairs performed by others will not void the warranty and contractor shall reimburse the government for the cost of said repairs. Contractor shall not bill the government for costs of repair work and delay of payment due to repair work executed to resolve defective workmanship and/or problems with materials used and services provided.
18 Safety
Refer to Attachment #4 for the government safety requirements.
19 Security
19.1 Access and General Protection/Security Policy and Procedures All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures, which will be provided by a government representative. The contractor shall also provide all information required for background checks or background investigation and to meet installation/facility access requirements to be accomplished by the Army Corps of Engineers (USACE) Buffalo District Emergency Management Office. Contractor workforce must comply with all personal identity verification requirements as directed by the Department of Defense (DOD), Headquarters Department of the Army (HQDA) and/or USACE Buffalo District policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any installation or facility change, the government may require changes in contractor security matters or processes.
19.1.1 Candidate Pre-Screening
The contractor must pre-screen candidates using the E-verify Program (http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements. The vendor must ensure that the candidate has two valid forms of government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible candidates must be provided to the COR no later than 3 business days after the initial contract award.
19.1.2 Background Investigation
For all prime and subcontractor employees working at Mount Morris Dam, the following Personally Identifiable Information (PII) shall be provided for the US Army Corps of Engineers to perform a limited background check:
Complete name, social security number, driver’s license number, date of birth, place of birth, and current address of residence. This information is mandatory before any workers will be allowed to enter the facility. The contractor shall allow up to twenty-one (21) days from time of receipt for government completion of background checks. Work may not begin until government clearance of employees is given to the prime contractor.
19.1.3 Suspicious Activity Reporting Training
The contractor and all associated sub-contractors shall receive a brief/training provided by the PM on the local suspicious activity reporting program. This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the project manager, security representative or law enforcement entity. This training shall be completed within 30 calendar days of contract award and within 30 calendar days of new employees commencing performance with the results reported to the PM no later than five calendar days after the completion of the training.
19.2 Start of Work
Contractor shall notify the government of their planned arrival at the project to start work at least seven (7) calendar days in advance. Contractor shall submit a weekly work schedule thereafter by no later than the Friday before the coming work week.
19.3 Arrival/Departure
Contractor is required to notify the government QA person Park Manager of their arrival to and departure from the project site. A similar notification is required if there shall be any deviation or cancellation of a scheduled work day during the life of the project.
19.4 Video/Camera Equipment
Contractor shall not transport or use video or camera equipment while working at Mount Morris Dam unless permission to do so is requested through the Park Manager and approval is granted by the Buffalo District Security Officer.
20 Submittals
20.1 Bid Submittals
The following submittals are required from the contractor under this Scope of Work to be submitted with the bid proposal:
• Complete Bid Schedule with prices for all base and optional items. (See Attached)
• Contractor experience as stated in Section 7.1.
• Proposed DAS manufacturer and product datasheets
• Other documentation as required by the DCO:
o Representations and certifications
20.2 Pre-Construction Submittals
All submittals are required as per the timeline described in Section 7.2 of this SOW:
• Contractor timeline showing start and completion of contractual work
• Contractor employees Personally Identifiable Information (PII)
• Accident Prevention Plan (APP) with Activity Hazard Analysis (AHA’s): Safety and health submittals as described in Attachment #3 of this Scope of Work will be submitted to the DCO. Safety and health submittals will be reviewed and accepted in writing by the DSO prior to commencement of work.
• Completed design drawings
• All materials/equipment data sheets provided under the contract, including but not limited to:
o All DAS equipment to be installed, cabling/wiring
20.3 Design Submittals
The 100% design submittal shall be provided to the government no later than sixty (60) days past the initial award.
20.4 Project Completion Submittals
All submittals are required as per the timeline described in Section 7.2 of this SOW:
• All required submittals required under this contract.
• Manufacturer warranty for newly installed equipment and material.
• Contractor warranty for all work performed under this contract.
• Documentation for disposal of project waste material.
• As-built design drawings (Attachment #7)
• Monthly Man-Hour Exposure Report (Attachment #6)

1.1 REFERENCES

1.2 DEFINITIONS
c. Medical Treatment. Treatment administered by a physician or by registered professional personnel under the standing orders of a physician. Medical treatment does not include first aid treatment even through provided by a physician or registered personnel.
e. Recordable Injuries or Illnesses. Any work-related injury or illness that results in:
(1) Death, regardless of the time between the injury and death, or the length of the illness;
(2) Days away from work (any time lost after day of injury/illness onset);
(3) Restricted work;
(4) Transfer to another job;
(5) Medical treatment beyond first aid;
(6) Loss of consciousness; or
(7) A significant injury or illness diagnosed by a physician or other licensed health care professional, even if it did not result in
(1) through (6) above.

1.3 SUBMITTALS

1.4 REGULATORY REQUIREMENTS
In addition to the detailed requirements included in the provisions of this contract, comply with the most recent edition of USACE EM 385-1-1, and the following federal, state, and local laws, ordinances, criteria, rules and regulations . Submit matters of interpretation of standards to the appropriate administrative agency for resolution before starting work.
Where the requirements of this specification, applicable laws, criteria, ordinances, regulations, and referenced documents vary, the most stringent requirements govern.
1.5 SITE QUALIFICATIONS, DUTIES AND MEETINGS
1.5.1 Personnel Qualifications
1.5.1.1 Site Safety and Health Officer (SSHO)
The SSHO must meet the requirements of EM 385-1-1 section 1 and ensure that the requirements of 29 CFR 1926.16 are met for the project. Provide a Safety oversight team that includes a minimum of one (1) person at each project site to function as the Site Safety and Health Officer (SSHO). The SSHO or an equally-qualified Designated Representative/alternate shall be at the work site at all times to implement and administer the Contractor's safety program and government-accepted Accident Prevention Plan. The SSHO's training, experience, and qualifications shall be as required by EM 385-1-1 paragraph 01.A.17, entitled SITE SAFETY AND HEALTH OFFICER (SSHO), and all associated sub-paragraphs.
A Competent Person shall be provided for all of the hazards identified in the Contractor's Safety and Health Program in accordance with the accepted Accident Prevention Plan, and shall be on-site at all times when the work that presents the hazards associated with their professional expertise is being performed. Provide the credentials of the Competent Persons(s) to the the Contracting Officer for acceptance in consultation with the Safety Office.
1.5.1.1.1 Additional SSHO Requirements
The SSHO shall have completed the 10 hour Construction Safety Hazard Awareness Training Course for Contractors.
1.5.1.1.2 Contractor Quality Control (QC) Person:
The Contractor Quality Control Person can be the SSHO on this project.
1.5.2 Personnel Duties
1.5.2.1 Site Safety and Health Officer (SSHO)
The SSHO shall:
a. Conduct daily safety and health inspections and maintain a written log
SECTION 01 35 26 Page 2 which includes area/operation inspected, date of inspection, identified hazards, recommended corrective actions, estimated and actual dates of corrections. Attach safety inspection logs to the Contractors' daily [production][quality control] report.
b. Conduct mishap investigations and complete required reports. Maintain the OSHA Form 300 and Daily Production reports for prime and sub-contractors.
c. Maintain applicable safety reference material on the job site.
d. Attend the pre-construction conference, pre-work meetings including preparatory inspection meeting, and periodic in-progress meetings.
e. Implement and enforce accepted AHAs.
f. Maintain a safety and health deficiency tracking system that monitors outstanding deficiencies until resolution. Post a list of unresolved safety and health deficiencies on the safety bulletin board.
g. Ensure sub-contractor compliance with safety and health requirements.
h. Maintain a list of hazardous chemicals on site and their material safety data sheets.
i. Failure to perform the above duties will result in dismissal of the superintendent, QC Manager, and/or SSHO, and a project work stoppage.
The project work stoppage will remain in effect pending approval of a suitable replacement.
1.5.3 Meetings
1.5.3.1 Preconstruction Conference
a. Contractor representatives who have a responsibility or significant role in accident prevention on the project shall attend the preconstruction conference. This includes the project superintendent, site safety and health officer, quality control supervisor, or any other assigned safety and health professionals who participated in the development of theActivity Hazard Analyses (AHAs).
b. Deficiencies in the submitted APP will be brought to the attention of the Contractor at the preconstruction conference, and the Contractor shall revise the plan to correct deficiencies and re-submit it for acceptance. Do not begin work until there is an accepted APP.
1.5.3.2 Safety Meetings
Conduct and document meetings as required by EM 385-1-1. The first meeting will be held on site at the project. The USACE PM, QA, prime and sub-contractor personnel and appropriate USACE personnel shall be in attendance. Minutes of the meeting showing contract title, signatures of attendees and a list of topics discussed shall be prepared by the USACE PM and a copy provided to the District Safety Officer and kept in the project files. Additional meetings shall be held as required by the USACE or the Contractor

1.6 ACCIDENT PREVENTION PLAN (APP)/ACTIVITY HAZARD ANALYSIS (AHA)
For this project, an Activity Hazard Analysis (AHA) may serve as the Accident Prevention Plan. The AHA be prepared in accordance with EM385-1-1 Section 01.A.14 by the contractor. The USACE PM and District Safety Officer will provide guidance in the preparation of the AHA. The AHA shall be prepared by a qualified person and be project specific. The AHA shall address the contents of the Pre-liminary Hazard List (PHL), also prepared by the Contractor. The PHL shall be submitted to the USACE along with the AHA. Prepare the AHA and PHL in accordance with the format and requirements of USACE EM 385-1-1 and as supplemented by the District Safety Officer. Specific requirements for PHL will address the elements described following this paragraph.
